    Description / Table of Contents: Zusammenfassung In einem 30-Monats-Zeitraum wurden 200 Inline-Skatingverletzungen bei 182 Patienten behandelt; 14 % der Patienten wurden stationär aufgenommen, 10 % operiert. In 49 % handelte es sich um Frakturen, in 27 % um Prellungen oder Schürfungen und in 16 % um Kapsel-Band-Verletzungen. Bei Kindern lag der Frakturanteil mit 62 % noch deutlich höher. 44 % aller Verletzungen und 75 % aller Frakturen entstanden durch den Sturz auf die ausgestreckten Arme. Der zweithäufigste Verletzungsmechanismus (13 %) war ein Verdrehtrauma des Beins; Stürze auf die Knie oder die angewinkelten Ellbogen betrafen nur je 5 %; 55,5 % aller Verletzungen und 71 % aller Frakturen traten an Ellbogengelenk, Unterarm, Handgelenk und Hand auf; andere häufig verletzte Regionen waren Kopf (13 %), Knie (9,5 %) und Sprunggelenk (9 %). Während die Skater ihre Gelenkschützer an den nicht verletzten Gelenken ebenso häufig angelegt hatten wie unverletzte Vergleichsskater, waren gerade die verletzten Regionen auffallend schlecht geschützt.
    Notes: Summary One hundred and eighty-two patients presented with 200 inline-skating injuries over a 30-month period. Of these, 14 % were admitted to hospital, 10 % required operative treatment. Fractures (49 %), contusions/lacerations (27 %) and capsular/ligamentuous injuries (16 %) were the most prevalent types of injury. Children had an even higher risk of sustaining fractures (62 %). Falling on the extended arm caused 44 % of all injuries. Thirteen percent resulted from torque mechanisms of the leg, whereas direct trauma to elbow or knee (5 % each) were uncommon injury mechanisms. Injuries of the elbow, forearm, wrist and hand accounted for 55.5 % of all cases and 71 % of all fractures. Head (13 %), knee (9.5 %) and ankle (9 %) were other regions frequently involved. Protective equipment was often used only for uninjured regions, whereas the injured regions had most often been left unprotected.

    Description / Table of Contents: Zusammenfassung Die Kollagenfasertextur in den Epiphysenfugen kindlicher Röhrenknochen wurde polarisationsoptisch und rasterelektronenmikroskopisch untersucht. In der Sdulenknorpelzone verlaufen Kollagenfaserbündel longitudinal in den intercolumnären Knorpelsepten. Teilweise gabeln sie sich spitzwinklig auf und umgeben rhombenförmige Knorpelzellverbände. Am Übergang zur Zone des ruhenden Knorpels gehen die Kollagenfasern in Bögen fiber, die eine arkadenartige Fasertextur bewirken. In der Zone des ruhenden Knorpels sind die Kollagenfasern scherengitterartig angeordnet. Die Bedeutung der Kollagenfasertextur für die Belastbarkeit der Epiphysenfuge wird diskutiert.
    Notes: Summary The architecture of the collagenous fibers in epiphyseal plates of infantile long bones was investigated using polarized light and scanning electron microscope. In the zone of columnar cartilage the collagenous fiber bundles are arranged longitudinally in the intercolumnar cartilaginous septa. In addition to this main longitudinal orientation, some of the fibers branch off at an acute angle and flow through the cartilage columns surrounding rhombus-shaped groups of chondrocytes. In the area adjaced to the inert cartilage the collagen fibers become curved and form a network of arcades. In the zone of inert cartilage the collagenous fibers are arranged in a honeycomb-like fashion. The importance of the arrangement of the collagen fibers to the loading strength of the epiphyseal plate is discussed.

    Notes: Abstract The histocompatibility of hydroxyapatite-ceramic (HAC) has been proven extensively. For the reconstruction of juxta-articular cancellous bone defects with this synthetic material, the mechanical properties of the HAC-bone regeneration complex needed to be investigated. In order not to alter the specific ability of the articular structures to distribute and absorb loading stress, the physiological force-transmitting performance of the subchondral zone must be achieved by filling the defect within HAC. This study deals with the influence of a physiological load on the remodelling within HAC-filled subchondral bone defects. As orientation is the important factor affecting the physical properties of hard tissue, we show the morphological aspect of functional adaptation of the hydroxyapatite-bone compound determined by the orientation of the bone collagen fibres. By biomechanical methods, the elastic properties of the resulting ceramo-osseous regeneration complex were tested. Reproducible subchondral bone defects were prepared in medial femoral condyles of rabbits, leaving a 0.5 mm coplanar layer of bone and cartilage. The defects were filled with granules of HAC. Polarizing microscopy revealed the dynamical aspect of the bony integration of the material and the remodelling process under physiological locomotion. It showed a rapid ongrowth of collagen fibres on the ceramic surface. By its increasing orientation to domains from woven texture to economical trabecular architecture, the load-bearing facility is documented. Indenting the articular surface on an impressive force testing machine 18 months after HAC implantation proved the equal elastic response of the ceramo-osseous regeneration complex with the overlying structures in comparison with the integrity of not-operated femoral condyles. When integrated by bone, HAC fulfils in our dynamic animal model physiological demands even in large bone defects close to articular surfaces.

