Es ergab sich, daß die Werte der beiden Methoden gelegentlich bis zu ± 0,25‰ differieren, im allgemeinen jedoch recht gut übereinstimmen. 2. Bei Untersuchung von Diabetiker-Nüchternbluten nachWidmark und nachFriedemann-Klaas zeigte sich, daß im allgemeinen nur sehr geringe „Alkohol”-Konzentrationen gefunden werden. Relativ wenige Werte liegen über 0,10‰. Auf keinen Fall wurden höhere Konzentrationen beobachtet, als sie schonWidmark angegeben hat. 3. Die Angaben vonMöllerström dürfen nicht zur Grundlage forensischer Beurteilung gemacht werden.

    Notes: Zusammenfassung 1. Es wurden Tri und Tetra als solche und in Wasser- und Blutlösungen nachWidmark untersucht. Dabei zeigte sich, daß die gefundenen Konzentrationen im Rahmen des theoretisch Möglichen liegen. 2. Bei verschieden langem Aufenthalt bei Versuchstieren in reinem Tri und Tetra sowie deren Luftgemischen lagen die scheinbaren Blutalkoholwerte nie über 0,10‰. Auch die bei der sog. Tri-Narkose entnommenen Blute hatten nur Konzentrationen unter 0,1‰. 3. Im Gehirn fanden sich dagegen etwas erhöhte Werte: Bei Tri bis 0,55‰, bei Tetra,bis 0,24‰. Man muß annehmen, daß Tri und Tetra wegen ihrer Affinität zu lipoidreichen Organen sich besonders im Gehirn anreichern. 4. Eine tatsächlich vorhandene Alkoholkonzentration wird durch gleichzeitiges Einatmen von Tri nicht scheinbar erhöht. 5. Ein festgestellter Blutalkoholgehalt von 1,80‰ kann auf keinen Fall auf das Einatmen von Tri und Tetra zurückgeführt werden.

    Notes: Zusammenfassung 1. Es wird der Barbituratnachweis aus der Haut an Hand von 10 Fällen, die aus einer größeren Anzahl Untersuchter herausgenommen wurden, gezeigt. 2. Dabei wird festgestellt, daß in der Haut nicht unerhebliche Mengen von Barbituraten nachgewiesen werden können. 3. Die aus der Haut gewonnenen Barbiturate ergeben recht reine Krystallformen bei der Sublimation, so daß die damit erzielten physikalischen Konstanten hinreichend gut sind.

    Notes: Abstract Six months after the onset of visual loss a 23-year-old male patient with Leber's optic neuropathy associated with a mitochondrial DNA mutation developed brain stem involvement with Parinaud's syndrome and oculopalatal myoclonus. Magnetic resonance imaging (MRI) revealed a high signal area in the brain stem, corresponding to a hypodense area in the CT scan that did not show contrast enhancement. Distinct diminution but not complete remission of the MRI findings was found in the 5-year follow-up, which was not accompanied by clinical improvement. Although the MRI findings were compatible with a demyelinating lesion, neither extensive evoked potential studies nor spinal fluid examination supported this.

Ein akuter Fall einer Thalliumvergiftung wird beschrieben und die dabei gefundenen Thalliummengen angegeben. 3. Im Fötus und in der Placenta einer thalliumvergifteten Schwangeren konnte Thallium nur in minimalsten Spuren nachgewiesen werden, während in Leber und Dünndarm + Inhalt noch reichlich Thallium vorhanden war.

    Notes: Abstract The effect of the short-acting benzodiazepine lorazepam on motor cortex excitability was investigated in 11 healthy volunteers using the technique of focal transcranial magnetic stimulation. The threshold intensity for evoking an electromyographic response in the resting and active abductor digiti minimi muscle, the size of the motor evoked potential, the duration of the cortical and peripheral silent periods, the corticocortical inhibition and facilitation after paired magnetic stimuli, and the transcallosal inhibition were used as parameters to assess various aspects of motor system excitability. Baseline values were compared with data obtained 2, 5 and 24 h after a single oral dose of 2.5 mg lorazepam. Resting and active motor thresholds and the size of the motor evoked potential remained unchanged. The duration of the cortical silent period was prolonged with a maximum effect 5 h after drug intake, while the peripheral silent period did not show any lengthening at that time. The corticocortical inhibition showed a tendency toward more inhibition, while the corticocortical facilitation was almost completely suppressed. The transcallosal inhibition showed an inconsistent trend to less inhibition. In parallel to the pharmacokinetics of lorazepam, all effects peaked at 2 h and 5 h, and were (partially) reversible after 24 h. It is hypothesized that most of these findings are due to the reinforcement of GABA action by lorazepam at the level of the motor cortex. The lack of effect on motor threshold and on the size of the motor evoked potential may indicate that these parameters are physiologically distinct from corticocortical excitability and the cortical silent period. The relevance of the present data in clinical epileptology is discussed.

    Notes: Abstract  To evaluate changes in the motor system during the silent period (SP) induced by transcranial magnetic stimulation (TMS) of the motor cortex, we investigated motor thresholds as parameters of the excitability of the cortico-muscular pathway after a suprathreshold conditioning stimulus in the abductor digiti minimi muscle (ADM) of normal humans. Since the unconditioned motor threshold was lower during voluntary tonic contraction than at rest (31.9±5.4% vs. 45.6±7.5%), it is suggested that the difference between active and resting motor threshold indicates the magnitude of the voluntary drive on the cortico-muscular pathway. Therefore, we compared conditioned resting and active motor threshold (cRMT and cAMT) during the SP. cRMT showed an intensity-dependent period of elevation of more than 200 ms in duration and approximately 17% of the maximum stimulator output above the unconditioned threshold, due to decreased excitability of the cortico-muscular pathway after the conditioning stimulus. Some 30–40 ms after the conditioning stimulus, cAMT approximated cRMT, indicating complete suppression of the voluntary motor drive. This suppression did not start directly after the conditioning stimulus since cAMT was still significantly lower than the cRMT within the first 30–40 ms. Threshold elevation was significantly longer than the SP (220±41 vs. 151±28 ms). Recovery of the voluntary motor drive started late in the SP and was nearly complete at the end of the SP, although thresholds were still significantly elevated. We conclude that the SP is largely due to a suppression of voluntary motor drive, while the threshold elevation is a different inhibitory phenomenon that is of less importance for the generation of the SP, at least in its late part. It is argued that the pathway of fast cortico-spinal fibers activated by TMS is partially different from the pathway involved in the maintenance of tonic voluntary muscle activation.

    Notes: Summary We have recorded postural performance in 50 HIV-infected patients in different stages of the disease (Walter Reed (WR) stages I–VI) by means of a force measuring platform. The results were compared with 50 age-matched controls. A significant instability was particularly evident when standing on an unstable foot support. In patients standing with “eyes closed”, postural sway was significantly higher in every patient group (WR I–II:P〈0.02, WR III–V:P〈0.001, WR VI:P〈0.001). Patients in stage WR I–II showed no relevant neurological abnormalities. In agreement with other neurophysiological data in the literature we suggest that postural imbalance could be an early sign of central nervous system penetration of HIV. No correlation with electromyographic or cerebrospinal fluid findings could be found.
