    Notes: Basic design requirements are presented for pyroelectric detectors to be used effectively when the radiation source emits part or all of its power in the vacuum ultraviolet range, as is the case for a typical plasma in magnetic confinement fusion research. The difficulty associated with the VUV wavelength range lies in the high photoelectric yield of the associated energetic photons. A relatively inexpensive, commercially available detector was modified to eliminate the spurious currents resulting from photoelectrons. The modifications have little effect on the basic characteristics of the commercial detector (time response, sensitivity, etc). © 1996 American Institute of Physics.

    Notes: Fresh gas flow into a circle system can affect the delivered minute ventilation because fresh gas flow augments the flow delivered by the ventilator bellows during inspiration. After establishing a stable ventilatory pattern with 3.0 l·min-1 fresh gas flow into a circle system, changes in peak inflation pressure, minute ventilation and end-tidal carbon dioxide were measured at 1.5 l·min-1 and 6.0 l·min-1 in 10 toddlers (10–20 kg) and 10 children (30–60 kg). Changes in all variables were observed but these changes were greater in toddlers compared with children (P 〈 0.001). Some toddlers were noted to have as much as a 37% change in ventilatory parameters when fresh gas flow was altered between 1.5 and 6.0 l·min-1. Whenever changes are made in fresh gas flow, compensatory changes in minute ventilation should be considered to avoid unintended hyperventilation or hypoventilation. This is especially important during anaesthesia for toddlers.

    Notes: Time-resolved spectroscopy (with a 2 psec temporal resolution) of plasmas produced by the interaction between solid targets and a high contrast subpicosecond table top terawatt (T3) laser at 1016 W/cm2, is used to study the basic processes which control the x-ray pulse duration. Short x-ray pulses have been obtained by spectral selection or by plasma gradient scalelength control. Time-dependent calculations of the atomic physics [Phys. Fluids B 4, 2007, 1992] coupled to a Fokker–Planck code [Phys. Rev. Lett. 53, 1461, 1984] indicate that it is essential to take into account the non-Maxwellian character of the electron distribution for a quantitative analysis of the experimental results.

    Notes: The magnetic properties of the spheromak configuration produced by a combination of slow theta and Z discharges in the University of Maryland Spheromak experiment (MS) are reported. The magnetic structure of the plasma in MS has been mapped out by arrays of passive magnetic pickup coils. The Taylor relaxation process is observed during the formation phase. The magnetic profile evolves in such a way that the ratio of poloidal current Ip to poloidal flux ψ in the plasma approaches a constant value, where μ0Ip=kelψ. When the spheromak is formed, the magnetic field configuration is close to Taylor's minimum energy state, μ0j=kB. This constant k is related to the size of the spheromak produced. A spheromak with 1.0 T maximum field, corresponding to 650 kA poloidal current, has been produced in MS. However, due to the high plasma density (6–8×1020 m−3) and the presence of low-Z impurities (mainly carbon and oxygen), the plasma is radiation dominated with electron temperature ≤15 eV. The magnetic field decays exponentially during the decay phase. Axisymmetric equilibrium states that could exist in the configuration are calculated with a Grad–Shafranov equilibrium code. Comparison of the numerical calculation with the experimental measurements indicates that the magnetic-field structure stays close to the equilibrium state as the plasma decays.

    Notes: The effect of increasing the laser pulse rise time on the hot electron energy distribution is studied using one or three prepulses near or on the rise of a main CO2-laser pulse. For some slower rising pulses, the pressure from a hot and long gradient plasma balances the ponderomotive laser force in such a way that the hot electron energies are significantly reduced while the overall resonance absorption fraction remains almost the same. The experimental results are modeled with a 1-D hydrocode using multigroup fast electrons and a dependence of the absorption and the hot electron temperature on the density gradient scale length at the critical density. Qualitative agreement is obtained over a wide range of laser pulse conditions and rise times.

    Notes: Two cases of malignant hyperthermia are described where the earliest sign was a rise in the end-tidal CO, concentration. This led to nearly immediate detection and adequate treatment with sodium dantrolene. These cases demonstrate the efficacy of monitoring end-expired CO2 concentrations in patients at risk from malignant hyperthermia, as well as a means for following the adequacy of treatment.

    Notes: In a prospective, randomized, single-blind study of combined pulse oximetry and capnography in 196 children whose anaesthetic was managed by endotracheal intubation we observed 10 patients with 14 episodes of endobronchial intubation (EBI). Pulse oximetry provided the first diagnostic clue in 13 events; the one event first diagnosed by capnography was also accompanied by oxygen desaturation. Two EBI occurred immediately after the initial intubation (‘primary’ EBI). Twelve episodes of EBI in eight patients occurred after correct initial endotracheal tube position (‘secondary’ EBI). EBI gave rise to eight episodes of ‘minor’ desaturation (oxygen saturation (Spo2) ≤ 95%≥ 60 s); four resulted in ‘major’ desaturation (Spo2≤ 85% for 30 s). Only three EBI events produced clinical signs or symptoms. Seven patients were less than 1 year; EBI was not, however, related to age.A high frequency (5.1%) of clinically unrecognized EBI was found in infants and children. A persistent yet small, reduction in oxygen saturation provided the early evidence of secondary EBI in the majority of cases. Clinical signs and capnography proved least effective in the early diagnosis of secondary EBI.
