    Notes: Purpose - The sensitivity of orifice plate metering to poorly conditioned and swirling flows are subjects of concerns to flow meter users and manufacturers. The distortions caused by pipe fittings and pipe installations upstream of the orifice plate are major sources of this type of non-standard flows. These distortions will alter the accuracy of metering up to an unacceptable degree.Design/methodology/approach - The design of orifice plate meters that are independent of the initial flow conditions of the upstream is a major object of flow metering. Either using a long straight pipe or a flow conditioner upstream of an orifice plate usually achieves this goal. The effect of cone swirler flow conditioner for both standard and non-standard flow conditions has been carried out in the experimental rig. The measuring of mass flow rate under different conditions and different Reynolds numbers were used to establish a change in discharge coefficient relative to a standard one.Findings - The experimental results using the cone swirler flow conditioner showed that the combination of an orifice plate and cone swirler flow conditioner is broadly insensitive to upstream disturbances. The results clearly show that this flow conditioner can attenuate the effect of both swirling and asymmetric flows on metering to an acceptable level.Originality/value - Previous work on the orifice plate has shown that the concept has promise. The results of using a combination of a cone swirler and orifice plate for non-standard flow conditions including swirling flow and asymmetric flow show this package can preserve the accuracy of metering up to the level required in the standards, providing that a new discharge coefficient is used for the combined swirler and orifice plate.

    Notes: Abstract The central problem in microsurgery is the reconstruction of small vessels. The long operating time, foreign body granuloma formation around the suture material as well as aneurysmal alterations of the vessel wall after conventional suture technique make the search for alternatives indispensable. Some of these disadvantages can be avoided as demonstrated by our animal experiments and histological examinations in laser-assisted anastomosing. The aim of this study is to show these aspects in connection with laser application and compare them with conventional suture techniques.

    Description / Table of Contents: Zusammenfassung Traumatisch bedingte Ventrikelseptumrupturen nach stumpfen Thoraxtraumen sind als Seltenheit anzusehen. Einem 19jährigen Studenten war während der Arbeit beim Entladen eines LKW eine 30 kg schwere Kiste auf den Brustkorb gefallen. Bei der Aufnahme bestand ein kardiogener Schock and über dem Herzen war ein lautes systolisches Geräusch zu hören. 6 Std nach dem Unfall konnte der Septumdefekt durch einen Teflonpatch verschlossen werden. Postoperativ kam es zu einer Anurie, bedingt durch den schweren prä-operativen Schock. Der Patient verstarb am 11. postoperativen Tag am akuten Nierenversagen und einer Bronchopneumonie.
    Notes: Summary This report concerns the case of a 19-year-old student who developed a loud systolic murmur over the precordium and signs of venous inflow obstruction following a bland chest trauma. The diagnosis of a traumatic VSD was confirmed by right- and transseptal left heart catheterization. The patient was operated upon 6 h after the accident with the aid of a cardiopulmonary By-pass. The defect was 1.5 cm in diameter and was located within the membranous part of the septum. Postoperatively the patient developed acute renal failure, which was most probably related to the state of shock which had been present before the operation. He died 11 days after surgery. The terminal event was bilateral pneumonia.

    Description / Table of Contents: Zusammenfassung Von 1962–1975 wurden in der Chirurgischen Universitätsklinik Heidelberg 815 Herzklappenprothesen implantiert. 474 Aortenklappen und 341 Mitralklappen, einschließlich 70 Doppelklappen wurden ersetzt. 22 Patienten (3%) mußten wegen einer Funktionsstörung der Prothese reoperiert werden. 19 Aortenklappen (3 Cross-Jones-, 16 Starr-Edwards-Prothesen) und 3 Mitralklappen (Cross-Jones) mußten wegen ausgerissener Fäden refixiert, wegen Thrombosierung oder defekter Scheiben ausgewechselt werden. Die häufigsten Komplikationen der gestörten Klappenfunktion waren: Herzinsuffizienz, Hämolyse und periphere Embolien. 7 Patienten sind verstorben. 4mal an Herzinsuffizienz, 2mal am Nierenversagen und 1mal an einer Hirnembolie.
    Notes: Summary In the Department of Surgery at the University of Heidelberg, 815 prosthetic heart-valve replacements were carried out from 1962 to 1975, 474 of aortic valves and 341 of mitral valves, including 70 double valve replacements. Twenty-two patients (3%) needed second operations due to insufficiency of the prothetic valve. In these 22 patients, 19 aortic valves (3 Cross-Jones and 16 Starr-Edwards prostheses) and 3 mitral valves (Cross-Jones) had to be refixed or changed due to thrombosis or a defect of the valve. The most common clinical complications of these valve malfunctions were: Cardiac insufficiency, hemolysis, and peripheral embolism. There were 7 deaths: 4 due to cardiac insufficiency, 2 due to renal failure and 1 due to brain embolism.

