    Notes: Aim : To evaluate two simplified Helicobacter pylori eradication treatment alternatives for children and adolescents.Methods : Study subjects were identified by enzyme-linked immunosorbent assay and immunoblot in a family screening project. Helicobacter pylori infected 10–21 year olds were offered treatment, individuals with abdominal pain underwent upper endoscopy and those with peptic ulcers were excluded. Participants were randomized to either azithromycin 500 mg daily and tinidazole 500 mg two tablets daily in combination with lansoprasole 30 mg daily for 6 days (ATL-group) or with placebo (ATP-group). Urea Breath Test was performed at inclusion and after a minimum of 6 weeks after end of therapy.Results : In total, 131 individuals were randomized, of whom 31 (24%) had undergone upper endoscopy. Full compliance was achieved in 93% (122 of 131). The intention-to-treat eradication rate was 67% (44 of 66) and 58% (38 of 65) for the ATL- and the ATP-group, respectively.Conclusion : The double-blind randomized clinical trial did not identify a simplified, successful once daily H. pylori treatment for children and adolescents. Thus, twice daily proton pump inhibitor (PPI)-based triple therapies for 7 days remain as the choice of treatment in children. Further, powerful and controlled studies are needed to elucidate the best treatment strategies for H. pylori eradication in this age group.

    Notes: Physical perfomance was studied uniformly after intake of different antihypertensive drugs, and a comparison was made with placebo. The time needed to run 5000 m was studied in a nonhypertensive subject. In one of the different study periods, heart rate was continuously recorded during and after exercise. Physical perfomance was significantly reduced when a β-blocker was taken. The effect on physical capacity was strongly dose-dependent. With a high but conventional dose of a β-blocker, the time needed to run 5000 m was increased by more than 33%. No differences were observed between types of β-blockers. Physical capacity was slightly but significantly impaired when an α-blocker was taken; no differences in physical capacity between the drugs used and placebo were observed for a diuretic, a calcium antagonist or an inhibitor of the angiotensin-converting enzyme. The reduced physical perfomance seems to be caused by a reduction of the maximal heart rate as a result of the β-blockade. Thus, heart rate did not exceed 120 beats/min when the subject ran at maximal long-distance speed after intake of 100 mg of metoprolol, as recorded by continuous heart rate registration.

    Notes: Summary Of 1302 women aged 44–66 years in a population study in Göteborg, Sweden, in 1974–75, who were representative of women of all the ages studied in the area, 165 were taking antihypertensive drugs, mostly β-blockers and diuretics. The prevalence of sleep disturbances, nightmares, tiredness and melancholia or depression was studied in the total population sample, and a comparison was made between women who were or were not taking antihypertensive drugs. In the entire population sample no significant difference was found between the various age strata studied, although with increasing age there was a trend towards fewer complaints of nightmares, but a larger number of sleep disturbances as a whole. No difference was found between women taking or not taking various types of single-drug therapy or combinations of antihypertensive drugs.

    Notes: Summary Pafenolol, a new selective adrenergic beta1-blocking agent, has been tested for the first time in 6 hypertensive patients. After single oral doses of pafenolol 25 to 100 mg, there was a marked reduction in heart rate and systolic blood pressure during exercise tests. These effects were dose dependent. A significant positive correlation was found between the reduction in heart rate during exercise and the plasma level of pafenolol 5 hours after drug intake (correlation coefficient r=0.94). Side effects were mild and seemed to be dose dependent. It is concluded that this new beta1-blocking agent was effective in reducing blood pressure and was well tolerated.

    Notes: Summary Two years after an extensive health examination at a Swedish industry, a follow-up study was carried out in 110 employees (94% of those initially examined). The control included a history of the subject's health, a physical examination, an electrocardiogram, urine and faeces examinations and 16 chemical analyses of whole blood or serum. Except for repeat examinations of those who had had initial values outside reference values, most of the chemical analyses meant nothing, leading to unnecessary expense and possible risk of either worried or complacent participants. The history and physical and laboratory examinations, aimed at finding factors which can be improved by changing the life style seem to be most beneficial. It is concluded that extensive health examinations, including a large number of laboratory examinations which are carried out at many industries as a health control, should be critically evaluated at these industries.

    Notes: Abstract In connection with the publication of two papers on the clinical features and on the functional pattern in our material of patients with renal hypoplasia, an effort has been made to review the literature dealing with this condition. The frequency of unilateral small kidneys has been found to be about one in 500 autopsies. A kidney may be small because of congenital hypoplasia or pyelonephritic shrinkage or as a combination of both. The hypoplastic kidney is probably predisposed to infections. There are principally two kinds of hypoplastic kidneys: those with remnants of embryological development and those without. Kidneys without remnants of embryological development contain as a rule few calyces but may otherwise be of normal shape and of ordinary microscopical appearance. Primitive ducts and cartilage have been considered to be definite signs of embryological misdevelopment. Colloid cysts resembling thyroid tissue may probably be found both in hypoplastic kidneys without superimposed infections and in atrophic kidneys due to pyelonephritis. Radiologically the hypoplastic kidneys may be found to contain few calyces and the minor calyces may arise directly from the pelvis. The contralateral kidney is often enlarged. Good excretion of contrast medium favours the diagnosis of hypoplasia and makes pyelonephritic shrinkage less plausible. The artery of the hypoplastic kidney is described to be uniformly narrow in its whole length, while in pyelonephritis the mouth of the artery (the point where the renal artery arises from the aorta) has as a rule normal width, although the rest of the artery may be narrow. Split function tests have shown a good concentrating ability in hypoplasia in relation to the reduction in glomerular filtration rate, especially when compared with that in chronic pyelonephritis. Local pain, hypertension and urinary tract infections are common manifestations in renal hypoplasia. Blood pressure may be normalized after nephrectomy. Hypertension seems to be the most dangerous feature. If the blood pressure can be satisfactorily controlled, the prognosis in unilateral renal hypoplasia seems to be rather good.

    Notes: Abstract As observed in a cross-sectional population study of 1,302 women, aged 44–66 years and representative of middle-aged women living in a Swedish city, symptoms and complaints were found to be unevenly distributed in the female population. Factors such as foreign origin, low education, different kinds of isolation such as not working outside of the home, being divorced or widowed seemed to be factors which increased the risk of experiencing different symptoms and complaints. When meeting a patient with a complaint it is important to take all possible causes into consideration including socio-economic factors.
