    Notes: Summary A 31 year old female was admitted with a one month history of left foot drop and diplopia. CT of the posterior fossa revealed gross displacement of the 4th ventricle by a large non-enhancing cerebellar mass but gave no indication of its nature. MRI sequences showed two masses within the left cerebellar hemisphere extending into the vermis and demonstrated a very unusual septation within the lesion. In addition both cerebellar tonsils were displaced below the foramen magnum and there was a syrinx extending from C 2 to the conus. At operation a demarcated lesion was excised from the apparently normal surrounding cerebellar tissue and proved histologically to be Lhermitte-Duclos disease. We believe that these are the first diagnostic magentic resonance images of this condition.

    Notes: Summary Primary intracranial plasmocytomas are rare in neurosurgical practice. The accessible literature indicates that they fall into two groups: 1. Extramedullary plasmocytoma originating from the dura and 2. Plasmocytoma originating from the cranial vault. Four patients with primary lesions of the cranial vault were surgically treated and common characteristic clinical and radiological features discussed. Although the immediate outcome of treatment was excellent in all cases, the disemination of disease occurred in two patients within the period of one year. It is therefore essential that the patients with so called solitary cranial plasmacytoma were rescreened for systemic myelomatosis at regular intervals.

    Notes: Summary Gadolinium (Gd)-DTPA enhanced magnetic resonance imaging (MRI) was performed in 15 systemic lupus. erythematosus patients with past (12) or present (3) features suggesting central nervous system (CNS) involvement. Symptomatic Gd-DTPA enhancing lesions were seen in 2 patients, and immunosuppressive treatment was associated with a rapid reversal of enhancement. The pattern of enhancement was different from that usually seen in multiple sclerosis. Gd-DTPA enhanced MRI may sometimes be useful in demonstrating the activity of CNS lupus.

    Notes: Summary Sixty-nine patients with cervical spondylotic myelopathy (CSM), radiculopathy (CSR), or both (CSMR) were studied with computed tomography (CT). Computer-assisted myelography (CAM) accurately determines the site and nature of spondylotic protrusions and provides good visualisation of the subarachnoid space and cord deformities even in areas with dilute metrizamide. However, excessive vertebral movement and bulging ligamenta flava with their effects on cord deformity, so easily visualised in myelograms, are completely or partially missed. In the assessment of CSM, metrizamide myelography (MM) followed by CAM should be performed, particularly when the myelographic images are unsatisfactory due to contrast dilution or blockage, when cord compression cannot be ascertained with MM and when cord atrophy is suspected. In CSR, the diagnostic information from MM and CAM is comparable. The diagnostic criteria in CAM are, however, less direct and since MM is adequate in uncomplicated cases, CAM is generally not necessary. The APD, APD/TD ratio, area and circularity are sensitive indices of cord deformity and the first two should be used more often to assist visual assessment of cord deformity. The relation between cord parameters and treatment response is better reflected in CSM cases managed conservatively and the results suggest that the degree of cord deformity is helpful in determining the outcome and hence the choice between surgical and conservative treatment. In plain CT, the osteophytes and calcified discs are adequately visualised and canal dimensions measured with accuracy, but the cervical cord and roots cannot be properly assessed and the diagnosis of CSM or CSR cannot be ascertained. At present, its role in cervical spondylosis is therefore limited.

    Notes: Summary The manifestations of angiomas on computed axial tomography (CAT) are described. Diagnostic changes were present in 66% and a focal abnormality, which was frequently suggestive of the diagnosis, in a further 27.5% of a consecutive series of 41 patients. It failed to show some focal abnormality in only two cases in which intravenous contrast was not given and in one case with a dural angiomatous malformation. The value of CAT in elucidating the nature of the complications of arteriovenous malformations is emphasized.

    Notes: Summary A case in which a remarkable seriies of CT studies demonstrated a dermoid cyst is reported. There was leakage of fat into the ventricular system which caused hydrocephalus due to adhesions partly obstructing the fourth ventricular outlets. The latter was confirmed by a ventriculogram using Myodil which dissolved in, or combined with, the fat produced by the tumour. This resulted in a high attenuation substance of lower specific gravity than CSF, which appeared to cause acute ventriculitis.

    Notes: Summary The clinical and computed tomographic (CT) findings in 11 proven cases of mitochondrial cytopathy (mitochondrial myopathy, Kearns Sayre syndrome, ophthalmoplegia plus) were studied. The CT changes included focal low density lesions in the bassal ganglia and white matter and atrophy which could be slight or diffuse and severe. Calcification has been described in the basal ganglia, but did not occur in our series. Serial CT showed progression of the abnormalities. The differential diagnosis is discussed.

    Notes: Summary Three patients with biopsy-proven acute haemorrhagic leukoencephalitis are described. All presented with an abrupt onset of focal neurological signs, pyrexia and leukocytosis. In two cases there was disturbance of consciousness. Computed tomography in each case revealed low density virtually confined to the white matter of the cerebral hemisphere; in one case there was central enhancement within the low density. The combined clinical and CT signs should strongly suggest the diagnosis.
