    Notizen: During the years 1980–1989, 450 patients with an acute anterior cruciate ligament (ACL) rupture were treated at the University Hospital of Tampere, Finland. ACL ruptures were diagnosed by arthroscopy or open surgery. The data were analyzed in terms of patient age, sex, injury etiology, injury type and sports activity to determine the trends in the 1980s. The number of ACL tears seen in the Hospital increased by 247% over this period. Sixty-three percent of the patients were male (mean age 33 years) and 37% female (mean age 39 years), and there were no changes in the sex and age distribution in 1980s. The most common injury type was the isolated ACL rupture (51%), followed by a combination injury of the ACL and medial collateral ligament (38%). Isolated ACL ruptures in the 1980s increased 6.5 times. Sports accounted for 54% of all injuries with out any sign of change in their relative proportion in 1980s. The most common sports causing the rupture were soccer (29%), downhill skiing (20%), cross-country skiing (12%) and volleyball (12%). From 1980 to 1989, there was a decrease (2 times) in ACL injuries sustained in crosscountry skiing and a substantial (30 times) increase in ACL injuries sustained in downhill skiing.

    Notizen: Summary Fifty adult female rats were used to study the effect of 1-alpha-hydroxyvitamin D3 (1α-OH-D3)on bone after oophorectomy. The experimental period was 6 months. At the end of the experiment the femurs and the tibias were investigated for bone mass and composition. Significant signs of osteopenia occurred as a result of oophorectomy. The treatment with 1α-OH-D3 induced only minor changes in blood chemistry but increased bone mass significantly. The findings support the view that 1α-OH-D3 may be a valuable tool in the treatment of osteoporosis resulting from ovarian insufficiency.

    Notizen: Abstract The effects of two kinds of bioactive glass and two kinds of phosphate-free glass on new bone development induced by dernineralized bone matrix (DBM) were studied in the rat abdominal muscle pouch model. After 8 weeks' implantation histomorphometric analysis revealed that the amount of new bone in DBM combined with bioactive glass was comparable to DBM without bioactive glass. DBM grafts combined with phosphate-free glass showed significantly less new bone formation. Scanning electron microscopic examination confirmed that new bone bonded to the surface of bioactive glass. The release of ions from the glass seemed to slow down after new bone had bonded to it. Exclusion of phosphate from a bioactive glass resulted in loss of ability to develop the Ca,P-rich surface layer needed for bone bonding. contains BMP and other growth factors capable of inducing bone formation when implanted in various sites in laboratory animals [17–19]. Bioactive glasses have several beneficial properties as a bone substitute. The crystal chemistry of the surface formed in in vivo apatite contributes to a high bone bonding rate [6, 8], and the rate of reactivity can be controlled by choice of glass composition [2]. In addition, bonding of glass to soft tissues has been reported [7, 20]. We have previously reported formation of new bone directly on bioactive glass, induced by DBM in rat muscle tissue [13]. In the present study, the effects of four different glasses on new bone formation in DBM were studied in an extraskeletal site.

Clinical evaluation revealed no significant differences between patients with a straight stem and those with a curved anatomic stem. Loosening was statistically associated with young age, male sex, heavy weight, diagnosis other than osteoarthritis and year of operation. Survivorship analysis of the Lubinus IP implant at 8 years revealed a 95% chance of survival. The survival of the SP implant was 96%. After this time point the curve for SP implants began to fall at a faster rate than that for IP implants.

    Notizen: Abstract The effect of inserting a tricalcium phosphate (TCP) spacer stabilized by a rigid or non-rigid fixation technique on the healing of segmental tibial defects of critical size was established. The osteotomized tibiae, 11 with and 8 without TCP spacers, were fixed by an external circular device in 11 mature sheep and by plates in 8 mature sheep, respectively. Healing was evaluated roentgenographically 16 weeks after the operation. Compared with the defects without TCP spacers, enhanced stability and healing were observed in the defects with TCP spacers under an identical external fixation. Furthermore, a significantly higher incidence of healing was obtained with plate fixation than with external device fixation in the TCP-implanted defects (P 〈 0.04). An abundant bridging callus was roentgenograpically demonstrated in most of the healed defects, but none in the unhealed defects. The TCP spacer with its mechanical integrity enhances the stability of external fixation, and the stable immobilization provided by rigid fixation is essential for osteoconduction of an inserted TCP spacer in the healing of segmental diaphyseal defects in sheep.

    Notizen: Abstract We studied the healing of tibial double osteotomies in rats during clodronate administration 10 mg/kg daily subcutaneously for 6 weeks. Clodronate treatment did not inhibit the endochondral bone formation in callus or in the epiphyseal plate. The differentiation of osteoblasts into mature osteocytes was disturbed, and the bone structure around the osteocytes was not normal. In the clodronatetreated group, the amount of newly formed bone matrix seemed increased, but mineralization of the bone matrix was delayed. The bundle bone trabeculae in callus were not so spatially well organized as in the controls. No delay in the union of the bone segment by new bone was seen during clodronate administration.

    Notizen: Summary Of 155 institutionalized epileptics, 16 (10%) were noted to have suffered from different kinds of fractures when observed over a period of 1 year. The patients with fractures were further investigated with respect to blood and urine chemistry and bone morphology. The serum and urinary calcium values were below the average values for the population. Elevated values for serum alkaline phosphatase and for immunoreactive parathyroid hormone were found in most patients. The values of serum phosphate were distributed close to those of the normal population, while the urinary phosphate values were lower. Examination of bone biopsies revealed a significantly increased amount of osteoid and a significantly increased osteoclastic resorptive activity in epileptics, compared with age-matched controls. Furthermore, a slightly diminished volume of trabecular bone was noted. The high incidence of fractures may be explained by bone disease — a combination of osteoporosis, osteomalacia, and hyperparathyroidism due to chronic treatment with anti-convulsant drugs.
