    Notes: Abstract The aim of this study was to determine MRI findings in patients successfully operated upon for lumbar disc herniation. We investigated 20 patients with a successful outcome after L4-5 or L5-S1 disc operations clinically and with MRI preoperatively, and at 5 days, 6 weeks, and 4 months after surgery. Postoperatively, T1- and T2-weighted images were obtained. At 4 months gadolinium-enhanced images were added. Pronounced intraspinal MRI changes were seen during follow-up. Deformation of the dural sac was seen in 13 patients preoperatively, in 19 at 5 days after operation, in 15 at 6 weeks, and in 12 at 4 months. Nerve root involvement was seen in all cases both preoperatively and at 5 days after operation, in 17 at 6 weeks, and in 15 at 4 months. No correlation between symptoms or the straight leg raising test and the size or nature of the abnormal tissue in the spinal canal postoperatively could be demonstrated. It was concluded that early postoperative MRI after lumbar discectomy must be interpreted carefully, and that oedema and scar formation are probable reasons for difficulties in interpretation.

    Notes: Summary In a prospective and consecutive study we evaluated the prevalence of pain-related symptoms, the results of the strainght leg raising (SLR) test and neurological disturbances by age group in a total of 150 patients operated on due to lumbar disc herniation. On admission, all patients were interviewed, and pain at rest, at night and on coughing was recorded. Walking capacity was recorded under four categories:1–5 km, 0.5〈1 km and〈0.5 km. Results of the SLR test were also registered as category data: positive 0–30°, positive 30–60°, positive 〉60° or negative. Findings from examination of tendon reflexes and power of the extensor hallucis longus (EHL) muscle were registered, as were sensory disturbances. The above mentioned parameters were analysed separtely for five different age groups: 20–29 years, 30–39 years, 40–49 years, 50–59 years and above 60 years of age. There was an age-related change in the prevalence of certain parameters. Highly restricted positive SLR test results and pain on coughing was most commonly found in the youngest patient group. With increasing age there was a decreasing prevalence of highly restricted positive SLR test results, while the prevalence of severe reduction of walking capacity increased. In short, the youngest patient group showed the most obvious clinical picture of disc herniation and, with increasing age, the clinical picture gradually changed towards the picture associated with spinal stenosis.

    Notes: Summary Post-shift and next-morning urine was sampled from workers exposed to hexahydrophtalic anhydride (HHPA), an epoxy hardener, sensitising at low exposure levels. Exposure levels of HHPA in air gas chromatography, GC) in the range of 30–270 μg/m3 corresponded to urinary concentrations of 0.9–2.8 μmol hexahydrophthalic acid (HHP acid; GC-mass spectrometry)/mmol creatinine. In the morning samples the concentrations were 〈0.04-0.3 μmol HHP acid/mmol creatinine. In unexposed controls, the level was 〈0.1 μmol/mmol creatinine. A correlation was found between the time-weighted levels of HHPA in air and HHP acid in the post-shift urine (r s = 0.93; P 〈 0.023), indicating that the determination of HHP acid in urine is suitable for biologic monitoring of HHPA exposure.

    Notes: Abstract Objectives: To clarify whether the intensity of exposure to organic acid anhydrides (OAAs) is associated with the risk of sensitisation to these allergens. Methods: The investigations were carried out in three different manufacturing plants (A, B, and C) where OAAs were used in the production of epoxy resins. Methyltetrahydrophthalic acid anhydride (MTHPA) was used in all three plants. The exposure assessment included stationary and ambient air monitoring (OAAs in the air) and biological monitoring (metabolites in urine). In plant A 20, in plant B 86 and in plant C 113 employees were examined by a physician (anamnesis, skin-prick test, specific IgE, spirometry). In plants B and C, the exposure areas were classified as high, medium, and low, without the results of the exposure assessment being known. Results: The ambient air concentrations (in μg/m3) of MTHPA were 37.2 and 58.5 in plant A (number of samples n=2), ranged from 〈0.5–26.2 in plant B (n=5) and from 2.1–57.9 in plant C (n=3) with stationary air collecting, and from 8–45 (n=6), from 〈4.7–35.7 (n=3) and from 2–37.8 (n=3) with personal air collection. The metabolites of OAAs in urine (in nmol/mmol creatinine) ranged from 5.7–645 (median of MTHPA: 346) in plant A, from 〈1–213 (median of MTHPA: 10.1) in plant B and from 0.1–830 (median of the sum of the OOA metabolites: 108.6) in plant C. The prevalence of sensitisation was 35% in plant A, 21% in plant B and 29% in plant C. A higher prevalence in the highly exposed areas, however, could not be seen. Levels of IgE specific for conjugates of MTHPA were not associated with the metabolites in the end of shift urine. Levels of IgG specific for conjugates of MTHPA, however, were associated with the metabolites in the end of shift urine. Conclusions: The data showed that biological monitoring is a useful tool in the exposure assessment of OAAs. Comparing the prevalence of sensitisation and the results of biological monitoring, between the three plants, we found that sensitisation increased with increasing exposure. Within a plant a higher risk of sensitisation in persons working in highly exposed areas at the time of the examination could not be seen, possibly due to frequent job rotation.

