ZusammenfaBung Als Erweiterung der diagnostischen Cholangiographie hat sich die endoskopische Papillotomie entwickelt, die wir bei 516 (94,7%) von 545 Patienten erfolgreich durchführen konnten. Die Hauptindikationen waren die Choledocholithiasis (405 Fälle) und die organische Papillenstenose (97). 376 (93 %) von 405 Patienten mit Choledocholithiasis wurden nach dem endoskopischen Eingriff steinfrei. Es traten insgesamt 38 Komplikationen auf, die bei 11 (2%) Patienten eine Notfalloperation erforderlich machten, 5 Patienten verstarben (Letalität 1 %). Die schwerwiegendste Komplikation war die Cholangitis. Es zeigt sich, daB die endoskopische Papillotomie den chirurgischen Zweitangriff z. T. ersetzen kann.

Summary Endoscopic papillotomy is a further development of endoscopic cholangiography. This therapeutic procedure was succesful in 516 (94.7%) of 545 cases. Choledocholithiasis (405 cases) and papillary stenosis (97) were the main indications for the procedure. Of 405 patients with choledocholithiasis, 376 (93%) are now free of stones. There were 38 (7.4%) complications and 5 (1%) of these had a lethal outcome with (11) (2%) requiring emergency laparotomy. Cholangitis was the most serious complication. It is becoming increasingly evident that the endoscopic papillotomy is replacing reoperation in many cases of choledocholithiasis.
