Abstract For the early detection of prostate cancer, a patient should undergo digital rectal examination (DRE) and PSA investigation. Follow-up of increased PSA levels detects nearly 80% of cancers. Positive predictive value of suspicious DRE and PSA is about 50%. Whereas in the first evaluation of this case-finding trial about 70% of the patients had organ-confined cancers, nearly all of the detected cancers in the follow-up investigation were organ confined. The increased number of organ-confined cancers detected in early periodical examinations can lead to a reduction of mortality and morbidity from prostate cancer.

Zusammenfassung Die Früherkennungsuntersuchung muss im Sinn einer qualitätsgesicherten Untersuchung geprüft und standardisiert angeboten werden. Die Bestimmung des prostataspezifischen Antigens (PSA) erhöht die Fallfindung von nichttastbaren Prostatakarzinomen, sog. T1c-Tumoren, bis zu 7fach. Sind beide Untersuchungen, digitorektale Untersuchung (DRU) und PSA-Wert auffällig (PSA-Schwellenwert 〉4,0 ng/ml), liegt der positiv-prädiktive Wert (PPW) der kombinierten Untersuchung bei 50%. Bei jedem 2. Mann mit dieser Befundkonstellation wird somit durch eine Biopsie der Prostata ein Karzinom entdeckt. Rund 60% der organbegrenzten Karzinome sind allein durch das PSA entdeckt worden. Während 1997 70% der operierten Patienten ein pT2-Stadium aufwiesen, zeigten 1998 nahezu alle Patienten ein organbegrenztes Tumorstadium in der definitiven Histologie. Die Sicherung der Diagnose eines Prostatakarzinoms erfolgt standardisiert durch die Sextantenstanzbiopsie nach Möglichkeit ultraschallgesteuert. Ein auffälliger Tastbefund wird zusätzlich biopsiert.
