Notes: This paper presents nine different methods and approaches useful for performing human reliability and error analysis in health care. These methods are failure modes and effect analysis (FMEA), root cause analysis (RCA), fault tree analysis (FTA), cause and effect diagram (CAED), hazard operability study (HAZOP), probability tree method, error cause removal program (ECRP), man-machine systems analysis (MMSA), and the Markov method. The applications of many methods are demonstrated through nine solved examples.

Notes: Presents five newly developed models for a Triple Modular Redundancy system having zero, one, two, three, and N voters. Expressions for reliabilities and mean time to failure with and without repair are developed. The plots for state probabilities, reliabilities and mean time to failure are shown.

Notes: This paper presents a review of published literature on robot reliability and safety. The literature is classified into three main categories: robot safety; robot reliability; and miscellaneous. Robot safety is further categorized into six classifications: general; accidents; human-factors; safety standards; safety methods; and safety systems/technologies. The period covered by the review is from 1973 to 2001.

Notes: Presents two newly developed Markov models (I and II) to perform reliability analysis of a system composed of a robot and its associated safety system. Model I presents expressions for robot reliability, mean time between failure, and state probabilities under non-repairable conditions. Similarly, Model II presents reliability, mean time to failure, and state probability expressions for a repairable robot system. Shows plots for robot system reliability and mean time to failure.

Notes: Presents a newly developed stochastic model for performingreliability and availability analyses of a repairable general standbyhuman-machine system with increasing human error rates and arbitraryfailed system repair rates. Also, the model takes into consideration theoccurrence of common-cause failures. The joint-density functionapproach, supplementary variable techniques, and linear ordinarydifferential equation methods were used to perform analyses. Developsthe expressions for steady state availability, the system time-dependentavailability, reliability and mean time to failure.

Notes: This paper presents two mathematical models to perform probabilistic safety analysis in maintenance environment. Both models assume system operating in safe and unsafe environments. Model I represents a repairable system with three states: working normally, working in unsafe mode, and failed safely. Model II represents a repairable system with four states: working normally, working in unsafe mode due to human error, working in unsafe mode due to hardware failure, and failed safely. Equations for state probabilities, system availabilities, reliability, and mean time to failure are developed.

Notes: This paper presents reliability and availability analyses of a robot-safety system having one robot and n-redundant safety units with common-cause failures. The system failure rates and the partially failed system repair rates are assumed constant, and the failed system repair time is assumed arbitrarily distributed. Markov and the supplementary variable methods were used to perform mathematical analysis of this model. Generalized expressions for state probabilities, system availabilities, reliability, mean time to failure, and variance of time to failure are developed. Some plots of these expressions are shown.

Notes: Points out the steadily growing role of computers in the industrial sector. Highlights some accidents involving robots and touches on safeguards which have been introduced. Presents reliability and availability analysis of a robot machine/system having a redundant safety system. Formulas for system reliability, steady state availability, and mean time to failure are developed. Selective plots of the resulting formulas are shown.

Notes: This paper presents the results of a survey of Canadian robot users concerning robot reliability and safety. Data on 26 questions were analyzed and the resulting findings are presented in the form of tables, histograms, pie charts, etc. Provides conclusions including the fact that approximately 75 per cent of companies are using robots for commercial purposes; most general types of robots used in industry are intelligent robots; frequently ineffective maintenance manuals are provided by the robot manufacturers; and robot-related problems are generally less than 50 per year.
