    Notes: Summary A case of cerebral air embolism from a rather unusual cause is reported; an esophago-cardiac fistula permitted food particles and air to enter the systemic arterial circulation. Massive embolization caused the patient to become deeply comatose rather suddenly. The computed tomogram (CT) revealed massive cerebral edema with the contradictory finding of wide superficial subarachnoid spaces. These subarachnoid spaces on further evaluation proved to be air in the cerebral vessels.

    Notes: Summary This study of 14 follicular adenomas, 10 papillary carcinomas and 11 follicular carcinomas of the human thyroid gland demonstrates the possibility of a cytological tumor classification using digital picture processing. Routinely prepared, HE-stained imprints of surgical specimens were scanned under a light microscope at high resolution with a colour TV camera. The cell nuclei were segmented and analysed with an image processing system. The computer-aided cytophotometric methods detected the most significant differences in the chromatin texture with a criteria variance of texture line distances and texture points per texture knots. Using these criteria benign and malignant tumor types could be successfully differentiated.

    Notes: Summary Seventy-five female Wistar rats of 150 g each were given 3×40 mg/kg body weight nitrosomethylurea (NMU) and 4 weeks later until death (in the 9th–60th week) methylthiouracil (MTU), as a 0.1% solution in their drinking water. After 2 months, in the almost colloid-less thyroid gland individual foci appeared which consisted of sometimes colloid containing follicles with increased cytoplasmic and nuclear basophilia; they increased in number during the subsequent weeks and finally resulted in nodes with different morphologic structures. From the 16th week on there were tumors infiltrating the capsule, and after the 24th week there were angioinvasive carcinomas. Thirteen rats had developed lung metastasization after the 30th week. Cytology could only distinguish between the follicular carcinomas of low differentiation and the follicular adenomas. If these findings are applied to human pathology, one would have to reckon with adenomas which can develop their malignant characteristics without necessarily changing their histologic and cytologic appearance.

Alle Patienten wurden operiert. Zu 92% hatte der Tumor die Schilddrüsenkapsel durchbrochen. Eine chirurgische Radikalität erwies sich als wenig sinnvoll, da die Komplikationsrate hoch ist und die Prognose nicht verbessert werden kann. Die durchschnittliche Überlebenszeit der Patienten mit anaplastischen Carcinomen betrug 117 Tage. Operation, Bestrahlung und Cytostase hatten nur geringen Einfluß auf die schlechte Prognose.
    Notes: Summary Of 422 patients with malignant tumors of the thyroid, 91 had an undifferentiated neoplasm (73 anaplastic, 4 squamous cell carcinomas, 7 hemangioendotheliomas, 6 sarcomas, and 1 carcinosarcoma). All patients had been operated upon. In 92% the tumor had penetrated the capsule of the thyroid. A radical surgical procedure did not prove to be successful because those operations carry a high risk and patients do not improve thereafter. The average survival time of patients with anaplastic tumors was 117 days. Operation, radiation, and cytostasis had little influence on the prognosis.

    Description / Table of Contents: Zusammenfassung Plattenepithelcarcinome der Schilddrüse sind sehr seltene Tumoren (0,3–1,4% aller malignen Schilddrüsentumoren). Unter 422 Patienten mit einer Struma maligna waren 5 Frauen bei denen ein Plattenepithelcarcinom nachgewiesen werden konnte. Zwei hatten eine seit Jahren bestehende Struma mit verkalkten Cysten. Histologisch stand die Plattenepithelmetaplasie mit den degenerativen Veränderungen in Verbindung. Der immunhistochemische Nachweis von Thyreoglobulin war positiv. Der Tumor ist extrem bösartig. Die überlebenszeit liegt etwa bei einem Jahr. Ein Tumor war hochdifferenziert und vermutlich branchiogenen Ursprungs.
    Notes: Summary Squamous cell carcinomas of the thyroid are extremly rare (0.3–1.4% of the malignant thyroid tumors). Since 1970, 5 women of 422 patients with a malignant thyroid tumor have been found to have squamous cell carcinoma. Two of them had a pre-existent large goiter with calcareous cysts. Histologically, the squamous cell metaplasia was combined with the degenerative changes. Thyreoglobulin radioimmunoassay was positive. The tumor is highly lethal (about 1-year survival). One of the tumors was highly differentiated and of branchiogenic origin.
