    Notes: Abstract We examined the correlation among preoperative serum carcinoembryonic antigen (CEA) levels, staining properties of the tumors by CEA immunohistochemistry and the tumorigenicity of their xenografts in nude mice, in 28 patients with gastric cancer. Eleven (40 per cent) of them were positive for serum CEA (≧2.5 ng/ml) and seven (25 per cent) of the xenografts were tumorigenic in nude mice. All the tumorigenic cases were positive for serum CEA (p〈0.001) and the mean value of the serum CEA level in the patients with tumorigenic neoplasms was 20.8 ng/ml, being significantly higher than that (1.4 ng/ml) in the patients with nontumorigenic neoplasms (p〈0.001). Twenty-five of the 28 carcinomas (89 per cent) were positive for CEA staining in their cancer cells by the ABC method and CEA localization correlated with tumorigencity (p〈0.05). These results suggest that the serum CEA level in patients is correlated with the tumorigenicity of their gastric carcinoma xenografts in nude mice and may account for the poor prognosis of patients with high serum CEA.

    Notes: Abstract The mitogen-activated protein kinase (MAPK) family is considered to be activated by stress, but the role of the MAPK family is still unknown in cardiac pathology. In the present study, not only the localization of MAPKs such as the extracellular responsive kinase (ERK), c-jun N-terminal kinase (JNK), and p38 MAPK (p38), but also ultrastructural changes were investigated in the ischemia-reperfusion model of Wistar rats. At 5, 10, 30, 60, and 180 min reperfusion after 30 min ischemia by occluding the coronary artery, the expression of these MAPKs was increased in blood vessels and cardiomyocytes by Western blotting and immunohistochemical methods. In addition, after ischemia reperfusion, various ultrastructural changes such as decreased glycogen granules, mitochondrial swelling, and myolysis were observed in the blood vessels and cardiomyocytes. These results suggest that protein kinases may regulate numerous biological processes, including the regulation of contraction and ion transport.

    Notes: Abstract The network of elastic system fibers in human fibrotic liver was investigated by histological methods, immunohistochemical staining, and electron microscopy. Type III collagen was seen not only in regions of portal fibrosis but also in the sinusoidal wall. However, elastic system fibers were not found in the Disse space of the sinusoidal wall. Elastic system fibers including oxytalan, elaunin, and elastic fibers were found successively in the course of elastogenesis. A few normal oxytalan fibers and abnormal oxytalan fibers were observed in the periportal tracts. Few normal elaunin and abnormal elaunin fibers were observed in regions of portal fibrosis but not in the surrounding margin. Elastic fibers, only in scarce amounts, were observed around the portal veins in the case of chronic active hepatitis but not in acute hepatitis. Abnormal oxytalan fibers were seen as a bundle of wavelike microfibrils and had an irregular arrangement. Abnormal elaunin fibers were not associated with bundles of microfibrils. Abnormal elaunin fibers in large amounts were found interspersed with spiraled collagen, which most likely indicates that the oxytalan fibers degenerated in the course of elastogenesis. Thus, in a fibrotic liver it is possible that synthesis of normal elaunin and elastic fibers does not occur or that the quantity of such fibers synthesized may be small because of the effect of the degenerated oxytalan fibers. As a characteristic of liver fibrosis, the composition of abnormal elastic system fibers and spiraled collagen differs from that in other fibrotic organs.

    Notes: Abstract A 67-year-old woman had frequent subacute ileus, hearing difficulty, muscle atrophy and stroke-like episodes. Computed tomography revealed multiple low-density areas, which did not correlate with the vascular supply, in the cerebral cortex. She had metabolic disturbance comprising lactic acidosis and elevated pyruvate level. Her skeletal muscle biopsy specimen showed ragged-red fibers, and mitochondrial DNA analysis revealed a point mutation at position 3243, findings consistent with MELAS. Examination of her small intestine revealed a necrotic zone and numerous abnormal large mitochondria in the smooth muscle cells, vascular media and endothelium, and intestinal ganglion cells. The cerebral cortex showed multiple microcystic necrotic foci in cerebral cortex. Cactus-like pathology resembling the changes associated with Menkes’ kinky hair disease and torpedoes were observed in the cerebellar Purkinje cells. The intestinal dysmotility due to MELAS and cerebellar changes were presumed to be associated with a disturbance of copper metabolism.

