Zusammenfassung Verletzungen des Beckenrings sind oft weft komplexer als radiologisch zunächst vermutet. Besonders die dorsale Beteiligung der Ileo-sacral-Gelenke, bzw. des Sacrum wird leicht verkannt oder unterschätzt, was zu invalidisierenden Spätschäden führen kann. Eine sorgfältige klinische Untersuchung sowie besonders die Computertomographie helfen eine Instabilitdt des Beckenrings zu diagnostizieren. Neben der bekannten Fixation der Symphyse mittels Zuggurtungsplatte wird auf die Stabilisierungsmöglichkeiten der Sacro-iliacal-Fuge mittels Schrauben oder Platten hingewiesen and mit Fallbeschreibungen reichlich illustriert. Der anderorts propagierte Fixateur externe kommt bei uns nur ausnahmsweise zum Einsatz.

Summary Injuries to the pelvic ring often appear to be much more complex than judgd by X-ray pictures alone. The dorsal components of the sacroiliac joint and sacral body are especially all too frequently misinterpreted, which can lead to considerable late sequelae. Clinical examination and computed tomography will help to identify the unstable lesions requiring surgery. For the anterior part of the pelvic ring, we advocate plate fixation across the symphysis rather than the use of external fixators, especially if a laparotomy had to be performed. For posterior instability, the application of screw fixation is described, using a posterior approach.
