    Notes: A noncontact, nondestructive optical technique with potential to measure the acceptor impurity concentrations in bulk CdTe and Cd1−xZnxTe alloys is described here. The technique, called optical quenching, uses an infrared light source in conjunction with the above-band-gap excitation beam used in standard photoluminescence spectroscopy. The quenching, or decrease, of acceptor-bound exciton emission is related to the incident infrared beam flux. The rate at which the infrared photons ionize neutral acceptors can be used to determine relative concentrations of the substitutional centers. We have applied this technique to the copper acceptor-bound exciton in CdTe and Cd1−xZnxTe bulk samples. The 1.064 μm output from a Nd:YAG laser provided near-resonant absorption with the copper acceptor level. Relative copper concentrations in the range from 4×1014 to 1×1016 cm−3 were obtained. These values agree reasonably well with sample comparisons based on atomic absorption data.

    Notes: A general formalism is presented for the production, characterization, and use of uniform supersonic flows for the study of structural and collisional molecular properties. These flows make possible the generation of thermally equilibrated gaseous environments at temperatures generally extending from near 10 to above 300 K at pressures between 0.1 and 10 Torr. In addition, the invariance of flow conditions for distances of many nozzle diameters (5–30) beyond the exit makes the flows ideal for collisional or temporal studies. A concise outline for the design of the Laval nozzles necessary to produce specified density and temperature conditions is presented. Pulsed operation of these expansions is demonstrated, and a variety of useful characterization tools providing information on flow uniformity and thermal characteristics is considered. Recent applications of these flows for the study of low-temperature chemical reactions are reviewed. © 1995 American Institute of Physics.

    Notes: The origins of gyroremanent magnetization (GRM) in single domain particles are studied using a dynamic model based on the Landau–Lifschitz equation. The results show an asymmetry in the motion of the magnetic moment of a particle with three unequal axes of anisotropy. This asymmetry is interpreted in terms of an effective bias field which gives a good qualitative explanation of the origins of GRM.

    Notes: Four-level microwave–microwave double-resonance measurements have been carried out for a number of pump–probe pairs in the inversion spectrum of NH3 in dilute mixtures of NH3 in He. The results completely confirm corresponding measurements previously carried out by Oka with a different apparatus in a different laboratory [Adv. At. Mol. Phys. 9, 127 (1973)]. The original data had been questioned by Davis and Green on the basis of a comparison of observed and calculated results [J. Chem. Phys. 78, 2170 (1983)].

    Notes: Background:  The impact of gastro-oesophageal reflux disease on work productivity has become increasingly important, as the symptoms of gastro-oesophageal reflux disease affect individuals in their productive years of life.Aims:  To assess the impact of gastro-oesophageal reflux disease on reduced work productivity and to identify the predictors of reduced productivity.Methods:  A sample of employed individuals reporting chronic heartburn was selected from US household mail survey respondents. Heartburn severity and frequency were recorded using a diary, and work productivity was assessed using the Work Productivity and Activity Impairment Questionnaire for Patients with Symptoms of Gastro-oesophageal Reflux Disease. Predictors of reduced productivity were evaluated.Results:  Over 30% of heartburn sufferers reported reduced productivity. Individuals with symptoms of gastro-oesophageal reflux disease (n = 1003) reported 6.0% reduced productivity attributable to symptoms. Over 48% of respondents with severe symptoms reported reduced productivity, compared with 40% and 12% of respondents with moderate and mild symptoms, respectively. Using logistic regression, severity, a younger age and nocturnal symptoms were associated with increased odds of reduced productivity. In those reporting nocturnal heartburn, medication use and sleep interference increased the odds of reduced productivity.Conclusions:  Reduced work productivity is seen in a large proportion of subjects on prescription medication for gastro-oesophageal reflux disease. Symptom severity and nocturnal heartburn are significantly associated with reduced work productivity, particularly when nocturnal heartburn interferes with sleep.

    Notes: Background : Near-infrared spectroscopy (NIRS) is a noninvasive optical monitor of regional cerebral oxygen saturation (rSO2). The aim of this study was to validate the use of NIRS by cerebral oximetry in estimating invasively measured mixed venous oxygen saturation (SvO2) in pediatric postoperative cardiac surgery patients.Methods : Twenty patients were enrolled following cardiac surgery with intraoperative placement of a pulmonary artery (PA) or superior vena cava (SVC) catheter. Five patients underwent complete biventricular repair – complete atrioventricular canal (n = 3) and other (n = 2). Fifteen patients with functional single ventricle underwent palliative procedures – bidirectional Glenn (n = 11) and Fontan (n = 4). Cerebral rSO2 was monitored via NIRS (INVOS 5100) during cardiac surgery and 6 h postoperatively. SvO2 was measured from blood samples obtained via an indwelling PA or SVC catheter and simultaneously correlated with rSO2 by NIRS at five time periods: in the operating room after weaning from cardiopulmonary bypass, after sternal closure, and in the CICU at 2, 4, and 6 h after admission.Results : Each patient had five measurements (total = 100 comparisons). SvO2 obtained via an indwelling PA or SVC catheter for all patients correlated with rSO2 obtained via NIRS: Pearson's correlation coefficient of 0.67 (P 〈 0.0001) and linear regression of r2 = 0.45 (P 〈 0.0001). Separate linear regression of the complete biventricular repairs demonstrated an r = 0.71, r2 = 0.50 (P 〈 0.0001). Bland–Altman analysis showed a bias of +3.3% with a precision of 16.6% for rSO2 as a predictor of SvO2 for all patients. Cerebral rSO2 was a more accurate predictor of SvO2 in the biventricular repair patients (bias −0.3, precision 11.8%), compared with the bidirectional Glenn and Fontan patients.Conclusions : Regional cerebral oximetry via NIRS correlates with SvO2 obtained via invasive monitoring. However, the wide limits of agreement suggest that it may not be possible to predict absolute values of SvO2 for any given patient based solely on the noninvasive measurement of rSO2. Near-infrared spectroscopy, using the INVOS 5100 cerebral oximeter, could potentially be used to indicate trends in SVO2, but more studies needs to be performed under varying clinical conditions.

    Notes: Summary Background: Tetralogy of Fallot with absent pulmonary valve is a rare form of congenital heart disease. The records of patients with this lesion were reviewed over a 6½ year period, and the perioperative management of 13 patients is described. Methods: We found that our patients could be divided into two groups by age. The younger group failed medical management and was referred for surgery at less than 1 year of age. This group of patients had considerable respiratory disease at the time of surgery, greater ventilatory problems during surgery and prolonged recovery. Most patients in this group have residual respiratory disease. Results: There were significant differences in postoperative outcome between younger versus older patients: days of ventilation 37 ± 41 versus 1 ± 0.6 days, length of ICU stay 28 ± 31 versus 3 ± 1 days and length of hospital stay 64 ± 48 versus 9 ± 5 days.
