Zusammenfassung Retrospektiv wurden jeweils 111 Polytraumatisierte mit und ohne Splenektomie untersucht. Verletzungsschwere und Verletzungsmuster waren gleich. Untersucht wurde die Zahl der positiven Blutkulturen, die Symptome des multiplen Organversagens (MOV), die Letalität und die Todesursache.Ergebnisse: 1. nach Splenektomie fanden sich signifikant mehr positive Blutkulturen (35%) als ohne Splenektomie (20%). 2. Symptome des multiplen Organversagens waren in beiden Gruppen gleich (septiforme Reaktion). 3. Sepsis war bei Splenektomierten häufiger Todesursache als bei nicht Splenektomierten.Schlußfolgerung: Eine Splenektomie bei polytraumatisierten Patienten steigert das Risiko der frühen Sepsis. Auch bei Polytraumatisierten sollte die Milz erhalten werden. Eine verletzte Milz ist bei unstabilen Kreislaufverhältnissen zu entfernen.

Summary In a retrospective study, 111 multiple traumatized patients who were treated with splenectomy were compared with 111 non-splenectomized patients. The severity and patterns of injury were equal. The factors examined were the number of positive blood cultures, symptoms of multiple organ failure (MOF), mortality and the cause of death: (1) The incidence of bacteriaemia was significantly higher after splenectomy in multiple injured patients (35% positive blood cultures compared to 20% without splenectomy); (2) MOF symptoms were no different in either group as MOF is probably more related to “septic” reactions; (3) The clinical diagnosis of “sepsis” as the cause of death was more common in splenectomized than in non-splenectomized patients. Splenectomy increases the risk of early sepsis after multiple trauma. If possible the spleen should be saved in multiple trauma, but an injured spleen should be removed if the circulation is unstable.
