Fragestellung: Die Therapie der Zytomegalievirusretinitis bei Aids-Patienten erfolgt bisher systemisch oder lokal durch wöchentliche intravitreale Injektionen mit Ganciclovir oder Foscarnet. Durch einen intravitreal implantierten Ganciclovirmedikamententräger steht nun eine neue Möglichkeit der lokalen Therapie zur Verfügung. Wir haben untersucht, inwieweit sie geeignet ist, eine Progredienz der CMV-Retinitis auch ohne zusätzliche systemische Therapie zu verhindern, und welche Risiken und Nebenwirkungen mit ihr verbunden sind. Patienten und Methode: In unserer Klinik wurden 28 Patienten mit 46 Implantaten versorgt. Bei Auswahl zeigten alle eine vortherapierte CMV-Retinitis. Am Operationstag wurde die systemische Medikation abgesetzt. Ergebnisse: Ernste perioperative Komplikationen traten nur in 1 Fall in Form einer Ablatio retinae auf. Bis auf wenige Ausnahmen blieben die Patienten durch das Ganciclovirimplantat seit der Erstdiagnose CMV-Retinitis auch ohne systemische Therapie rezidivfrei, durchschnittlich 8,1 Monate. Dies bedeutet eine Verdoppelung der rezidivfreien Zeitspanne im Vergleich zur systemischen Therapie. 35 % (n = 17) der primär einseitig betroffenen Patienten entwickelten eine CMV-Retinitis am Partnerauge. Sie konnte nach Implantation eines Pellets auch in das 2. Auge ohne zusätzliche systemische Therapie beherrscht werden. Überraschend, daß nur bei 20 % (n = 28) unserer Patienten extraokulare CMV-Manifestationen beobachtet wurden. Schlußfolgerung: Das Ganciclovirimplantat erwies sich als eine zuverlässige und gleichzeitig komplikationsarme Therapieform der CMV-Retinitis. Es erzielt in aller Regel eine deutlich längere Rezidivfreiheit als dies unter systemischer Therapie beobachtet wurde. Für Arzt und Patient bedeutet diese Langzeittherapieform eine Erleichterung in der lokalen Anwendung. Die Nebenwirkungen der Medikamente sind dabei nur gering ausgeprägt, so daß wir bei klinisch rein okularer CMV-Infektion das Ganciclovirpellet als Therapie befürworten können.

Background: Cytomegalovirus (CMV) retinitis with AIDS has been treated either systemically or locally by weekly intravitreous injections. An intraocular device now offers a new therapeutic approach. We investigated its efficacy in preventing progression of CMV retinitis without additional systemic therapy. Conversely, we also studied the risks and disadvantages of this method of drug administration. Patients and methods: In our study 46 devices were implanted in 28 patients. All patients were pretreated with systemic medication. Systemic treatment was stopped on the day of surgery. Results: Severe perioperative complications occurred in one patient, who developed retinal detachment after surgery. Most patients showed no relapse of retinitis with the implant, though they did not receive systemic treatment for 8.1 months on average. Only 20 % of our patients presented with extraocular CMV disease. Thirty-five percent (n = 17) of patients with unilateral retinitis developed CMV retinitis in the primary uninvolved fellow eye. After implantation of a device into this eye also progression could be stopped without additional systemic treatment. Two patients showed progression of retinitis due to an empty ganciclovir reservoir. A second device was implanted without removal of the first. Conclusions: The intraocular ganciclovir device appears to be an effective treatment for CMV retinitis with few disadvantages. Time to progression of retinitis tends to be prolonged compared to systemic treatment.