    Notes: Abstract Enzyme-cytochemical studies were performed to clarify the pathogenesis of myocardial damage in endotoxemia. The reaction product of Ca++-ATPase was localized on the pinocytotic vesicles and plasma membrane of endothelial cells of capillaries and sarcolemma, sarcoplasmic reticulum and mitochondria of cardiomyocytes in control rats. However, the enzyme reaction product was reduced in amount and irregularly distributed on the plasma membrane, and on the increased number of pinocytotic vesicles of the endothelial cells of capillaries and the sarcolemma and sarcoplasmic reticulum of edematous cardiomyocytes after endotoxin injection. It is concluded that endotoxin induces structural changes and decreased activity in Ca++-ATPase in the vascular endothelium and myocytes of the heart.

    Notes: Abstract To evaluate the relationship between pulmonary damage and the induction of nitric oxide synthase (NOS) in endotoxin shock, we injected 10mg/kg ofE. coli endotoxin intraperitoneally to Wistar male rats and observed the changes of the lung during the following 8h by electron microscopy, immunoelectron microscopy, and in situ hybridization. Morphological observation revealed infiltration of macrophages, aggregation of neutrophil in stasis in vascular lumens, and intraalveolar hemorrhage accompanied by epithelial damage. Endothelial constitutive NOS (ecNOS) was immunohistochemically localized in the endoplasmic reticulum of the endothelium of pulmonary arteries and in the cytoplasm of bronchial epithelial cells of control rats. After endotoxin administration, inducible NOS (iNOS) was detected in vascular endothelial cells, vascular smooth muscle cells, bronchial epithelial cells, bronchial smooth muscle cells, alveolar epithelial cells, and macrophages. Reverse transcription polymerase cham reaction (RTPCR) confirmed the expression of ecNOS mRNA and iNOS mRNA in the lung in endotoxin-treated rats and controls. In situ hybridization showed that ecNOS mRNA was expressed in vascular endothelial cells of pulmonary arteries in control rats. After endotoxin administration, iNOS mRNA was expressed in vascular endothelial cells. vascular smooth muscle cells, bronchial epithelial cells, alveolar epithelial cells, and macrophages that had infiltrated the alveolar and perivascular regions. After endotoxin administration, morphological changes and NO overproduction were observed, and it is concluded that NO may play an important role in maintaining the homeostasis of the bloodair barrier in pulmonary structures.

    Notes: Abstract A solitary fibrous tumor of the pleura (SFTP) in a 37-year-old woman was reported as a rare pleural tumor based on morphological, immunohistochemical, and electron microscopic studies. The results showed that the tumor was composed of spindle-shaped cells forming fascicular and interlacing patterns with a mixture of mature collagen. The tumor cells coexpressed vimentin and CD34 and lacked cytokeratin reactivity. Ultrastructurally, there were abundant collagenous fibrillae surrounding spindle-shaped cells in which the junctional complex, basement membrane-like structure, and microvilli were not seen. From literature review and observation of the morphological features around the tumor, we consider that the tumor originated from the stromal cells subjacent to the mesothelium.

    Notes: Abstract Nerve invasion is one of the biological features of pancreatic cancer, and its mechanism remains to be determined. In this paper, we report on 37 pancreatic cancer specimens observed by immunohistochemical and electron microscopical techniques. The results showed that pancreatic cancer directly invaded and destroyed the perineurium. At the early stage of disease, the peripheral nerve and synaptic membrane were easily destroyed by cancer cells, and invasion and metastasis continuously advanced along the perineural space and central side of nerves. These results suggest that the soft tissue and nerve plexus on the dorsal region of the pancreas may contribute to the recurrence of pancreatic cancer after duodenopancreatectomy.