    Notes: Abstract Objective: The principal aim of the study was to estimate the level of exposure to organic solvents of graffiti removers, and to identify the chemicals used in different cleaning agents. A secondary objective was to inform about the toxicity of various products and to optimise working procedures. Methods: Exposure to organic solvents was determined by active air sampling and biological monitoring among 38 graffiti removers during an 8-h work shift in the Stockholm underground system. The air samples and biological samples were analysed by gas chromatography. Exposure to organic solvents was also assessed by a questionnaire and interviews. Results: Solvents identified were N-methylpyrrolidone (NMP), dipropylene glycol monomethyl ether (DPGME), propylene glycol monomethyl ether (PGME), diethylene glycol monoethyl ether (DEGEE), toluene, xylene, pseudocumene, hemimellitine, mesitylene, ethylbenzene, limonene, nonane, decane, undecane, hexandecane and γ-butyrolactone. The 8-h average exposures [time-weighted average (TWA)] were below 20% of the Swedish permissible exposure limit value (PEL) for all solvents identified. In poorly ventilated spaces, e.g. in elevators etc., the short-term exposures exceeded occasionally the Swedish short-term exposure limit values (STEL). The blood and urine concentrations of NMP and its metabolites were low. Glycol ethers and their metabolites (2-methoxypropionic acid (MPA), ethoxy acetic acid (EAA), butoxy acetic acid (BAA), and 2-(2-methoxyethoxy) acetic acid (MEAA)) were found in low concentrations in urine. There were significant correlation between the concentrations of NMP in air and levels of NMP and its metabolites in blood and urine. The use of personal protective equipment, i.e. gloves and respirators, was generally high. Conclusions: Many different cleaning agents were used. The average exposure to solvents was low, but some working tasks included relatively high short-term exposure. To prevent adverse health effects, it is important to inform workers about the health risks and to restrict the use of the most toxic chemicals. Furthermore, it is important to develop good working procedures and to encourage the use of personal protection equipment.

    Notes: Summary Urinary hexahydrophthalic acid (HHP acid) levels were determined in 20 workers occupationally exposed to hexahydrophthalic anhydride (HHPA) air levels of 11–220μg/m3. The levels of HHP acid in urine increased rapidly during exposure and the decreases were also rapid after the end of exposure. The elimination half-time of HHP acid was 5h, which was significantly longer than in experimentally exposed volunteers, possibly indicating distribution to more than one compartment. There was a close correlation between time-weighted average levels of HHPA in air and creatinine-adjusted levels of HHP acid in urine collected during the last 4 h of exposure (r = 0.90), indicating that determination of urinary HHP acid levels is suitable as a method for biological monitoring of HHPA exposure. An air level of 100 μg/m3 corresponded to a postshift urinary HHP acid level of ca. 900 nmol/mmol creatinine in subjects performing light work for 8h. Percutaneous absorption of HHPA was studied by application of HHPA in petrolatum to the back skin of three volunteers. The excreted amounts of HHP acid in urine, as a fraction of the totally applied amount of HHPA, were within intervals of 1.4%–4.5%, 0.2%–1.3%, and 0%–0.4% respectively, indicating that the contribution from percutaneous absorption is of minor importance in a method for biological monitoring.

    Notes: Abstract Aims/hypothesis. The purpose of this study is to investigate the cost-effectiveness of simvastatin in diabetic patients, using prospectively collected outcomes data from the Scandinavian Simvastatin Survival Study. Methods. Diabetic patients were identified using two different classifications schemes: Clinical history (diabetic, non-diabetic) and the new American Diabetes Association definition (diabetic, impaired fasting glucose, normal fasting glucose). The analysis is based on prospectively collected data from the trial on hospitalization for cardiovascular problems, study drug utilization and mortality. The incremental cost per life year saved with simvastatin is estimated using costs from Sweden (primary) and other European countries. Results. Hospitalizations for cardiovascular problems were considerably reduced with simvastatin therapy, with the greatest differences in the diabetic subgroups. Reductions in hospitalizations in the diabetic group resulted in substantial hospital cost savings that offset 67 to 76 % of the drug cost (depending on the classification used). For the diabetic patients, the estimates of the cost per life-year gained ranged from 1600 Euros (based on clinical history) to 3200 Euros (based on American Diabetes Association) using Swedish costs. In the other evaluated European countries treatment with simvastatin showed a favourable cost-effectiveness ratio independent of differences in local health care unit costs. Conclusion/interpretation. For all subgroups in the diabetic classification schemes, treatment with simvastatin resulted in estimates of cost per life-year gained that were well within the range generally considered to be cost effective. Based on the Scandinavian Simvastatin Survival Study, simvastatin therapy provides good value for money in both diabetic and non-diabetic patients with cardiovascular disease.     Notes: Abstract The self-diffusion of small molecules in colloidal systems is calculated using the cell model to describe the effect of varying concentration of colloidal particles. The relevant boundary conditions are found using arguments from the thermodynamics of irreversible processes. From a general description of the self-diffusion in systems with spherically symmetrical particles we derive expressions for the concentration dependence of the effective self-diffusion coefficientD eff for several cases of practical importance. It is shown that when the molecule studied is strongly attracted to the particle a minimum inD eff is expected around volume fractionΦ=0.35. It is also shown that the often made distinction between free and bound molecules is often problematic and a more general description is proposed. The obstruction effect generated by the excluded volume is discussed both for spherical and spheroidal systems. It is pointed out that the often used formula due to Wang ((1954) J Amer Chem Soc 76:4755) is incorrect for self-diffusion and for the obstruction factor for spheres we obtain (1+0.5Φ)−1. This expresion is tested both by experiments on water diffusion in systems containing latex particles and through computer simulations and it is found valid over a wide concentration range. For prolate ellipsoids the obstruction factor is not greatly different from that for spheres, while for oblate aggregates the limiting obstruction factor of 2/3 can be obtained at low concentrations. It is demonstrated that this effect can be used to distinguish between different aggregate shapes. It is also shown that the disorder present in a solution of colloidal particles leads to a decrease in the obstruction effect.
