    Notes: Abstract From 1980 to 1991, 59 patients with advanced urothelial cancer (pathological stage, 〉pT3) underwent radical operations. Of these 59 patients, 33 had nodal involvement. This study focused on those 33 patients with nodal involvement. The primary site was the urinary bladder in 20 patients and the upper urinary tract (renal pelvis and/or ureter) in 13. In all, 13 patients underwent adjuvant chemotherapy with an M-VAC or M-VEC [methotrexate (M), vinblastine, doxorubicin (ADM) or epirubicin, and cisplatin (CDDP)] regimen, and another 8 patients were treated with other insufficient chemotherapies [CDDP+ADM or CDDP+ADM_etoposide (VP-16)]. A group of 12 patients did not receive any additional treatment. Most of the patients in the M-VAC and M-VEC groups received more than 2 cycles of the regimen (median, 3.2 cycles; range, 1–9 cycles). The overall 5-year survival rate of the M-VAC and M-VEC group was 31%, whereas the rate was 0 for the other insufficient-chemotherapy groups and the no-chemotherapy group. Of the 13 patients in the M-VAC group, 4 (31%) patients were alive without disease progression and 9 (69%) were dead due to progressive disease. In the other groups, only I patient was alive without progression. Our results suggest that adjuvant M-VAC or M-VEC chemotherapy may extend the median survival of patients with advanced urothelial cancer, but it failed to reduce the rate of cancer death.

    Notes: Abstract This paper presents a novel parallel processing system for image synthesis using ray tracing. An object space is divided into parts (subspaces), each of which is allocated to a processor. The processor detects, simultaneously the intersections of the surfaces of each object and a fixed number of rays over the whole space, and calculates the local intensity on an object in each subspace. The global intensities of pixels on a screen are calculated by the other kind of processors simultaneously. We also present the optimal data structure, based on an adaptive division algorithm, for parallel processing of the object space.

    Notes: Abstract Static and dynamic load balancing strategies for a multiprocessor system for a ray tracing algorithm based on constant subdivision are presented. An object space is divided into regular cubes (subspaces), whose boundary planes are perpendicular to the coordinate axes, and these are allocated to the processors in the system. Here, load balancing among the processors is the most important problem. Firstly, in a category of static load balancing, strategies for mapping the subspaces into the processors are evaluated by simulation. Moreover, we propose a hierarchical multiprocessor system in order to realize dynamic load balancing with the static one. Its architecture can overcome the limitation of the static load balancing in a large scale multiprocessor system.

    Notes: Abstract In a patient with metachronous multiple primary lung cancer, bilateral lobectomy was performed, using bronchoplastic procedures. This case may be one of very few such cases reported in the literature. A 56-year-old man with squamous cell carcinoma was surgically treated for lung cancer. At the first operation, right upper lobectomy with wedge resection of the right main bronchus was performed, as the tumor occupied the orifice of the right main bronchus. Six months later, re-operation for stricture at the anastomotic line was done because of granulation. By means of sleeve resection of the strictured right main bronchus, the airway was reconstructed. The patient remained well for five years, then a similar cancer at the orifice of the left lower lobe and bulging into the left main bronchus became evident. Left lower sleeve lobectomy was done for the second primary cancer. The postoperative course was uneventful and he is well with no signs of recurrence 6 years and 10 months after the first operation and 19 months after the second sleeve lobectomy

    Notes: Abstract Eleven pediatric Wolff-Parkinson-White (WPW) syndrome patients underwent surgery. Four had left, 5 right cardiac type and 2 had right septal type WPW syndrome. Two patients had 2 accessory conduction pathways (ACP). Ebstein's anomaly and secundum type atrial septal defect were the associated congenital cardiac diseases in one patient each. Indications for surgery included repeated and/or long-lasting paroxysmal supraventricular tachycardia (PSVT), ineffective drug therapy, cardiac failure due to frequent tachycardia, short effective refractory period of the ACP, and simultaneous surgery for associated congenital cardiac diseases. Pre- and intra-operative examinations, including ECG, VCG, UCG, body surface mapping, intracavitary recording by catheter electrodes, computerized epicardial mapping, and endocardial mapping, were performed for the precise localization of the ACP. The surgical method was basically the same as is used for adults. Anterior median stermotomy was used primarily in right cardiac and right septal type and left anterior thoracotomy was used in 3 of 4 cases of the left cardiac type. Eight of 11 cases, two of which had 2 ACPs, were completely cured and in 3 there was evidence of postoperative pre-excitation. However, the PSVT attacks disappeared almost completely and drug therapy is not required at present.

    Notes: An investigation has been carried out to determine the influence of operating conditions on the character of the final product resulting from the polymerization of ethylene in a radio-frequency discharge. It has been found that low pressures and high ratios of discharge power to gas flow rate lead to the deposition of a powder as well as a film. From visual observations of powder deposition, it has been possible to divide the plane formed by the axis of pressure and flow rate into a region in which powder is deposited together with a film and a region in which only a film is deposited. Numerous physical measurements have been carried out in order to characterize the powder. From these measurements, it has been concluded that the powder is a relatively dense, amorphous, and highly crosslinked polymer composed of short-chain segments containing no more than two adjacent methylene groups. Many forms of unsaturation have been observed which, together with the high crosslink density, explain the measured hydrogen-to-carbon ratio of 2.7 to 2. Evidence of oxidation has been observed through the appearance of hydroxyl and carbonyl bands in the infrared spectrum of the polymer. A mechanism for the formation of the powder has been proposed which suggests that the polymerization of the powder takes place totally in the gas phase.
