    Notizen: Summary Graves' disease (GD) is a human leukocyte antigen (HLA) linked organ-specific autoimmune disease. In German GD patients the disease is associated with HLA specificities of the HLA-DRw52 family (HLA-DR3, -DR5, and DR6; HLA-DRB3 positive HLA haplotypes). Recently, a strong association with a HLA-DRB3 restriction fragment length polymorphism gene has been described. To study HLA-DRB3 alleles and their association with the disease, a large cohort of controls (n = 3724) and GD patients (n = 304) was analyzed. HLA-DR allelic combinations revealed an increase in HLA-DR3/DR5 heterozygous patients (relative risk 2.9; P〈0.001). HLA-DRB3 alleles, as defined by DNA typing in HLA-DR matched groups revealed a significant increase in DRB3*0101 homozygosity (relative risk 17.5; P〈 0.001) in HLA-DR3 homozygous patients. In GD patients with ophthalmopathy (grade II or higher, according to Werner) DRB3*0101/*0202 heterozygosity revealed an increased relative risk of 5.5 (P〈0.001). Non-HLA-DR3 homozygous, DRB3*0101/*0202 heterozygous patients were at the highest risk for endocrine ophthalmopathy (relative risk 10; P〈0.001). Our data, based on DNA typing methods of HLA-D genes, provide evidence that the susceptibility is strongly associated with HLA-DRB3 genes.

    Notizen: Abstract Objective: To evaluate the effectiveness of a continuous quality improvement (CQI) program in reducing the incidence of unplanned endotracheal extubation. Design: Prospective study over a 9-month period. Setting: Adult intensive care units (ICUs including coronary care unit, medical ICU, surgical ICU, and cardiovascular surgical ICU) in a university-affiliated medical center. Patients: 831 consecutive mechanically ventilated patients. Interventions: CQI program focusing on standardization of procedures, improvement of communication, and identification and management of high-risk patients. Measurements and results: With the implementation of this CQI program, the overall incidence density of unplanned extubation (defined as number of new unplanned extubations per mechanical ventilation patient-days) significantly decreased from 2.6% in the first trimester to 1.5% in the second trimester and 1.2% in the third trimester (p=0.01). This reduction was essentially the result of a decrease in unplanned extubation in orally intubated patients (incidence density 4.6, 1.7 and 1.0% for three trimesters, respectively; p〈0.0001). Unplanned extubation in nasally intubated patients remained largely unaffected (1.2, 1.4, and 1.4% for three trimesters, respectively; p=0.92). Conclusions: The implementation of a concerted CQI program is effective in reducing the overall incidence of unplanned endotracheal extubation.

    Notizen: Summary The nmr relaxation rate results show unequivocally that there are at least two fractions of tissue water in both normal and edematous white matter which do not exchange on an nmr time scale (i.e. at times of the order of milliseconds to fraction of a second). In conjunction with the electron microscopic determination of the extracellular volumes of normal and edematous white matter, the relaxation results can be interpreted in terms of the following model. The two slowly-exchanging water components giving rise to the non-exponential relaxation correspond to cellular and extracellular water; edema changes the relaxation rate of the extracellular component much more than that of the cellular component (the extracellular component becoming more “liquid-like” in its relaxation). Such behavior is consistent with the properties of the extracellular water being due to rapid exchange between motionally restricted water adsorbed at the surface of myelin sheaths, and relatively unrestricted, bulk water. Edema presumably increases the relative amount of the “bulk” water between the axons.