    Notes: Abstract It is absolutely necessary to classify children's fractures for quality controls and clinical research. It is not possible that a classification of children's fractures will follow hierarchies, such as the prognosis of growth, the kind of treatment, the severity of the lesion, the patient's age, as well as medical expenditures and different techniques of therapy. The prognosis of growth is dependent on the patient's age and the localisation of the fracture in the bone and in the skeleton. The kind of treatment is dependent on the extent of displacement as well as on the localisation in the bone. Thus, a classification of children's fractures can only be characterized by the localisation of the fracture and the morphology. In addition, a parameter of “tolerable displacement” should be involved in the classification – as a sign for the possibility of spontaneous correction of displacements by further growth. From the morphological/functional point of view, the epiphyseal plate injuries should be differentiated into a metaphyseal part without proliferation but with mineralisation potential, and an epiphyseal part with proliferation but without mineralisation potential. From this aspect of functional morphology the epiphyseal separation – as a lesion in the metaphyseal part of the physis – belongs to the metaphyseal fractures (so to speak, it is the most peripheral shaft fracture), and the epiphyseal fracture – as a lesion in the epiphyseal part of the physis – belongs to the joint fractures. The localisation is differentiated into 4 main segments: upper arm, forearm, femur, and lower leg. In the segment itself, a subdivision is made into proximal, middle, and distal, with proximal and distal being further differentiated in metaphysis and epiphysis. In addition, it is possible to differentiate between radial and ulnar and tibial and fibular. The morphology is differentiated into typical fractures of the epiphysis, the metaphysis and the shaft fractures as well as avulsion fractures of ligaments, avulsion fractures of muscles and flake fractures. Exceptions in the consequent classification of morphology are only made at the distal end of the humerus and at the neck of the femur. the parameter of “tolerable displacement” showes up already known “ spontaneous corrections” of displacements in the joints, the metaphysis and the epiphysis. The proposed classification should be used in prospective clinical studies to evaluate the prognosis of growth and the results of different therapeutic approaches.

The indications for operation were second or third degree compound fractures, vessel or nerve lesions, serial fractures in both arms and for easier nursing of the aged and polytraumatised. Out of 79 conservatively treated patients 12 required secondary surgery after 8 weeks due to prolonged fracture healing. 62 (93 %) out of 67 fractures treated solely by hanging cast showed very good and good results 1–5 years after the accident — transverse fractures included — and only 5 (7 %) poor results.

    Description / Table of Contents: Zusammenfassung Erste Untersuchungen haben gezeigt, daß durch simultane Bestimmung von CAl9-9, CEA and CAl2-5 die Primar- and Rezidivdiagnostik fur Magen-, Pankreas-, Colon- and Gallenblasencarcinomen verbessert wird. Die Sensitivitat von CAl9-9 beträgt für das Pankreascarcinom 87%, fur CAl9-9 and CEA 90% and fur CA19-9, CEA and CA12-5 insgesamt 92%. Fur die anderen Carcinome liegt die Sensitivität von CA19-9 and CEA etwa in gleicher Größenordnung (ca. 25%). Bei simultaner Bestimmung aller drei Antigene 51%.
    Notes: Summary Early examinations have shown that through simultaneous determination of CAl9-9, CEA and CAl2-5, primary and recurrent diagnostics are improved in the stomach, pancreas, colon, and gallbladder. For pancreas carcinoma the sensitivity of CAl9-9 is 87%, for CAl9-9 and CEA it is 90%, and for CAl9-9, CEA and CAl2-5 the sensitivity is a total of 92%. The sensitivity for the other gastrointestinal cancers is approximately 25% for both CAl9-9 and CEA. Simultaneous determination of all three antibodies shows a sensitivity of 51 %.

    Description / Table of Contents: Zusammenfassung Wegen der besonderen anatomischen und funktionellen Gegebenheiten des Sprungbeines ist die Behandlung seiner Frakturen langwierig und oft unbefriedigend. Aus einem Zeitraum von 20 Jahren wurden 40 Patienten mit 42 zentralen konservativ und operativ behandelten Talusfrakturen nachuntersucht. Die Primärbehandlung — ob operativ oder konservativ —bot die besten Ergebnisse. Das operative wie auch das konservative Vorgehen bei Frakturen älter als 24 h ergab erschreckend häufig schlechte funktionelle Ergebnisse. Bei den Nachuntersuchungen fiel auf, daß bei schlechten Röntgenbefunden oft gute klinische und bei guten klinischen Ergebnissen oft schlechte Röntgenbefunde vorlagen. Ausgedehnte Sklerosen waren in der Regel nicht mit Talusnekrosen gleichzusetzen. Bei negativen intraossären Venographien fand sich oft histologisch lebendes Knochengewebe. Als Fazit dieser Spätergebnisse — Nachuntersuchungszeiten 1–18 Jahre — ergab sich: frische zentrale Talusfrakturen sofort operieren, veraltete konservativ behandeln, bei unbefriedigendem Repositionsergebnis notgedrungen offen einrichten.
    Notes: Summary Because of the anatomical and functional peculiarities of the astragallus surgical and conservative treatment of central fractures are often unsatisfactory. A follow-up study of 40 patients with 42 central talus fractures was performed over an average of 9 years after the initial trauma. The number of poor results was extremely high; only primary surgical or conservative treatment showed good or fair results. Best results were obtained by primary surgical treatment whereas fractures operated at later stages showed worse results. Therefore fresh central astragallus fractures should be operated upon, fractures older than 1 day should be treated conservatively. If a satisfactory reduction cannot be obtained, the fracture should be operated upon in order to prevent arthrosis in the upper ankle joint.
