Notes: Summary The supra sellar extension of even small sized pituitary adenomas can be delineated with accuracy by angiotomography. This technique avoids the discomfort of pneumoencephalography in many patients. Angiotomography is also useful for the differential diagnosis of empty sellae.

Notes: Summary Symptomatic intrasellar Rathke's cleft cysts are rather rare: only a few cases with CT findings have been reported. The authors describe the CT appearance of 3. Histological aspects and embryology are discussed. Rathke's cleft cysts have to be differentiated from other intrasellar tumours: cystic craniopharyngiomas, cystic or necrotic pituitary adenomas, cysticercosis.

Notes: Summary In the last few years complete or partial regression of prolactinomas has been demonstrated in nonpregnant women treated by bromocriptine. Thus bromocriptine therapy appears as an attractive alternative to surgery for management of infertility related to hyperprolactinemia. However, numerous reports emphasized the possibility of an excessive growth of the pituitary adenoma with visual field defects during the last 3 months of pregnancy. To avoid these complications, the authors followed with serial CT scans the growth of microprolactinoma at the 5th or 6th month of pregnancy. Among six pregnant women, one patient presented a marked upward extension of the adenoma. Bromocriptine was then reintroduced and the effectiveness in reducing tumor growth was proved by CT scan at the 7th month. Regarding low risk of using intravenous iodinated contrast medium in pregnant women and of fetal radiation damage, the authors emphasize the value of CT in the follow-up of bromocriptine-induced pregnancies.

Notes: Summary The authors report three cases of lumbar apophyseal joint lipoma located at the L4/L5 or L5/S1 level. CT scan makes the diagnosis easily possible; indeed erosive changes with enlargement of the upper portion of the lumbar articular joint and anterior displacement of the top of the superior facet are associated with intra-articular fatty densities. Clinical symptoms are rarely encountered and may be related to narrowing of the upper portion of the intervertebral foramen.

Notes: Summary Tomoechographic examination with automatic displacement of the ultrasonic probe is usually looked upon as an inadequate technique for cranial exploration. However, in children it allows excellent examination of ventricular walls. In fact, such devices make echotomography a simple, reliable, non-painful diagnostic method in hydrocephaly, especially for post-operative monitoring of ventricular size. It seems a more reliablemethod and more in line with radiology than plain Aechography.

Notes: Abstract We describe the MRA and MR angiography (MRA) features of persistent trigeminal artery (PTA) found incidentally in eight patients, with special attention to its origin, site and course. The different patterns of posterior communicating arteries were also noted. The PTA were shown on sagittal, coronal and axial MRI and on MRA. In four cases, the PTA arose from the lateral aspect of the intracavernous internal carotid artery, ran caudally, passing round the bottom of the dorsum sellae to join the basilar artery. In the other four cases, it arose from the medial aspect, ran caudally through the sella turcica and pierced the dorsum sellae to join the basilar artery. The posterior communicating arteries were present unilaterally in five cases and bilaterally in one, and absent bilaterally in two. Identification of a PTA with a trans-sellar course is crucial if a trans-sphenoidal surgery is planned.

Notes: Summary The authors stress the frequency of metastases of the cervico-occipital junction unrecognized in medical literature: 30 cases have been diagnosed within four months. They report their different patterns and complications and insist on the prefractured pattern with osteolysis of the body of C-2 which can be shown only by polytomography. The authors propose that a tomographic study of the cervico-occipital junction should be systematically included in every analysis of metastatic extension of osteophilic tumors.

Notes: Summary Five anatomical and radiological observations of a spine protruding into the pituitary fossa are reported. This osseous spine, about 4 mm long, arises in the midline from the inferior part of the anterior aspect of the dorsum sellae and extends upward and forward. The possible origins of this malformation are discussed.

Notes: Summary The experience with percutaneous puncture of intraspinal cystic lesions is reported in four cases: an extradural cyst demonstrated with air, a spinal cord tumor with lipiodol and two cases of syringomyelia with metrizamide. Percutaneous puncture of cystic lesions makes possible an accurate delineation of the lesion and a histological approach through biochemical and cytological analysis of the aspirated cystic fluid, while aspiration of the fluid of cystic tumors decompresses the spinal cord and thus plays a therapeutic role.
