    Description / Table of Contents: Abstract When a biologic-elastic surgical technique is used, internal plate fixation of the femur is indicated in the polytraumatized patient, particularly with severe thoracic or cranial trauma, for fractures near the joints, for fractures in growing children and in fractures associated with total joint arthroplasty. Intramedullary nailing is the method of choice in all other indications for internal fixation in the femoral shaft. The external fixator is a temporary implant for use in severely polytraumatized patients. Rigid internal plate fixation with anatomical reconstruction of the fragments and compression was formerly widely applied, but this technique interfered with the vascularization of the bone. This resulted in bone necrosis and complications arising from the interfragmentary stability that was necessary until direct bone healing was achieved. Biologic-elastic internal plate fixation, in contrast, preserves the vascularization and favours adequate micromovement of fragments relative to each other as the plate retains elasticity since there are several plate holes without screws. This results in a spontaneous fracture healing with rapid callus formation, but without cortical bone necrosis. The surgical procedure does not require stripping of the periosteum, anatomical repositioning of fragments, adjustment of third fragments, interfragmentary lag screws, the tension device, compression by means of DC holes or any cancellous bone graft. The plate can also be inserted by means of two small incisions in a “push-through” technique. So far 120 biologic-elastic internal plate fixations have been performed in the Trauma Department of the University of Göttingen, including 76 in the femur. All fractures healed within the normal time. The patients walked with reduced weight-bearing at first, and with full weight-bearing after 12 weeks at the most. We saw 2 cases of late infection 5 months after third-degree open fractures, 4 patients with rotational deformities of more than 10 ˚ and 2 with axial deformities. In addition, 13 fractures of the femur were treated in the same manner in children without any complications. Internal plate fixation for a fracture of the femur with a total hip or knee arthroplasty may require an additional autogenous cancellous bone graft because of the reduced vitality of these bones. In the case of articular fractures a rigid internal fixation technique must always be used.
    Notes: Zusammenfassung Unter Verwendung der biologisch-elastischen Operationstechnik besteht die Indikation zur Plattenosteosynthese am Femurschaft bei Polytrauma, besonders mit schwerem Thorax- und Schädel-Hirn-Trauma, bei Frakturen in Gelenknähe, bei Frakturen im Wachstumsalter und bei Frakturen bei liegender Hüft- oder Knieprothese. Die Marknagelosteosynthese ist bei allen anderen Indikationen am Oberschenkelschaft das Verfahren der Wahl. Der Fixateur externe ist ein temporäres Implantat zur Versorgung des schwer Polytraumatisierten. Bei der früher dominierenden rigiden Plattenosteosynthese mit anatomischer Reposition und interfragmentarer Kompression wurde der Erhalt der Blutzirkulation des Knochens zu wenig beachtet. Knochennekrosen und die Notwendigkeit der interfragmentaren Stabilität bis zum Eintritt der direkten Knochenbruchheilung führten bei der rigiden Plattenosteosynthese zu Komplikationen. Die biologisch-elastische Plattenosteosynthese schont dagegen die Fragmentvaskularisation und erlaubt unter langstreckiger Elastizität der Platte durch unbesetzte Plattenlöcher dosierte Mikrobewegungen zwischen den Fragmenten. Folge ist eine spontane Frakturheilung mit rascher tragfähiger Kallusbildung unter Vermeidung von kortikalen Knochennekrosen. Es wird auf ein Ablösen des Periosts, eine fugenlose Reposition, auf das Einpassen dritter Fragmente, auf interfragmentare Zugschrauben, auf den Plattenspanner, auf die Kompression über die DC-Löcher sowie eine primäre Spongiosaplastik verzichtet. Die Platte kann auch über 2 kleine Inzisionen in durchgeschobener Technik eingebracht werden. Von bisher 120 biologisch-elastischen Plattenosteosynthesen wurden 76 am Oberschenkel durchgeführt. Alle Frakturen heilten regelrecht bei sofortiger entlastender Gehfähigkeit und Vollbelastung spätestens nach 12 Wochen. Es traten 2 Spätinfekte 5 Monate nach drittgradig offener Fraktur, 4 Drehfehler 〉 10° sowie 2 Achsfehler auf. Weitere 13 Femurfrakturen wurden bei Kindern im Wachstumsalter ohne Komplikationen behandelt. Die Plattenosteosynthese bei liegender Endoprothese erfordert dagegen wegen der gestörten Vitalität des Knochens häufig eine zusätzliche autogene Spongiosaplastik. Bei Gelenkfrakturen muß die Osteosynthese nach wie vor rigide durchgeführt werden.

    Notes: Abstract Two genomic fragments capable of driving the expression of the hygromycin B resistance gene (hph) were isolated from the phytopathogenic ascomycete Gibberella pulicaris (anamorph Fusarium sambucinum) using a “promoter-probe library” strategy. Two libraries consisting of random, 0.5–2.0-kb fragments of genomic DNA inserted 5′ of a promoterless hph gene were constructed and used for transformation of G. pulicaris. Both libraries transformed G. pulicaris at a low frequency. Transformants tolerated up to 800 μg/ml of hygromycin B, while untransformed colonies were inhibited completely by 50 μg/ml of the antibiotic. Plasmids were re-isolated from transformants by simply digesting, the genomic DNA with KpnI, which cuts once in the polylinker 5′ to the insert, and transforming E. coli with the re-ligated DNA. The recovered plasmids transformed G. pulicaris with a frequency of up to 4.4 transformants/μg of DNA. Both promoter fragments were sequenced and found to contain TATA and CAAT boxes as well as CT-rich sequences. This method makes it possible to easily isolate many fragments with promoter activity from filamentous fungi, and should facilitate the investigation of the promoter structures necessary for the expression of fungal genes.

    Notes: Abstract Fusarium sporotrichioides and Gibberella saubinetti O-demethylate 3,9-dimethoxypterocarpan to 3-methoxy-9-hydroxypterocarpan and then 3,9-dihydroxypterocarpan. F. anguioides Sherb., F. avenaceum and F. graminearum convert the same substrate to a mixture of 3-methoxy-9-hydroxypterocarpan and 3-hydroxy-9-methoxypterocarpan. Induction of pterocarpan degradation by pisatin in F. avenaceum leads to preferential conversion of 3,9-dimethoxypterocarpan to 3-hydroxy-9-methoxypterocarpan (medicarpin) and the isoflavan vestitol. The data are discussed with respect to phytoalexin degradation by phytopathogenic Fusarium fungi.
