    A further case of giant intrasellar carotid aneurysm mimicking a pituitary adenoma: The relevance of a multivariate approach in differential diagnosis (1994)

    Description / Table of Contents: Sommario Viene descritto il caso di una donna di 53 anni la quale, affetta da una oftalmoparesi estrinseca del III nervo cranico sinistro insorta lentamente fece sospettare in prima istanza una miastenia oculare. Esclusa tale affezione con il test del Tensilon, la TC cranica fece porre il quesito di adenoma ipofisario, che era confortato dalle indagini endocrinologiche e dalla risonanza magnetica. Solo un'angiografia carotidea eseguita il giorno precedente l'intervento programmato per via transfenoidale rivelò un aneurisma intrasellare gigante del sifone carotideo sinistro, sul quale è stato effettuato un by-pass extra-intracranico con occlusione della carotide interna.
    Notes: Abstract We describe a 53 year old woman who progressively showed a left extrinsic oculomotor paresis, at first suggesting ocular myasthenia, which was ruled out by a negative Tensilon test. A CT scan of the brain revealed a pituitary mass consistent with an adenoma which was confirmed by endocrinological investigation and standard MRI. Carotid angiography, performed the day before the scheduled trans-sphenoidal surgery, clearly showed a giant intrasellar aneurysm of the left carotid syphon. An extra-intracranial by-pass with occlusion of the internal carotid artery was successfully executed.

Come per le altre rare osservazioni presenti nella letteratura, alla rimozione dell'ematoma era seguita la graduale risoluzione dei sintomi extrapiramidali.
    Notes: Abstract A 48 year-old man with chronic subdural hematoma presented with a parkinsonian syndrome. Removal of the hematoma was followed by gradual disappearance of the extrapyramidal symptoms, as in other, rare, published cases.

    Description / Table of Contents: Sommario Viene riportato il caso di uno shock emorragico con un quadro EEG caratterizzato da scariche pseudoperiodiche parossistiche lateralizzate, ripetute crisi motorie focali sul lato opposto delle prevalenti anormalità EEG e turbe della coscienza in assenza di una lesione cerebrale documentata. Il caso era quello di una donna di 45 anni senza precedenti personali o familiari di epilessia. Questo aspetto unusuale del quadro elettroclinico appare poco spiegabile in quanto intervenuto parecchi giorni dopo lo shock emorragico quando tutti i dati di laboratorio erano ritornati nella norma. L'unica spiegazione di questo quadro sembra essere l'ipossia cerebrale e il disordine metabolico conseguente allo shock emorragico.
    Notes: Abstract We report a case of hemorrhagic shock associated with an EEG pattern resembling pseudoperiodic lateralized paroxysmal discharges, repeated focal motor seizures on the opposite side of the prevalent EEG abnormalities and consciousness disturbances in the absence of a demonstrable cerebral lesion occurring in a 45 year-old woman without a personal or familial history of epilepsy. This unusual electroclinical picture appeared inexplicably several days after the hemorrhagic shock when the laboratory data had already normalized. The only cause of this picture seemed to be brain hypoxia and the metabolic disorders consequent upon hemorrhagic shock.

    Description / Table of Contents: Sommario Pazienti affetti da tumori dell'angolo ponto-cerebellare sono stati sottoposti ad indagine vestibolare, dell'oculomotricità lenta e rapida e al test dell'interazione visuovestibolare (Visual Suppression Test-VST). In tutti i casi è stata utilizzata la registrazione elettronistagmografica (ENG). Nella patologia dell'angolo ponto-cerebellare l'indagine ENG deve tener conto dei seguenti rilievi: ny spontaneo ed evocato, alterazioni dismetriche dell'oculomotricità coniugata volontaria. Questo studio dimostra che tale indagine pud rivelare il coinvolgimento delle strutture vestibolari tronco-encefaliche e vestibolari e assai spesso i segni ENG precedono la comparsa dei segni clinici di compressione. Inpresenza di neurinomi dell'ottavo nervo cranico l'indagine Strumentale vestibolare ed oculomotoria fornisce significative informazioni per una diagnosi precoce del passaggio del tumore dalla fase otoiatrica alla fase neuro chirurgica.
    Notes: Abstract Patients with cerebellopontine angle tumors underwent vestibular examination, testing of voluntary fast and slow conjugate eye movements and of visual-vestibular interaction (visual suppression test — VST) by electronystagmography (ENG). Both spontaneous and evoked nystagmus (Ny) and dysmetric alterations of the voluntary conjugate eye movements are relevant to cerebellopontine pathology. This study shows that ENG may reveal damage to the vestibular brain-stem and archicerebellar structures, the ENG signs of which very often precede the clinical signs of compression. In cranial nerve VIII neuromas vestibular and oculomotor instrumental investigations yield valuable clues for early diagnosis of tumor transition from the otological to the neurosurgical stage.
