    Description / Table of Contents: Summary Deficits in current approaches of psychosomatic and psychiatric education for general practitioners are reviewed. A skill-based training was developed in line with the four step counceling approach of the „PLISSIT” approach: The GP helps in first step („permission”) the patient to accept his problem and assesses informations about causing and maintaining factors of his symptomatology. In the second step he should provide information about the disorder and reverse misunderstanding and passivity (Limited Information). The GP’s should develop preliminary strategies for change in the third step („coping strategies”, „symptom management”; „special suggestions”) and preparate the intensive phase of therapy as the last step. First results of the evaluation of the training program are presented demonstrate the effectiveness and acceptance of the education programm.
    Notes: Zusammenfassung Ausgehend von einer Analyse der Schwächen aktueller Ansätze der psychosomatischen Grundversorgung wird einerseits ein Ansatz vorgestellt, der störungsspezifisch im Rahmen eines Mehrebenenmodells pharmakologische und verbale Intervention aufeinander bezieht. Andererseits werden im Sinne eines zeitökonomisch abgestuften Vorgehens verschiedene Phasen durch das zugrundegelegte PLISSIT-Modell differenziert: In der Permission-Phase geht es um die Akzeptanzvermittlung und Problemanalyse hinsichtlich der vom Patienten geschilderten Beschwerden, in der Phase der „limited information” um die dialogische Erarbeitung eines Störungsmodells und den Aufbau einer aktiven Änderungsmotivation, in der Phase der „special suggestions” erfolgt eine konkrete Anleitung zur Selbstbeobachtung und -protokollierung des Problemverhaltens sowie die Vermittlung von Strategien zum Symptommanagement; in der letzten Phase der „intensive therapy” geht es um die Indikationsstellung und den Motivationsaufbau zur fachärztlichen bzw. fachpsychotherapeutischen Weiterbehandlung. Empirische Ergebnisse zur Evaluation des Weiterbildungskonzeptes an über 200 Teilnehmern werden referiert.

    Description / Table of Contents: Summary Atopic eczema is one of the most frequent dermatological diseases, with a still increasing incidence. Although its etiopathogenesis is still not completely understood, a variety of triggering factors are known. The following case report describes the clinical course of atopic dermatitis in a 2-year-old boy in whom both IgE-mediated allergic reactions and psychological factors were involved in triggering and maintenance of skin lesions. This demonstrates how important both dermatological therapy and close cooperation with the field of psychosomatic medicine can be.
    Notes: Zusammenfassung Das atopische Ekzem zählt zu den häufigsten dermatologischen Erkrankungen mit zunehmender Inzidenz. Obwohl die Ätiopathogenese letztendlich ungeklärt ist, sind eine Vielzahl auslösender Faktoren bekannt. Anhand der Falldarstellung eines 2jährigen Kindes, bei dem sowohl IgE-vermittelte allergische Reaktionen als auch psychische Faktoren an der Auslösung und Unterhaltung der Hautveränderungen beteiligt waren, soll aufgezeigt werden, daß eine erfolgreiche Behandlung des atopischen Ekzems in vielen Fällen neben dermatologisch-allergologischen Therapieansätzen ebenso eine enge Kooperation mit dem Fachgebiet der Psychosomatik erforderlich macht.

    Notes: Abstract In recent years many studies have indicated an involvement of inflammatory mechanisms in Alzheimer's disease (AD). Acute-phase proteins such as α1-antichymotrypsin and c-reactive protein, elements of the complement system, and activated microglial and astroglial cells are consistently found in brains of AD patients. Most importantly, also cytokines such as interleukin-6 (IL-6) have been detected in the cortices of AD patients, indicating a local activation of components of the unspecific inflammatory system. Up to now it has remained unclear whether inflammatory mechanisms represent a primary event or only an unspecific reaction to brain tissue damage. Therefore, we investigated whether IL-6 immunoreactivity could be found in plaques prior to the onset of neuritic changes, or whether the presence of this cytokine is restricted to later stages of plaque pathology. we confirmed our previous observation that IL-6 is detectable in a significant proportion of plaques in the brains of demented patients. In AD patients IL-6 was found in diffuse plaques in a significant higher ratio as would have been expected from a random distribution of IL-6 among all plaque types. This observation suggests that IL-6 may precede neuritic changes, and that immunological mechanism may be involved both in the transformation from diffuse to neuritic plaques in AD and in the development of dementia.

