    Notes: In 70 children with defined chronic chest disease, immunoglobulins, IgG subclass levels and antibody concentrations specific for Haemophilus influenzae b (Hib), and pneumococcal antigen, were related to disease severity. Bronchological examinations revealed 30 children with chronic bronchitis, 21 with bronchial deformations and 19 with bronchiectasis. Of the 70 children 12 (17.1%) showed an underlying immunodeficiency. The commonest finding was an IgG subclass deficiency, 7 were IgG2 and 1 IgG3 deficient, followed by IgA deficiency in 3 patients. All patients had normal IgG and IgM levels except one who had immunodeficiency with elevated IgM. Pneumococcal antibody levels were found to increase between patient groups in the order healthy children 〈 chronic bronchitis 〈 bronchiectasis 〈 bronchial deformations (p 〈 0.01), but this was not the case for Hib antibodies. We found no selective deficiency of pneumococcal and Hib antibodies in our patients. Pathogens were detected in bronchial cultures from 10% of patients with chronic bronchitis, 33% of those with bronchial deformations and in 63% with bronchiectasis. This increase (p 〈 0.01) reflects a more severe inflammation of the respiratory tract in such patients. However, immunodeficiencies were equally distributed between patient groups. We conclude that only a subgroup of children with chronic chest disease have an underlying immunodefiency, but most patients (83%) do synthesize normal or even high antibodies in the presence of a bacterial load.

    Notes: Abstract Two asymptomatic Turkish sibs are presented, a 4-year-old boy and his 7-year-old sister, with pulmonary alveolar microlithiasis (PAM) confirmed by transbronchial lung biopsy and bronchoalveolar lavage. Chest radiographs and high resolution CT demonstrated widespread intra-alveolar calcifications in both lungs. The lesions were sharply defined and less than 1 mm in diameter. CT documented a high concentration of microliths along the bronchovascular bundles, the intralobular fissure and the (sub)pleural lung parenchyma. The combination of bronchoalveolar lavage and roentgenographic appearance in high resolution CT are characteristic and pathognomonic, and can confirm the diagnosis. The more severe changes in the elder sib and the radiographic controls suggest that the pulmonary disease may be progressive in our patients. The described family of consanguineous, unaffected parents with two affected and one healthy child confirmed the autosomal recessive inheritance of PAM (McKusick 265100). In addition, the affected girl had autosomal recessive Waardenburg-anophthalmia syndrome (McKusick 206920), raising the question of whether this is a chance occurrence or possibly a contiguous gene syndrome.

To prevent lasting damage a causal therapy is strongly advisable. Thorough diagnosis and the exclusion of associated malformations will help to prevent a protracted course of disease and complications. Regular controll visits will ensure a long term sucess of therapy and where necessary on appropriate adjustment of therapy.

Durch sorgfältige Diagnostik und Ausschluß assoziierter Fehlbildungen werden langwierige Therapieverläufe und Komplikationen vermieden. Regelmäßige klinische und apparative Kontrollen sichern langfristig den Therapieerfolg und ermöglichen eine rechtzeitige Anpassung des therapeutischen Prozederes.

    Description / Table of Contents: Zusammenfassung Inaktive Riesenchromosomen der Speicheldrüsen ausgewachsener Chironomus thummi-Larven zeigten nach 60 min Inkubation mit 0,05 N Putrescin-, Spermidin-oder Spermin-Lösung erneute Syntheseaktivitäten, die sich im Einbau von radioaktiv markiertem Uridin autoradiographisch zu erkennen geben. Diese Beobachtung wird im Zusammenhang mit der bekannten genaktivierenden Wirkung von Mg++ diskutiert. Auf der Basis zahlreicher biochemisch analoger Wirkungen von Polyaminen und Mg++ wird ein ursprünglich für die genaktivierende Wirkung von Mg++ postuliertes Modell auf Polyamine übertragen.
    Notes: Summary Inactive salivary gland giant chromosomes of full grown Chironomus thummi larvae showed after 60 min of incubation in 0.05 N putrescine, spermidine or spermine solutions autoradiographically demonstrable incorporation of radioactiv labelled uridine caused by a reactivation of RNA synthesis. This observations is discussed in relation to the known gene activating effect of Mg++, and, since polyamines and Mg++ have numerous effects in common, it is suggested that polyamines affect gene activity in a way similar to Mg++.

    Notes: Summary The stimulation of glucose uptake and RNA synthesis by insulin was studied in cultured fibroblasts from patients with an inherited affinity defect of the insulin receptor. Additional cell cultures were set up of two patients with Alstrøm syndrome, a genetic disease with insulin resistance but normal insulin receptor, and of eight healthy individuals. In receptor-defective patients we found a corresponding defect of the insulin-mediated stimulation of RNA synthesis but a normal stimulation of glucose uptake. We conclude that both postreceptor effects are controlled by different insulin-directed mechanisms.

    Notes: Summary Using a fragment of the cDNA for human coagulation factor VIII as a hybridization probe, we have detected a new polymorphic Hind III site in intron 19 of the factor VIII gene. The frequency of the minor allele is 0.30. This polymorphism shows strong linkage disequilibrium with a previously described BclI polymorphism in intron 18.

    Notes: Abstract Endogenous synthesis of nitric oxide (NO) and its presence in exhaled air was observed in various species including humans. Particularly high levels were found in adults with bronchial asthma, possibly because of the underlying pulmonary inflammatory activity. We studied oral and nasal exhaled NO by chemiluminescence in 47 children aged between 6 and 10 years. Thirty children had bronchial asthma, 17 were healthy controls. In asthmatic children oral exhaled NO was 13.4±1.4 parts per billion (ppb) (mean±SEM), nasal exhaled NO was 21.7±1.5 ppb. In healthy controls oral exhaled NO was 7.2±1.0 ppb, nasal exhaled NO was 18.2±2.2 ppb. Oral exhaled NO was significantly higher in asthmatic children compared to healthy controls (P=0.0017). Nasal exhaled NO did not differ significantly in the two groups. There was a significant negative correlation between oral exhaled NO and forced expiratory volume in 1 s (FeV1). No significant correlation between oral or nasal exhaled NO and other markers of obstructive lung function impairment, oral minute ventilation, the body mass index and the presence of upper respiratory tract infection could be found.
