Zusammenfassung Von 1961–1979 behandelten wir 135 Patienten wegen nekrotisierender Pankreatitis. 63 behandelten wir konservativ. Die Letalität betrug 59%. 72 Patienten wurden nekrosektomiert. Deren Letalität ist nur 39%. Auch durch Spül-Saug-Drainagen-Behandlung ohne Nekrosektomie, gelang es, die Letalität von vorher 54% bei 84 Patienten auf 37% bei 51 Patienten, zu senken. Diebesten Ergebnisse mit nur 33% Sterberate hatten 27 Patienten, bei denen die Nekrosektomie mit einer Dauerspülung der Bauchhöhle kombiniert wurde. Dieschlechtesten Resultate wiesen 25 Patienten mit 72% Sterberate auf, bei denen weder nekrosektomiert noch gespült wurde. Es muß deshalb stets die Nekrosektomie mit Dauerspülung der Bauchhöhle kombiniert werden.

Summary Between 1961 and 1979 we treated 135 patients with acute, necrotizing pancreatitis. The lethality of 63 patients with conservative treatment was 59% - the lethality of 72 other patients with operative necrosectomy was 39%. — A third form of treatment was the continuous lavage of the abdominal cavity. The lethality of 51 thus treated patients was 37%. The best results of 33% lethality was seen in 27 patients, who were treated by combined necrosectomy and continuous lavage. The worst results showed 25 patients with a lethality of 72%. This group had neither necrosectomy, nor lavage. - Our results suggest the combination of necrosectomy and continuous lavage of the abdomen over a period of 3-5 days in all cases of necrotizing pancreatitis.
