Zusammenfassung Wir untersuchten, ob mit PGO exokrines Parenchym eliminiert und damit die chronische Entzündung beendet, dabei aber die endokrine Funktion erhalten werden kann. Methode: Die Entzündungsaktivität (Pankreas-Isoamylase, Lipase und Trypsin), exokrine (sekretinstimuliertes Trypsin) und endokrine Funktion (basal und maximal stimuliertes Insulin und C-Peptid) wurden bei Patienten mit Whipplescher Operation und PGO (A), mit endoskopischer PGO (B) sowie mit alleiniger Drainageoperation (C) verglichen. Ergebnisse: Allein Konzept A konnte die Entzündungsparameter normalisieren und gleichzeitig die postoperative endokrine Kapazität über 18 Monate konstant erhalten.

Summary We carried out a study to see if therapeutic PDO can terminate chronic inflammation by eliminating exocrine parenchyma and can preserve endocrine function in the treatment of CP.Method: Inflammatory activity (pancreatic isoamylase, lipase, and trypsin), exocrine (secretin-stimulated trypsin), and endocrine pancreatic function (basal and maximal stimulated serum insulin and C peptide) were determined in patients with Whipple's operation (A), with endoscopic duct occlusion (B), and with draining procedures (C) in a follow-up period of 18 months.Results: Only therapy A eliminated chronic inflammatory processes effectively enough while still preserving the remaining endocrine pancreatic function. Endoscopic PDO and drainage procedures cannot achieve these results.
