Zusammenfassung 1982 kamen von 72 Patienten 57 (79%) im klinischen Beschwerdestadium 111, 12 (16%) im Stadium IV, 3 (4%) im Stadium II zur Operation. Die Zeit zwischen Diagnosestellung und Operation betrug im Mittel 10 Jahre, zwischen Herzkatheter und Operation im Mittel 3 Monate. Frühletalität 8,3% (6/72), davon 3 Patienten im klinischen Stadium III, 3 im Stadium IV (Alter: 3 Patienten unter 60, 3 Patienten über 70 Jahre). Operations-Risiko im Stadium II nahezu 0%. Forderung: Operations-Indikation zum Aortenklappenersatz in das klinische Stadium II vorzuverlegen. Ziel: Weitgehendste Letalitätssenkung und Verhinderung irreversibler Myokardschäden.

Summary In 1982, of the 72 patients who underwent aortic valve replacement, 57 (79%) were NYHA class III, 12 class IV (16%), and 3 class II (4%). The mean time interval between diagnosis and operation was 10 years, between catheterization and operation 3 months. Early mortality was 8.3% (6/72), 3 patients NYHA class III, and 3 class IV 3 patients under 60 years of age, 3 patients over 70. The operative risk for class II patients is almost zero. Recommendation: advanced indication for aortic valve replacement to NYHA class II. Aims: further reduction of mortality; prevention of irreversible myocardial damage.
