    Notes: Summary An intravenous infusion regimen for flecainide was studied in 12 patients. Therapeutic drug levels were rapidly established and maintained throughout the 48-h infusion period. Two patients were withdrawn because of possible drug related problems, but in general the regimen seemed both effective and well tolerated.

    Notes: Summary Concentrations of verapamil and norverapamil were measured in the plasma and breast milk of a woman receiving 120 mg verapamil three times daily. Whilst the concentrations of verapamil in breast milk averaged about 64% of those in maternal plasma no drug could be detected in the plasma of the infant. It is concluded that maternal therapy with verapamil does not pose a hazard to the suckling infant.

    Notes: A simple HPLC method for penbutolol and 4-hydroxypenbutolol assay has been developed. Plasma or serum (200 μl) is vortex-mixed (30 s) with Tris solution (2 M, pH 10.6) containing an internal standard (50 μl) and methyl t-butyl ether (200 μl). After centrifugation, the extract (100 μl) is analysed using an unmodified silica column (250 × 5 mm ID) and iso-octane-methanol-methyl t-butyl ether (55:25:20) containing ammonium perchlorate (10 mM, pH 5.7) as eluent and with fluorescence detection. No interference has been encountered and the limit of accurate measurement for both compounds is 5 μg/l.

    Notes: A simple HPLC method has been developed to measure the antiarrhythmic agent penticainide and its N-desalkyl metabolite in biological fluids. Solvent extraction of a small (200 μL) sample volume with direct analysis of the extract is used to measure the plasma and urinary concentrations of these compounds attained during chronic therapy, although a larger sample volume (1.0 mL) and prior concentration of the extract are required for single oral dose work. In each case chromatographic analysis is performed using a microparticulate (5 μm) silica column and methanolic ammonium perchlorate (10 mM, pH 6.7) as eluent with UV detection (260 nm). No endogenous sources of interference have been encountered and potential interference from other drugs is minimal.

    Notes: A method is described for the measurement of lignocaine in small samples of fetal and placental tissue. Tissue samples (ca 100 mg) are digested using a preteolytic enzyme. Lignocaine and an internal standard are extracted into methyl tert-butyl ether and analysed by high-performance liquid chromatography with electrochemical detection (+1.0 V vs Ag/AgCl). The limit of accurate measurement is better than 0.1 mg/kg wet weight for a 100 mg sample. This method has been used to assess fetal exposure to the drug when used as a local anaesthetic during transabdominal placental biopsy (chorionic villus sampling). The range of lignocaine concentrations found in the tissue samples was large (from 〈0.04 mg/kg wet weight to 15.4 mg/kg wet weight) although most samples contained less than 1.0 mg lignocaine/kg wet weight.

    Notes: A simple high performance liquid chromatographic method has been devised for the measurement of bisoprolol in plasma or serum. The sample (200 μL) is vortex mixed for 30 s with 2 M Tris solution (50 μL), aqueous internal standard (benzimidazole, 2.0 mg/L, 50 μL) and methyl t-butyl ether (200 μL). After centrifugation (9950 × g, 2 min), a portion of the resulting extract is analysed on a microparticulate (5 μm) silica column using 1 mM camphorsulphonic acid in methanol as the mobile phase. Detection is by fluorescence at an excitatioh wavelength of 215 nM. The lower limit of accurate measurement for the assay is 10 μ/L (CV% = 8.9, n = 9) with a lower limit of detection of 5 μg/L. There is minimal interference from either commonly prescribed drugs or endogenous compounds.
