    Notes: Abstract The octapeptide neurohormone D (NHD), a member of the family of adipokinetic hormones (AKH-peptides), increases the frequency of spontaneous activity in dorsal unpaired median (DUM) neurones isolated from the terminal ganglion of the cockroach Periplaneta americana. The increase in spike frequency is accompanied by changes in the shape and the amplitude of the single action potentials, e.g. a more pronounced afterhyperpolarization. Effects of NHD on membrane currents were investigated in these DUM cells with whole-cell voltage-clamp measurements. A voltage-independent Ca2+ current flowing at the resting potential (ICa,R) was found. NHD, at nanomolar concentrations, enhanced this ICa,R in a concentration-dependent manner. 0.1 mM Cd2+markedly reduced ICa,R and in this case ICa,R was hardly potentiated by NHD. In the presence of NHD a fast activating Ca2+-dependent K+current sensitive to charybdotoxin and to low concentrations of tetraethylammonium was augmented. The enhanced afterhyperpolarization of action potentials can be accounted for by the increase in the Ca2+-dependent K+ current. The changes of the membrane currents induced by NHD are discussed with respect to further effects on the spike pattern and in relation to the previously described mode of action of AKH-peptides in other preparations.

    Description / Table of Contents: Zusammenfassung Ein 43-jähriger Patient klagte seit 2 Wochen über zunehmende Claudicatiobeschwerden im rechten Bein. Sowohl dopplersonographisch als auch angiographisch zeigte sich ein an der Aortenbifurkation beginnender Verschluß der A. iliaca communis (AIC). Initial war eine Herzkatheteruntersuchung durchgeführt wor-¶den.¶   Über einen cross-over-Zugang von der kontralateralen Seite ließ sich der Verschluss mit einem Draht überwinden. Nach Rekanalisation mit dem Excimer-Laser wurde die ipsilaterale Seite punktiert und, zum Schutz von Aorta und A. iliaca links, in „kissing-balloon”-Technik die AIC rechts dilatiert und die Beckenstrombahn mit 3 Stents versorgt. Der Patient war anschließend beschwerdefrei, klinisch und dopplersonographisch war die Durchblutung normal und der Laufbandtest unauffällig.¶   Schlussfolgerung: Auch bei direkt am Abgang aus der Aorta verschlossenem Beckengefäß lässt sich nach lasergestützter Rekanalisation in „kissing-balloon”-Technik ohne weitere Gefährdung des Patienten eine perkutane Revaskularisierung mit gutem Ergebnis durchführen. Ein gefäßchirurgisches Vorgehen ist nicht erforderlich.
    Notes: Summary A 43-year-old patient was admitted for severe claudicatio in his right leg, doppler ultrasound and angiography revealed an occluded right common iliac artery.¶   Using the cross-over-approach from the contralateral side, it was possible to navigate a guide wire through the occlusion, and after passage with an excimer laser an ipsilateral retrograde puncture with placement of a sheath was performed. In order to protect the the aortoiliac bifurcation, a primary stent implantation of the occluded side was carried out using the kissing balloon technique. Afterwards three stents were placed. Subsequently the patient completely improved; treadmill exercise test and duplex sonography showed good results.¶   Conclusion: Using the kissing balloon technique, laser-assisted recanalisation of iliac artery occlusions even involving the aortoiliac bifurcation compare favorable to other more invasive treatments. A surgical procedure can thus be avoided.
