    Description / Table of Contents: Résumé La chirurgie laparoscopique du pelvis masculin reste une chirurgie à risque, environ 15 % de complications péri-opératoires, si on la compare aux 0,03 % de la chirurgie laparoscopique en milieu gyné-co-obstétrical. Bon nombre de ces complications sont liées à la confrontation de l'opérateur à une situation anatomique nouvelle par son approche à travers un champ opératoire laparoscopique. Nous avons confronté aux données de la littérature notre expérience anatomique et clinique afin de préciser les bases anatomiques du risque chirurigical pour ce type d'interventions. Si la dissection en dehors des vaisseaux iliaques (chirurgie du cordon spermatique) est sûre, par contre les rapports anatomiques étroits et leurs variations rendent la dissection sur le bord médial des vaisseaux iliaques plus risquée (anomalies vasculaires et rapports variables de l'uretère en particulier).
    Notes: Summary Laparoscopic surgery of the male pelvis remains a risky procedure, with around 15% of perioperative complications, as compared to 0.03% in laparoscopic surgery for gynecologic and obstetric conditions. Many of these complications are due to the surgeon's being faced with an unfamiliar anatomic situation in his approach through the laparoscopic operative field. We have added to the data of the literature our anatomic and clinical experience in order to define the anatomic basis of surgical risk in this type of procedure. While dissection lateral to the iliac vessels (surgery of the spermatic cord) is safe, dissection medial to these vessels is more risky because of the confined anatomic relationships and their variations (particularly vascular anomalies and variable relations of the ureter).

    Description / Table of Contents: Résumé Cette étude anatomique de la région lombaire (selon la définition de H Rouvière) appliquée aux techniques d'imagerie multiplanaires a été menée conjointement post-mortem etin vivo. L'étude sur le cadavre (5 cas) repose sur des coupes anatomiques (transversales, sagittales, frontales, obliques) et des reconstructions tridimensionnelles informatisées après acquisition en tomodensitométrie réalisées sur des sujets injectés avec du latex coloré et radio-opaque. L'étude sur le vivant (4 cas) utilise des coupes en résonnance magnétique, des coupes en échographie tri-dimensionnelle et couplée au doppler pulsé. Les coupes obliques verticales référencées dans l'espace font découvrir sous des aspects inhabituels ces structures tout en les situant au sein du rétropéritoine avec un maximum de repères topographiques. La transposition de ces résultats (issus de l'anatomie sectionnelle du rétropéritoine) aux nouvelles techniques de reformatage multiplanaire après acquisition en IRM. échotomographie ou tomodensitométrie devrait permettre d'améliorer l'exploration de certaines structures rétropéritonéales en spécifiant pour chacune d'elle les plans de coupes idéaux tout en diminuant certains artéfacts propres aux acquisitions dans les plans de coupe traditionnels. Les coupes obliques verticales nous apparaissent parfaitement adaptées pour le repérage des loges surrénaliennes en échotomographie, l'étude des pédicules rénaux et le repérage topographique rétropéritonéal. Cette visualisation oblique verticale constitue un appoint fondamental pour le développement des interventions menées en vidéochirurgie car elle correspond exactement aux axes des voies d'abord en chirurgie percutanée du rein et des structures anatomiques adjacentes.
    Notes: Summary This anatomic study of the lumbar region (as defined by Rouvière [11]) applied to multiplanar imaging techniques was carried out both in the cadaver andin vivo. The cadaver study (5 cases) consisted of anatomic sections (transverse, sagittal, frontal and oblique) and computerised three-dimensional reconstructions after CT studies on subjects injected with colored and radio-opaque latex. Thein vivo study (4 cases) used MRI sections and three-dimensional ultrasound sections coupled with the pulsed doppler. The spatially referenced oblique vertical sections revealed the structures from unusual aspects, situating them amidst the retroperitoneal area with the maximum of topographic landmarks. The transposition of these results (obtained by sectional anatomy of the retroperitoneal region) to the new techniques of multiplanar formatting after MRI, ultrasound or CT data acquisition should optimise the investigation of certain retroperitoneal structures by specifying the ideal planes of section for each organ, while diminishing certain artefacts specific to acquisitions in the traditional planes of section. Oblique vertical sections seem eminently suitable for ultrasound location of the suprarenal compartments, study of the renal pedicles and topogaphic retroperitoneal location. This oblique vertical visualisation constitutes a fundamental resource for the development of video-monitored surgical procedures as it corresponds exactly to the axes of the access routes in percutaneous surgery of the kidney and the adjacent anatomic structures.

    Notes: Abstract Focal spinal cord lesions have been present in all previously reported cases of MRI appearances in myelopathy complicating vitamin B12 deficiency. We describe two further cases showing mild atrophy only and review the salient features of the previous 11 publications. MRI findings reflect quite closely the known pathological changes in this condition.

    Notes: Abstract Serum pancreatic enzyme behavior, exocrine function, and morphology of the pancreas were studied in 28 patients with end-stage renal disease undergoing regular hemodialysis, in order to better delineate and assess the clinical relevance of the pancreatic alterations that occur in these patients. Twenty-eight healthy subjects served as controls. Initial studies included serum amylase, isoamylase, and lipase assays; fecal chymotrypsin measurement; and abdominal ultrasonography. The amylase, lipase, and chymotrypsin determinations, as well as ultrasound examination, were repeated four years later. None of the patients had clinical evidence of pancreatic disease at entry into the study, but one had had previous attacks of pancreatitis and another developed mild acute pancreatitis one month after entry. Initial mean serum enzyme levels were significantly higher in patients than in controls (amylase, pancreatic isoamylase, and lipase,P〈0.001; salivary isoamylaseP〈0.05). Serum amylase was raised in 16/28 patients; pancreatic isoamylase in 15/28, and lipase in 7/28; these elevations were generally mild. Mean fecal chymotrypsin was significantly lower (P〈0.001) in patients than in controls: abnormally low values were found in 9/28 patients. Amylase, lipase and chymotrypsin measurements repeated after four years showed no significant difference with respect to the first study. Ultrasonographic changes were rare and mild: one patient had a small cyst in the pancreas head, another, an increase in echogenicity of the gland not related to age; these findings were unchanged at repeat examination. The results demonstrate that the frequent elevations of serum pancreatic enzymes and the rare sonographic changes found in patients undergoing hemodialysis do not generally reflect a relevant pancreopathy. However, the finding of significantly decreased fecal chymotrypsin may indicate the presence of pancreatic dysfunction in end-stage renal disease.

    Notes: Abstract A case of a 33-year-old pentazocine-addicted man (450 mg daily) is described. Histochemical and quantitative histographic analyses of the muscle biopsy disclosed type 2 atrophy. We consider that this indicates that there is a generalized toxic effect of pentazocine.
