    Notes: An 8-week survey was conducted to determine whether the introduction of low-flow anaesthesia (a fresh gas flow of 4 litres/minute or less) into routine use would be acceptable to members of a representative anaesthetic department and if the consequent reduction in use of volatile anaesthetics would result in financial savings. The hourly consumption of the volatile agents was measured during anaesthesia conducted using either conventional or low fresh gas flows. Anaesthetists' acceptance of low-flow anaesthesia was assessed using a questionnaire. Data were gathered on 286 patients undergoing inhalational anaesthesia for routine operative procedures. A 54.7% reduction in the consumption of isoflurane and a 55.9% reduction in that of enflurane was found. Of the 28 anaesthetists at the hospital, 21 would use low-flow anaesthesia routinely. The routine use of low-flow anaesthesia would therefore be acceptable and could result in annual savings of £26870 at Northwick Park Hospital.

    Notes: The practice of warming blood for transfusion by immersion in a water bath has been studied. Blood, 3 days after collection, with CPD-adenine anticoagulant, was warmed at 25°C, 37°C and 45°C for up to 2 hours. The procedure was repeated after 2 and 4 weeks' storage at 4°C. Only trivial biochemical and morphological changes resulted from warming. Within these limits, immersion warming of blood appears to be a reasonable alternative to using a blood-warming coil. Other aspects of immersion warming are discussed.

    Notes: The Datex ‘Normac’ infrared anaesthetic agent monitor has been evaluated for accuracy of calibration, stability and speed of response. Accuracy was assessed by comparing with a gas chromatograph calibrated using volumetric standards and was found to be very good for isopurane and enflurane but was slightly less accurate for halothane, due to a low signal: noise ratio. The stability of the instrument was found to be exceptionally good. The response time is dependent on the sampling rate and we found that, in practice, the instrument would record the end expired anaesthetic concentration on patients breathing up to 15 breaths/minute. The instrument is compact and convenient for use in operating theatres.

    Notes: When a turbulent flow of a carrier gas is passed over a liquid anaesthetic agent contained in a vaporiser of Goldman design, evaporation from the cooled surface leads rapidly to a succession of fluid instabilities which set in at characteristic critical conditions. An initially quiescent boundary layer at the surface is sequentially replaced by a thin layer of toroidal (Bénard–Marangoni) convection cells which are driven by surface tension gradients. These are then augmented by Rayleigh–Bénard convection driven by gravity, the whole process terminating in intermittent columnar plunging of cold fluid from a chaotic surface layer of pulsating thickness to the base of the liquid pool. Residual striations from these plunging columns persist throughout the bulk of the liquid so long as evaporation continues. The ultimate state is then one in which turbulence occurs throughout both liquid and vapour phases. In this paper, a semiquantitative analysis of the system dynamics is given with supportive experimental evidence where possible.

    Notes: Ninety patients scheduled to undergo minor gynaecological surgery were divided into three groups. Group 1 received propofol only, for both induction and maintenance of anaesthesia. Group 2 were given propofol for induction and maintenance but inhaled 66% nitrous oxide in addition. Group 3 had propofol for induction only and were given nitrous oxide and enflurane thereafter. The incidence of nausea in group 1 was 0%, in group 2, 3.4% and in group 3, 9.4%. No patient vomited.

    Notes: The Lamtec agent monitor is a compact anaesthetic analyser designed to measure halothane, isoflurane and enflurane. It shows good linearity and stability. The faster model can be used for end-tidal measurements up to 25 breaths per minute. Calibration using a standard of the gas to be measured is recommended.
