    Description / Table of Contents: Summary This study presents a survey on the demand for out-patient neurological-neuropsychological rehabilitation. Neurologists working in acute care and in in-patient rehabilitation settings as well as neurologists and neuropsychologists with out-patient practice were asked to rate the demand for and the present means to realize out-patient neuropsychological rehabilitation. The results show a great demand for out-patient rehabilitation. The preferred concept and model of out-patient rehabilitation depends on the setting where the participants of the study work with a tendency to favour flexible programs close to patients’ habitation.
    Notes: Zusammenfassung Dargestellt werden die Ergebnisse einer Umfrage zur ambulanten/teilstationären Rehabilitation bei Kliniken der neurologischen Akutversorgung, neurologischen Rehabilitationskliniken, niedergelassenen Nervenärzten, Neurologen und niedergelassenen Neuropsychologen. Von allen befragten Institutionen wird ein hoher Bedarf an ambulanten/teilstationären Rehabilitationsmaßnahmen artikuliert, wobei sich die bevorzugten Modelle und Konzepte in Abhängigkeit vom stationären und ambulanten Charakter der jeweiligen Institutionen deutlich unterscheiden. Über alle Untersuchungsteilnehmer zeichnet sich ein Trend zur Bevorzugung flexibler, wohnortnaher Angebote ab.

    Description / Table of Contents: Summary Neurological-neuropsychological out-patient rehabilitation in Southern Germany is realized mostly by neurologists and neuropsychologists in out-patient practice. However, profile and quantity of treatment as well as the structure of the health system fail to meet the requirements of out-patient rehabilitation as described in the first part of the study.
    Notes: Zusammenfassung Die Analyse der aktuellen Versorgungssituation im Bereich der ambulanten neurologisch-neuropsychologischen Rehabilitation zeigt, daß vorwiegend niedergelassene Nervenärzte, Neurologen und Neuropsychologen auf diesem Gebiet tätig sind. Der dargestellte Bedarf an ambulanten Rehabilitationsmaßnahmen kann von diesen Behandlergruppen sowohl bezüglich Leistungsumfang als auch Leistungsprofil nur rudimentär abgedeckt werden. Weiterhin erweist es sich, daß bislang keine gesundheitspolitischen und infrastrukturellen Rahmenbedingungen etabliert sind, die die Grundlage einer effizienten ambulanten oder teilstationären neurologisch-neuropsychologischen Rehabilitation bilden.

    Description / Table of Contents: Abstract The official German guidelines for prevention of intravascular-device-associated infections were published by the Bundesgesundheitsamt, now called the Robert-Koch-Institut, 12 years ago. The recently published official „Guidelines for Prevention of Intravascular-Device Associated Infections” of the Centers for Disease Control and Prevention (CDC) are categorized according to scientific evidence. The American guidelines are very detailed and differ in some aspects from the offical German guidelines. The purpose of the present paper is to inform the German-speaking anaesthesiologist about the official CDC guidelines and to provide an update on the prevention of intravascular-device-associated infections.
    Notes: Zusammenfassung Die offiziellen deutschen Richtlinien zur Prävention katheterassoziierter Infektionen durch das frühere Bundesgesundheitsamt (jetzt Robert-Koch-Institut) sind mittlerweile 12 Jahre alt. Die kürzlich publizierten offiziellen Empfehlungen der amerikanischen Centers for Disease Control and Prevention (CDC) beruhen auf vorhandenen wissenschaftlichen Daten und sind daraufhin bezüglich ihrer Wertigkeit kategorisiert worden. Diese umfassenden Empfehlungen sind wesentlich ausführlicher als die offiziellen deutschen Richtlinien und weisen zu diesen teilweise beträchtliche Unterschiede auf. Mit dieser kurzen Darstellung der amerikanischen Richtlinien soll dem Kliniker für die eigene praktische Tätigkeit bezüglich vieler alltäglicher, aber keineswegs nebensächlicher Probleme eine fundierte Entscheidungshilfe zur Verfügung gestellt werden.

    Notes: Abstract The preventive effect of maternal avoidance of cow’s milk and eggs on the development of atopic dermatitis and sensitization to food allergens was studied in a prospective trial with families at high risk for atopy. intervention included encouraging exclusive breast feeding for at least 3 months and delaying the controlled introduction of solid foods. In addition, one group received a maternal diet with the complete avoidance of milk and eggs in the last trimester of pregnancy and during the period of exclusive breast feeding, another group had the same diet starting after delivery, and mothers of the third group had no dietary restrictions. The period prevalences of atopic dermatitis as well as the rates of specific sensitization to eggs and milk at 6 and 12 months were not significantly different between groups. We were unable to demonstrate a significant preventive effect of maternal diet. Conclusion While breast feeding should be promoted for children at risk for atopy, mothers can be encouraged to stay on normal diet during pregnancy and the breast feeding period.
