    Notes: Summary Twenty-three patients with aplastic anemia (18/23 with severe aplastic anemia) were treated with an immunosuppressive regimen consisting of cyclosporin A (CsA) and methylprednisolone (MP) (n=7) or CsA, MP, and antithymocyte globulin (ATG;n=16). Nineteen patients are alive with a follow-up of 4 to 25 months; three patients died of infections and one of a gastrointestinal hemorrhage. Within 3 months, improvement of hematopoiesis was seen in 14 patients (61%). First signs of a response after 23 to 88 days were followed by complete remission in eight patients, partial remission in three patients, and minimal improvement in three patients. Two of the patients with only minimal improvement were treated with a second course of immunosuppression and reached a complete remission and partial remission. Interestingly, remission proved to be dependent on the continued administration of CsA in four of five patients with partial or complete remission who could be evaluated up to now. Thus, CsA must have been effective in the induction and/or maintenance of remission in three patients. This observation is a very strong argument for the role of T cells in the pathogenesis of at least some cases of aplastic anemia and warrants further evaluation of the role of CsA in the treatment of aplastic anemia.

    Notes: Summary High-titered spontaneous interferon (IFN) antibodies were detected in a patient with pure red cell aplasia (PRCA), a benign mediastinal tumor, and recurrent cutaneous carcinomas. The circulating IFN antibodies reacted broadly with various human IFN-α subtypes (20–140×103 neutralizing units/ml serum) but not with IFN-β or IFN-γ, and they neutralized the antiviral activity of the patient's endogenous IFN-α. The IFN-αbinding activity was restricted to the IgG1 subclass in a nonmonoclonal manner. Whereas the PRCA repeatedly responded to immunosuppression with high-dose cyclosporin A (CSA) and CSA plus plasmapheresis, IFN antibody production continued during treatment with cyclophosphamide and CSA. Serological analysis revealed past infection with parvovirus B19 and persistent B19 IgM titers. Antibody-mediated impairment of the IFN-α system might have favored the development of both PRCA and the various cutaneous carcinomas in this patient.

    Description / Table of Contents: Zusammenfassung Es wurden Untersuchungen zur Bestimmung des Erythrocytenvolumens beim Menschen mit dem kurzlebigen Isotop 99m-Tc durchgeführt. Dabei zeigte sich, daß eine ausreichend stabile Markierung durch halbstündige Inkubation mit 99m-Tc und anschließende kurzzeitige Zugabe von SnCl2 erreicht werden kann. Die Inkorporationsrate ist vom Plasmaanteil und vom Hämatokrit der zu markierenden Erythrocytensuspension abhängig. Gleichzeitige Markierung derselben oder einer gleichen Probe mit51Cr ergab gut übereinstimmende Resultate für das Erythrocytenvolumen.
    Notes: Summary Incubation of human red cells with 99 m-Tc and subsequent addition of SnCl2 resulted in stable red cell labeling. Simultaneous tagging of the same or another red cell specimen by51Cr and calculation of the red cell volume from both isotopes gave almost identical results. The 99m-Tc-technique may become the method of choice for red cell volume determination.