    Description / Table of Contents: Zusammenfassung In Form von 4 Tierversuchen (Kälber) wird eine in vitro an Rinder-, Kälber- und Schafsherzen erarbeitete Methodik zur röntgenologischen Darstellung des RSL des Herzens bezüglich ihrer in-vivo-Tolerabilität erprobt. Klinisch kann die in-vivo-Tolerabilität grundsätzlich nachgewiesen werden; wir müssen jedoch betonen, daß durch die Verwendung hypertonischer Röntgenkontrastmittel Veränderungen im Bereich der Mikro- und Ultrastrukturen der RLS-Zellen induziert werden können, deren Dignität momentan nicht sicher eingestuft werden kann.
    Notes: Summary A method for radiological presentation of the left ventricular conduction system was tested for in-vivo tolerability. In-vivo tolerability could be demonstrated clinically; however, we should emphasize that hypertonic X-ray contrast dyes may cause morphological changes of unknown significance in the micro- and ultrastructures of the specialized musculature.

    Description / Table of Contents: Zusammenfassung Diskutiert wird der Fall einer 28jährigen Motorradfahrerin, die bei einem Unfall in das Speichenrad ihres Fahrzeuges geraten war. Es resultierte eine ausgedehnte Weichteilverletzung des Mittel- und Rückfußes, eine Defektfraktur des Calcaneus, ein knöcherner Abriß der Achillessehne, eine Außenknöchelfraktur Weber A und eine Metatarsale II-Fraktur. Die Primärversorgung erfolgte in folgender Weise: Ausgedehntes Debridement der stark verschmutzten Weichteile, Refixation der Achillessehne am Calcaneus mit zwei Osteosyntheseschrauben. Die noch erhaltenen, jedoch kontusionierten Hautlappen wurden spannungsfrei adaptiert. Dies war allerdings nur in einer erheblichen Spitzfußstellung möglich. Im weiteren Verlauf kam es zu einer ausgedehnten Haut- und Weichteilnekrose über dem Rückfuß. Um einen belastungsfähigen Rückfuß zu erhalten, eine Calcaneusosteitis abzuwenden, die Spitzfußstellung auszugleichen Bowie eine Resensibilisierung in der Belastungszone der Fußsohle zu erzielen, wurde ein neurovaskulär gestielter Unterarmlappen frei übertragen. Es kam zu einem guten Einheilungsergebnis, die Spitzfußstellung wurde weitgehend ausgeglichen, die Calcaneusosteitis abgewendet, und die Funktion der Achillessehne blieb erhalten. Darüber hinaus kam es zu einer befriedigenden Resensibilisierung des Rückfußes, so daß die Patientin heute in der Lage ist, in Konfektionsschuhen mit einer umfassend gewalkten Einlage zu laufen.
    Notes: Summary A discussion is presented on the case of a 28-year-old motorcyclist who had an accident and got caught in the spoked wheel of her vehicle. The result was an extensive soft-tissue injury of the metatarsus and calcaneal part of the foot, a defect fracture of the calcaneus, an osseous disinsertion of the Achilles tendon, a lateral malleolus Weber type-A fracture, and a fracture of the second metatarsal bone. The primary treatment consisted in extensive débridement of the strongly contaminated soft parts and refixation of the Achilles tendon on the calcaneus with two osteosynthesis screws. The bruised skin flaps remaining were adapted in a tension-free manner. However, this was possible only in a considerable talipes equinus position. In the further course, the patient developed an extensive skin and soft-tissue necrosis over the calcaneal part of the foot. A neurovascularly pedicled forearm flap was freely grafted in order to maintain the load capacity of the calcaneal part of the foot, to prevent osteitis of the calcaneus, to correct the talipes eqinus position, and to achieve a resensibilization in the load zone of the sole. The healing result was good, and it was possible to achieve extensive correction of the talipes equinus position as well as prevention of calcaneus osteitis; the function of the Achilles tendon was also maintained. Moreover, there was satisfactory resensibilization of the calcaneal part of the foot, so that the patient can now walk in ready-made shoes with a fully molded arch support.

    Notes: Abstract Objective: Our objective was to investigate the fertilizing ability of human sperm from severe male-factor patients, by microinjection of single sperm into the hamster oocyte. Design: Semen samples of severe male factor either with a 0% penetration rate in the zona-free hamster test or with a very low number of motile sperm for which performing the standard penetration test was impossible were used. For the control study, oligozoospermic semen samples with at least 10% penetration rate in zona free hamster test were used. Setting: All materials were collected from the National University Hospital, Singapore. Methods: There were 10 patients in both the experimental and the control groups. Intracytoplasmic sperm injection (ICSI) was carried out. The main outcome measures were sperm head decondensation and pronuclear formation. Results: Twenty-one percent of the injected sperm could decondense and undergo male pronuclear formation. This rate was not significantly different from that in the control study group (28%; P=0.13). A small proportion of the oocytes was damaged during the procedure (9.2 and 8.75% in experimental and control groups, respectively). Conclusions: Hamster-ICSI assay may be of benefit in predicting the sperm's ability for further development before allowing the patient to undergo the clinical program.
