Zusammenfassung Von 1968–1981 wurden 136 Patienten mit Oesophagus-Carcinom einer Oesophagus-Resektion und Oesophago-Gastrostomie unterzogen (119 Männer, 17 Frauen, 39–76 Jahre, $$\mathop x\limits^--$$ = 58,8, Plattenepithel-Carcinom 88, Adeno-Carcinom 48). Die Anastomose wurde bei 74 Patienten supraaortal (zumeist rechtsthorakal) und bei 48 Patienten infraaortal angelegt. Die Hospitalletalität betrug 14,8%. Von den 116 Überlebenden lebten nach 2 Jahren noch 48% und nach 5 Jahren 11%, von den Patienten mit sog. kurativer Resektion (entsprechend Operations-Befund und Histologie) nach 2 Jahren noch 66% und nach 5 Jahren 22%. Patienten mit hoher Anastomose (rechtsthorakal) waren zu 80% beschwerdefrei, mit tiefer Anastomose zu 64%, bedingt durch geringere Refluxincidenz (15% vs. 28%). Das derzeitige Regime beinhaltet: Carcinome im mittleren und unteren Drittel: Oesophguo-Gastrostomie mit Anastomose supraaortal, rechtsthoracal, Carcinome im oberen thorakalen Drittel: thorakale Oesophagektomie und Colon-Bypass, Kardia-Carcinom: totale Gastrektomie, distale Hemioesophagektomie und Jejunum- bzw. Colon-Interposition.

Summary Between 1968 and 1981, 136 patients underwent esophageal resection and esophagogastrostomy for esophageal carcinoma (119 men, 17 women, age 39–76 years, mean 58.8 years, squamous cell ca. 88, adeno ca. 48). Location of anastomosis was supraaortic in 74 patients (mostly right chest approach) and infraaortic in 48 patients. Hospital mortality was 14.8%. Of 116 hospital survivors, 48% were alive after 2 years and 11% after 5 years; of those patients with a ‘curative resection’ (operative findings and histology), 66% were alive after 2 years and 22% after 5 years. Whereas 80% of the patients with a high anastomosis (right chest approach) were free of symptoms, this was true in only 64% of the patients with a more distal anastomosis, mainly depending on the incidence of reflux (15% vs 28%). Our present regime includes: midthoracic and lower thoracic ca.: supraaortic esophagogastrostomy, upper thoracic ca.: thoracic esophagectomy and colon bypass, cardia ca.: total gastrectomy, and distal hemiesophagectomy and jejunum- or colon interposition.
