    Notes: Summary A young woman with long-standing systemic lupus erythematosus and rheumatic fever presented with deteriorating renal function. She had severe gastritis treated with cimetidine and received methylprednisolone when her blood cultures were repeatedly negative. She developed spontaneous bruising and bleeding from venepuncture sites, leading to clotting studies and a diagnosis of thrombotic thrombocytopenic purpura. At postmortem, extensive evidence of disseminated zygomycosis was found. The likely portal of entry was the gastric route.

    Notes: Abstract The value of a change in summated electro-cardiographic voltage as a predictor of cardiac rejection is uncertain in patients treated with low-dose triple immunotherapy (cyclosporin, azathioprine, and corticosteroids) following orthotopic cardiac transplantation. Ten recipients were studied daily and the summated QRS voltages from 600 electro-cardiograms were calculated. A single observer graded 147 endomyocardial biopsies. During the study period there were 18 episodes of acute rejection with myocyte necrosis. Only three episodes of rejection were heralded by a significant change in QRS voltage, yielding a positive predictive value of 13% for the technique. Three episodes of severe bacterial infection were preceded by significant decreases in QRS voltage. In one patient with a global pericardial effusion, QRS voltage was related to the depth of effusion measured by echocardiography. These data show that QRS voltage is of extremely limited value in the prediction of cardiac rejection in patients treated with low-dose triple immunotherapy.

    Notes: Summary Two hundred and nine patients with moderate to severe chronic heart failure, all of whom remained symptomatic despite at least 80 mg of frusemide daily, were randomized to 12 months treatment with flosequinan or captopril. The patients were stratified into two groups, a treadmill group and a corridor walk test group, depending upon their exercise capability. Sixty-five out of 102 patients randomized to flosequinan and 43 out of 107 randomized to captopril (p〈0.001) did not complete the study. There was no difference between the groups in mortality: 19 patients died while taking flosequinan and 15 while taking captopril. Both drugs had similar effects on treadmill exercise tolerance; the mean increase at week 52 was 117 seconds in the flosequinan group and 156 seconds (p=0.57) for the captopril group. For those patients stratified to the corridor walk test only, there was also very little difference in the improvement at 52 weeks; the mean increase for patients randomized to flosequinan was 61 meters and captopril was 75 meters (p=0.65). However, when the walk tests from all patients are examined, captopril produced a significant improvement compared with flosequinan at week 52 (p=0.015). Flosequinan has similar long-term efficacy to captopril but is associated with a higher incidence of adverse events.

    Notes: Abstract A microcomputer-based pacemaker system for the evaluation of pacemaker treatment of tachycardia is described. The system may be used to study tachycardia initiation, tachycardia termination or a combination of the two. The software incorporates a visual display unit screen handling package which provides the user-system interface. System-patient interfacing is performed by a separate pacing and sensing unit which communicates with the computer via standard digital input/output lines. Several pacing options are available, selectable from a screen-displayed menu. Each selection also has an associated set of programmable parameters which may be adjusted, within allowed limits, to suit particular studies. Examples of the use of the system for tachycardia termination are given. The main programming language for the controlling software was Fortran IV. Some routines were necessarily written in assembly language. The system is useful for evaluation purposes and forms the basis of a cardiac pacemaker development tool.
