Notes: Abstract The diagnostic value of the nasal biopsy in the early diagnosis of Wegener's granulomatosis and its value in prognosis were examined in 11 patients with a clinicopathological diagnosis of the disease. The vascular lesions found included microabscess in the vascular walls in 82%, leukocytoclastic capillaritis in 73%, fibrinoid necrosis of blood vessels in 45%, leukocytoclastic endovasculitis in 27%, and palisading granuloma in vascular wall in 9% of cases. The extravascular lesions included palisading granuloma in all cases, microabscess in 91%, and diffuse granulomatous tissues in 82%. Palisading microgranuloma (82%) was more frequent than palisading macrogranuloma (45%). After therapy, complete remission occurred in 8 patients, but 3 patients died of sepsis, diffuse pulmonary haemorrhage, and cerebral haemorrhage. Comparison of the frequency of each finding in the nasal biopsy specimens between patients who achieved remission and those who died showed that leukocytoclastic vasculitis was found more commonly in fatal cases, and leukocytoclastic endovasculitis was observed only in fatal cases. Palisading granuloma as a vascular or extravascular lesion is the primary and most important finding in a histopathological diagnosis of Wegener's granulomatosis, microabscess in vascular walls is a secondary but the next most important finding, and leukocytoclastic vasculitis heralds dissemination of the disease and poor prognosis. It requires aggressive therapy.

Notes: Abstract Systemic lupus erythematosus (SLE) was observed in a 7-year-old boy with IgG2 and IgG4 subclass deficiencies who had been treated with immunoglobulin (100–200 mg/kg/4 weeks) since 2 years of age. The mother and the half-brother displayed the same deficiency. Serum IgG mainly consisted of IgG1 (98.9%) during the acute phase of SLE due to transient IgG3 deficiency. While he had no common manifestations of SLE such as arthritis or nephropathy, he developed cardiac tamponade due to massive pericardial effusion. Conclusion The clinical features of SLE in the present case such as the development of cardiac tamponade and the absence of renal involvement may result from the markedly imbalanced IgG subclass distribution among auto-antibodies.

Notes: Abstract Some cationic surfactants such as cetyltrimethylammonium bromide (CTAB) show a very spectacular viscoelasticity in solution, in even very diluted states, with some aromatic substances added, such as salicylic acid. Formerly, the authors established that the remarkable solution behaviour was the result of the entanglement of such enormously elongated giant micelles, based on recognition of the existence of large aggregates, through direct observation of them as substantial images under the electron microscope. In this report, we further confirm the relation between micelle length and solution viscoelasticity after arbitrarily obtaining different size aggregates by altering the media pH, or by admixing shorter chain length surfactants than CTAB, which alone did not induce viscoelasticity.

Notes: Abstract Using an elastic vibration model for phonons of an oxidized small metallic particle, size dependence of the superconducting transition temperature TC is derived. It is shown that the oxidization results in the reduction of TC with decrease in particle size. The main cause is that the surface oxide enhances the phonon frequencies as the particle size decreases. Superconducting character of amorphous thin films and its relation to an oxidized small particle is also discussed.
