Zusammenfassung Es wird ein auf der langen Kehlkopfmuskulatur gestielter Insellappen beschrieben, mit dem nach Erweiterung der trachealen oder laryngotrachealen Stenose über einem Silikonrohr die Luftröhre wieder verschlossen werden kann. Von vier so behandelten Patienten konnten inzwischen drei Patienten mit annähernd normalen Lumenweiten entlassen werden.

Summary After treatment of the tracheal or laryngo-tracheal stenosis and forming a groove or using Rethi's operation, putting a full thickness skin graft or mucosa into the posterior wall, we use a silicone rubber tube for dilatation. An island flap, pedicled upon the long muscles of the larynx is used for closing the defect in the anterior wall of the trachea. We treated four patients with this method, three left hospital with normal diameter of trachea and resistance between 17 and 20 mm H2O/l/s.
