Notes: Summary. To determine the microbiological causes of pelvic inflammatory disease, 43 women with acute salpingitis (AS) and 160 controls were studied. Amongst AS women there were significantly higher endocervical isolation rates of Neisseria gonorrhoeae (54%vs 6%), Chlamydia trachomatis (40%vs 13%), Mycoplasma hominis (60%vs 19%), enterobacteria (26%vs 11%) and anaerobic bacteria (58%vs 29%). A polymicrobial pathogenic endocervical flora was present in both gonococcal and non-gonococcal AS. Laparoscopic sampling of the fallopian tubes rarely provided useful microbiological data but did reveal the inaccuracy of clinical diagnosis of AS. Thirty-four male consorts of AS women were investigated; 20 had gonorrhoea and 27 had non-gonococcal urethritis (NGU). A high proportion of infected men had asymptomatic gonorrhoea (35%) and/or asymptomatic NGU (56%). These findings had implications for the management of AS.

Notes: Summary The need for chemoprophylaxis for bacterial endocarditis is partly dependent on the risk of bacteraemia associated with the procedure, which has not been adequately defined for skin surgery. The incidence of postoperative bacteraemia in 149 immunocompetent out-patients with noninfected lesions was 0.7% (95% CI 0.3–3.8%). Procedures included excisions. flaps, grafts and micrographically controlled surgery. Coagulase-negative staphylococcus was the most common skin isolate at the site of surgery, present in 68.5% of patients. The most effective chemoprophylaxis would be intravenous vancomycin, which is inconvenient and has an inherent risk of morbidity.Given the low incidence of bacteraemia and the disadvantages of the optimum chemoprophylaxis, surgery on non-infected lesions does not warrant prophylactic antibiotics to prevent the very low rosk of bacterial endocarditis.

Notes: Abstract A Sudanese girl became desperately ill with liver and kidney abscesses due to Nocardia asteroides. She did not have pulmonary or cutaneous infection. She recovered after surgical drainage of the abscesses and prolonged treatment with intravenous amikacin and high dosage cotrimoxazole and sulphadimidine. After recovery normal neutrophil function, cell-mediated and humoral immunity were demonstrated.

Notes: Abstract Urethral specimens from 247 heterosexual men — 118 patients with non-gonococcal urethritis and 129 controls with no evidence of urethritis — were examined for the presence ofBacteroides ureolyticus. Bacteroides ureolyticus was isolat-ed from 45 (34.9 %) of the controls and from 31 (26.3 %) of the urethritis patients; there was no significant difference between these rates (p=0.1). There was no association between the density ofBacteroides ureolyticus on primary isolation plates in either group.

Notes: Abstract The in vitro activity of the new macrolide azithromycin and erythromycin againstHaemophilus ducreyi, Gardnerella vaginalis, Bacteroides spp. andMobiluncus spp. was compared.Gardnerella vaginalis, Mobiluncus spp. andHaemophilus ducreyi were highly susceptible to both agents but azithromycin was more effective overall against all three groups of organisms. The activity of both antibiotics againstBacteroides spp. varied greatly, but azithromycin was again more active. Collectively, 62 % of allBacteroides strains were susceptible to erythromycin (MIC 1 mg/1) and 74 % were susceptible to azithromycin.

Notes: Abstract At a time whenGardnerella vaginalis was becoming accepted as the prime, causative agent of non-specific vaginitis, it has emerged thatBacteroides species are also implicated. A brief, historical account of non-specific vaginitis is presented and the role ofBacteroides species is discussed. A method for the laboratory diagnosis of the infection is described, therapeutic concepts are reviewed and a treatment regime is suggested.
