    Notes: Summary Esser described in 1918 a bi-lobed flap for reconstruction of a nose tip defect. We use this flaps nowadays to close little defects in nose, cheek and lip region. If we have to close large defects we use large “bi- and tri-lobed flaps”, if necessary we use the large regional flaps of neck, shoulder and thorax in this way.

    Description / Table of Contents: Zusammenfassung Es wird ein auf der langen Kehlkopfmuskulatur gestielter Insellappen beschrieben, mit dem nach Erweiterung der trachealen oder laryngotrachealen Stenose über einem Silikonrohr die Luftröhre wieder verschlossen werden kann. Von vier so behandelten Patienten konnten inzwischen drei Patienten mit annähernd normalen Lumenweiten entlassen werden.
    Notes: Summary After treatment of the tracheal or laryngo-tracheal stenosis and forming a groove or using Rethi's operation, putting a full thickness skin graft or mucosa into the posterior wall, we use a silicone rubber tube for dilatation. An island flap, pedicled upon the long muscles of the larynx is used for closing the defect in the anterior wall of the trachea. We treated four patients with this method, three left hospital with normal diameter of trachea and resistance between 17 and 20 mm H2O/l/s.

    Description / Table of Contents: Zusammenfassung Für die Mikrolaryngoskopie wurde das Keinsasser-Laryngoskop seitlich aufgetrennt, über Spindeltriebe ist der Durchmesser zwischen 15 und 26 mm verstellbar. Die untere Spatelhälfte ist abklappbar gestaltet. Es wird kurz der Arbeitsplatz beschrieben. Die bei uns an über 1000 Patienten erprobte Injektionsbeatmung zeigt auch beim offenen Rohr normale Blutgaswerte. Die Kombination der Vorteile des Kleinsasser-Rohres mit der Killianschen Schwebelaryngoskopie bietet durch die laterale Schlitzung des Rohres wesentlich mehr Platz für die mikrochirurgischen Instrumente.
    Notes: Summary A new laryngoscope was constructed in order to improve visualization of the larynx and to adjust the instrument to individual situation. The Kleinsasser laryngoscope was divided into two parts. The width between these two halfs can be changed by screws. Because the laryngoscope is open laterally there is more space for the operator and shorter instruments can be used.

    Notes: Summary For better working conditions to the endoscopist we developed a closed jet-ventilation system, combining open jet-ventilation and conventional ventilation. With a pressure-analizer and transducer we could prevent negative and high pressure in the trachea, removing air by suction during expiration time. The first clinical experiences, the bloodgas analizes and the circulatory parameters are discussed. This ventilation was used in tonsillectomy, chordectomy and tracheal reconstruction especially sleeve resection too. For longtime ventilation we use this system with a 20 Charrière flexible tube. In this way we are able to diminish damage of the larynx and we can wait with tracheostomy a longer time.

    Notes: Abstract To evaluate the transcriptional response of skin to tissue expansion in the dog, the expression of procollagen α1(I) mRNA and procollagen α1(III) mRNA were analyzed by in situ hybridization. This expression was evaluated in distinct skin areas (subepidermal zone, dermis, capsular zone) after 4–85 days of expansion. Within the first 4 days of expansion expression of procollagen α1(I) and α1(III) mRNA was not affected in the subepidermal and dermal zone. Only a slightly elevated level of type III procollagen mRNA was demonstrated in tissue surrounding the implanted silicone expander. After 7 days of expansion an enhanced level of procollagen α1(III) mRNA was observed in the dermis and capsular zone. A slightly enhanced type I collagen mRNA level occurred in all zones of the dermis that was even stronger in the capsular zone after 9 days of expansion. Concurrently, the number of transcriptionally active cells was significantly higher. Return to the basal level of procollagen α1(I) mRNA was attained after 40 days. At this time a significant expression of procollagenase mRNA was observed. Procollagen III mRNA expression reached its basal level on day 85.

    Notes: In dieser Ausgabe der “HNO” berichten U. Schröder und Mitarbeiter über “Die Wertigkeit der Feinnadelpunktionszytologie bei Neoplasien der Glandula parotis” [5]. Die Ergebnisse belegen eine hohe Sensitivität und Spezifität dieses diagnostischen Verfahrens. Obgleich von den Autoren erstaunlicherweise nicht durchgeführt, ist es doch wichtig zu betonen, dass die Feinnadelpunktion möglichst unter sonographischer Kontrolle erfolgen sollte, da hiermit gezielt repräsentative Tumorareale getroffen und auch kleinere, nicht palbable Knoten erreicht werden können.