    Description / Table of Contents: Summary In the light of highly successful somatic therapies psychotherapeutic interventions in erectile dysfunction are nowadays less considered. But they are nevertheless highly effective and needed according to the psycho-somatic nature of the condition. In this article we present an etiologic framework of psychogenic erectile dysfunction as well as the diagnostic approach and therapeutic interventions. Finally we discuss the need for an increased research in the combination of psychotherapeutic and medical therapy.
    Notes: Zusammenfassung Angesichts großer Erfolge in der medikamentösen Behandlung der erektilen Dysfunktion haben psychotherapeutische Interventionen in letzter Zeit wenig Aufmerksamkeit erhalten. Doch ist bei Erektionsstörungen als psycho-physisches Gesamtgeschehen auch die Kenntnis psychologischer Faktoren für eine erfolgreiche Behandlung nötig. Die vorliegende Arbeit stellt grundlegende ätiologische Konzepte psychogener Erektionsstörungen vor, erläutert die psychologische Diagnostik und die gängigen Modelle für Sexualberatung und Sexualtherapie. Schließlich wird die Möglichkeit einer Kombination von medikamentöser und psychotherapeutischer Behandlung diskutiert.

    Notes: We have reformulated the general problem of internal flow in a modern, high speed gas centrifuge with sources and sinks in such a way as to obtain new, simple, rigorous closed form analytical solutions. Both symmetric and antisymmetric drives lead us to an ordinary differential equation in place of the usual inhomogeneous Onsager partial differential equation. Owing to the difficulties of exactly solving this sixth order, inhomogeneous, variable coefficient ordinary differential equation we appeal to the power of perturbation theory and techniques. Two extreme parameter regimes are identified, the so-called semi-long bowl approximation and a new short bowl approximation. Only the former class of problems is treated here. The long bowl solution for axial drive is the correct leading order term, just as for pure thermal drive. New O(1) results are derived for radial, drag and heat drives in two dimensions. Then regular asymptotic, even ordered power series expansions for the flow field are carried out on the computer to O(ε4) using MACSYMA. These approximations are valid for values of ε near unity. In the spirit of Van Dyke, one can carry out this expansion process, in theory, to apparently arbitrary order for arbitrary but finite decay length ratio. Curiously, the flows induced by axial and radial forces are proportional for asymptotically large source scale heights, x*. Corresponding isotope separation integral parameters will be given in a companion paper.

    Notes: Linearized, multidimensional, thermally driven flow in a gas centrifuge can be approximately described in regions away from the ends by Onsager's homogeneous pancake equation.1 Upon reformulation of the general problem, we find a new, simple and rigorous closed form, analytical solution by assuming a special separable solution and replacing the usual Ekman end cap boundary conditions with idealized impermeable, free slip boundary conditions. Then the flow may be described by an ordinary differential equation with solutions in terms of simple, classical functions. By identifying a small parameter, say ∊, defining the semi-long bowl approximation, and assuming a power series expansion in ∊, a sequence of asymptotic approximations to the master potential is obtained. Not surprisingly, the leading order term involves the well known ‘long bowl’ solution. Using the so-called ‘solving’ property of the 1-D pancake Green's function,2 we determine the next higher order solution. This recursive process is carried out on the computer to find all the terms up to O(∊4).Consequently, the solution of some complex rotating, viscous, heat conducting flow problems that normally require large mainframe computers can be better understood.

    Notes: Linearized multidimensional flow in a gas centrifuge can be described away from the ends by Onsager's pancake equation. However a rotating annulus results in a slightly different set of boundary conditions from the usual symmetry at the axis of rotation. The problem on an annulus becomes ill-posed and requires some special attention. Herein we treat axially linear inner and outer rotor temperature distributions and velocity slip. An existence condition for a class of non-trivial, one-dimensional solutions is given. New exact solutions in the infinite bowl approximation have been derived containing terms that are important at finite gap width and non-vanishing velocity slip. The usual one-dimensional, axially symmetric solution is obtained as a limit. Our previously reported finite element algorithm has been extended to treat this new class of problems. Effects of gap width, temperature and slip conditions are illustrated. Lastly, we report on the compressible, finite length, circular Couette flow for the first time.
