Summary The partial and generalized ischemic crises may stronglys resemble epileptic crises. They are accompanied by phenomena of neuronal irritation corresponding to a liberation of subcortical structures not attained by ischemic anoxy. From a clinical point of view, ischemic and epileptic crises are often confounded. This is all the more important as both kinds of crises do not only differ by their pathophysiology but also as to their prognostics and therapeutics. A differential diagnosis comprising at the same time the registration of the cerebral and cardiac electric activities as well as blood-pressure and respiration proves to be of great practical importance. Description of the electroencephalographic and clinic semiology of the syncopal crisis with and without convulsions. Comparison with absence, petit and grand mal; exposition of characteristic EEG traces and of the chronology of the events. Ocular and sino-carotid compression as well as Valsalva proof are the methods of activation preferred for the ischemic crises. Hyperpnea, inspiration of azote, and photostimulation, however, activate the epileptic crises. In the second part of the report, the clinical and electroencephalographic diagnostics of the so-called media and basilaria crises and of the posterior cerebral crises, respectively, are compared with those of partial epileptic expressions. Résumé Les crises ischémiques partielles et généralisées peuvent fort ressembler à des crises épileptiques; elles s'accompagnent de phénomènes d'irritation neuronale correspondant à une libération de structures souscorticales non atteintes par l'anoxie ischémique. Au point de vue clinique on confond souvent les crises ischémiques et épileptiques. Ceci est d'autant plus lourd de conséquences, que les deux genres de crises ne diffèrent non seulement par leur pathophysiologie mais également en ce qui concerne leur pronostic et leur thérapeutique. Un diagnostic différentiel comprenant à la fois l'enregistrement de l'activité électrique cérébrale et cardiaque de même que de la tension artérielle et de la respiration s'avère donc de toute importance. Description de la sémiologie électroencéphalographique et clinique de la crise syncopale avec et sans convulsions. Comparaison avec les absences, le petit et le grand mal; exposé des tracés EEG caractéristiques et de la chronologie des événements. La compression oculaire et sinu-carotidienne ainsi que l'épreuve de Valsalva sont les méthodes d'activation par excellence pour les crises ischémiques; l'hyperpnée, l'inspiration d'azote et la photostimulation activent par contre les crises épileptiques. Dans la deuxième partie du rapport le diagnostic différentiel clinique et électroencéphalographique des crises dites de la Sylvienne et de la basilaire respectivement de la cérébrale postérieure est comparé avec celui des expressions épileptiques partielles. Zusammenfassung Fokale und generalisierte ischämische Anfälle können epileptischen Entäußerungen sehr ähneln; sie gehen mit neuronalen Reizphänomenen einher, die einer Enthemmung (im SinneJacksons) der subcorticalen, nicht von der ischämischen Anoxie betroffenen Strukturen entsprechen. Ischämische und epileptische Anfälle werden häufig klinisch verwechselt. Dies ist um so schwerwiegender, als die beiden Anfallsarten sich nicht nur in pathophysiologischer, sondern auch in prognostischer und therapeutischer Hinsicht unterscheiden, was ihrer Differentialdiagnose speziell unter Berücksichtigung gleichzeitiger Registrierung der cerebralen und cardialen elektrischen Aktivität sowie des Blutdruckes und der Atmung große praktische Bedeutung verleiht. Beschreibung der elektroencephalographischen und klinischen Semiologie des synkopalen Anfalles mit und ohne Konvulsionen. Die Unterschiede gegenüber Absencen, Petit- und Grand-Mal werden hervorgehoben; hierbei spielt neben den charakteristischen Hirnwellenbildern insbesondere der zeitliche Ablauf der Erscheinungen eine große Rolle. Okuläre-, Carotissinus-Kompression And Valsalva-Versuch werden als Provokationsmethoden der Wahl für ischämische Anfälle der Hyperventilation, Stickstoffatmung und Photostimulation als Maßnahmen zur Aktivierung epileptischer Entäußerungen gegenübergestellt. Im zweiten Teil der Ausführungen wird in ähnlicher Weise die klinische und elektroencephalographische Differentialdiagnostiks der sogenannten “Media- und Basilaris/Posterior-Anfälle” zu fokalen epileptischen Reaktionen in Parallele gesetzt. Pathogenetische Erörterungen schließen das Referat ab.
