    Notes: A new prostaglandin analogue, 16, 16-dimethyl-trans-Δ2-PGE1 methyl ester (16 me E1) was used for preoperative cervical dilatation in 23 pregnant women who had abortion induced for socio-economical reasons in the first trimester of pregnancy. A single vaginal pessary containing 1 mg 16 me E1was inserted into the posterior fornix at 3-hour intervals. Thirteen (56.5 per cent) of the 23 patients had complete abortions and two patients (8.7 per cent) had incomplete abortions. In six patients (26.1 per cent) the cervix was dilated to at least 10 mm and the remaining two patients (8.7 per cent) required mechanical dilatation at the time of vacuum evacuation. Apart from mild gastrointestinal symptoms, no serious side effects were noted. It is concluded that the new prostaglandin analogue, 16 me E1, could be used safely and effectively for preoperative dilatation of the cervix before surgical abortion in early pregnancy.

    Notes: Summary. Feedback of oestrogen on gonadotrophin release was studied in five normally cycling normoprolactinaemic women who were rendered moderately hyperprolactinaemic by oral administration of sulpiride (150mg daily) for 11 consecutive days. Five patients with peptic ulcer, who had been given 150 mg of sulpiride for 3–18 months and had chronic marked hyperprolactinaemia were also studied. Ethinyl oestradiol (200μg) was administered by mouth daily for 4 consecutive days and serum concentration of prolactin, luteinizing hormone (LH), follicle- stimulating hormone (FSH) and oestradiol were measured by a doubleantibody radioimmunoassay for 11 days. Oral administration of ethinyl oestradiol caused an initial decrease followed by a rebound of LH with a peak in all the women tested. The serum FSH levels showed only the initial decrease, followed by a slight rebound. Serum oestradiol levels decreased during oral administration of ethinyl oestradiol in all the women. The results of the present investigation suggest that oestrogen feedback effect on LH release is maintained in sulpiride-induced hyperprolactinaemic women as well as in women with normal cycles.

    Notes: Summary. Dehydroepiandrosterone sulphate (DHEA-S) was given intravenously twice a week after 38 weeks gestation to ripen the uterine cervix in 20 nulliparae and 24 parous women who subsequently went into labour at term. The condition of the uterine cervix was assessed by Bishop scoring. The increase in the score in nulliparous women given DHEA-S was significantly greater than that in the control group, who were given placebo injections, on the 7th, 14th and 21st days. The increase in the parous women was significant only on the 7th day. In nulliparous women the dilatation, effacement and consistency improved significantly, while in the parous women the effect of DHEA-S was apparent only in the consistency. The duration of labour was significantly shortened by treatment with DHEA-S in nulliparae but this effect was not found in parous women. The interval between the first administration of DHEA-S and the onset of labour in nulliparae was significantly shorter than that in the control group. There were no serious side effects or complications. Our results suggest that DHEA-S may be used safely and effectively to ripen the uterine cervix and to trigger the onset of labour in late pregnancy.

    Notes: The pituitary responsiveness to synthetic luteinizing hormone releasing hormone (LH-RH) in female patients with primary and secondary hypogonadism was tested and compared with that in normal women with regular menstrual cycles. A 200 μg dose of synthetic LH-RH was injected intravenously into 15 normal menstruating women, 8 patients with primary hypogonadism and 5 patients with secondary hypogonadism and the serum luteinizing hormone (LH) and follicle stimulating hormone (FSH) response to LH-RH was investigated using double-antibody radioimmunoassay. In normal women, the LH response to LH-RH in the pre-ovulatory phase was greater than that seen in the follicular (p〈0.05) and luteal phases (p〈0.05) of the menstrual cycle. In contrast, no significant difference in the FSH response was demonstrated among the three different phases of the cycle. The serum LH response to LH-RH in primary hypogonadism was greater than that in the follicular (p〈0.01) and luteal phases (p〈0.0l) of the cycle. The serum FSH response in primary hypogonadism was markedly greater than that in the follicular (p〈0.01), preovulatory (p〈0.0l) and luteal phases (p〈0.0l) of the menstrual cycle. Conversely, the LH levels after LH-RH administration in secondary hypogonadism were significantly lower than in the follicular (p〈0.01), preovulatory (p〈0.0l) and luteal phases (p〈0.01) of the cycle. The FSH response in secondary hypogonadism was slightly below that in the follicular phase (p〈0.05). Thus, the FSH response to LH-RH in primary hypogonadism and the LH response in secondary hypogonadism seemed to be sufficiently characteristic to have diagnostic value.

    Notes: Summary. Hyperprolactinaemia was found in 15 of 135 infertile patients with regular menstrual cycles, biphasic basal body temperature record and no galactorrhoea. In those 15 women, mean serum prolactin levels during the mid-follicular and mid-luteal phases of the menstrual cycle were 29.8 (SEM 1.8) ng/ml and 29–5 (SEM 1.3) ng/ml, respectively. Although serum FSH and LH levels were similar in normal and hyperprolactinaemic women, serum oestradiol level during the mid-follicular phase was subnormal in hyperprolactinaemic women (P〈0.05). In contrast, serum oestradiol and progesterone levels during the mid-luteal phase and luteal phase length were similar in nor-moprolactinaemic and hyperprolactinaemic groups. The results suggest that Hyperprolactinaemia is associated with defects of follicle development as measured by oestradiol production during the mid-follicular phase, but not with corpus luteum function as measured by progesterone production during the mid-luteal phase, and luteal phase length.
