Zusammenfassung Von 1969 bis 31. 12.1981 wurden 216 Cardiacarcinome durch Gastrektomie (n = 87) oder proximale Resektion (n = 129) reseziert. Die pathologische Klassifikation ergab 2 % pT1, 46 % pT2, 35 % pT3 und 17 % pT4-Stadien. Nach Lauren lag in 72 % ein Intestinalzelltyp, in 26 % ein diffuser Typ vor, nach Ming in 51 % ein infiltrativer, in 47 % ein expansiver Typ. Bei im Mittel 31 untersuchten Lymphknoten/Präparat waren die Lymphknoten an der Art. gastrica sin. in 77 %, im Hiatus Oesophagei in 40 % und paraoesophageal in 24 aller Fälle befallen. Die Ergebnisse sprechen für Gastrektomie und distale Oesophagusresektion mit Dissektion aller Lymphabflußgebiete, auch paraoesophageal.

Summary From 1969 to 1981 in 216 patients with adenocarcinoma of the cardia a total gastrectomy (n = 87) or proximal resection (n = 129) was performed. The pathological classification showed 2 % pT1, 46 % pT2, 35 % pT3, and 17 % pT4. According to Lauren's classification, there were 72 % intestinal-cell and 26 % diffuse type carcinomas; according to Ming 51 % infiltrative and 47 % expansive type carcinomas. Of an average of 31 examined lymph nodes per specimen, 77 % of all tumors showed positive lymph nodes at the left gastric artery, 46 % at the esophageal hiatus, and 24 % of lymph nodes were paraesophageal. We suggest total gastrectomy with resection of the lower third of the esophagus and dissection of lymphatic drainage areas for the treatment of cardia cancer.
