    Notes: Summary 105 patients were examined one to fifteen years after reduction mammaplasty or mastopexy. The technique of McKissock, Strömbeck and Pitanguy had been used. The appearance of the scars, the width of the scars and the sensation of the areola in the three different methods of operative treatment were compared. The study showed that hypertrophy of the scars and areola sensation do not depend on the type of operative technique. However, as expected the width of the scars is significantly greater in case of reduction mammaplasty compared to mastopexy.

    Notes: Summary The selective reunion of motor and sensory fascicles of severed mixed nerves appears indispensible for optimal recovery of the impaired motor function. Procedures available for rapid identification of motor and sensory fascicles rendered ambiguous results. The only highly reliable and simple method marking motor fascicles, namely acetylcholinesterase histochemistry, neccessitated two operations due to its long duration (28 h). In the present study rapid demonstration of acetylcholinesterase activity was accomplished by elevating the temperature and the concentration of 2 constituents of the incubation medium. Now incubation takes 2 h instead of 20 h. Thus, the time necessary for the entire diagnostic procedure could be reduced from 28 h to 4 h. Indeed, intraoperative motorsensory differentiation using acetylcholinesterase activity as a marker appears feasible. The new technique has already been applied to 6 clinical cases of acute nerve injuries of the forearm. The histochemical results obtained are comparable to those of the standard procedure indicating equally high reliability of both methods. Finally, opportunities for further reducing the duration of the modified histochemical procedure and its applicability to fascicular nerve grafting are discussed.

    Notes: Abstract A histological and morphometric study was undertaken in order to evaluate the alterations in sural nerves harvested for nerve grafting using a nerve stripper. In 18 nerves, biopsies were taken from the proximal and/ or the distal end of the stripped nerve graft. Cross sections were examined for alterations of the perineurium and the myelin sheaths. In four nerves, alterations inside to the perineurium were found, which involved 37.4% of the endoneurial space on the average. The results of the present study suggest that harvesting of a nerve graft by the stripper does not cause major injuries to the graft and, therefore, successful neurotization of the graft is not impaired.

    Notes: Abstract  The aim of this study was to compare the influence of two different types of antithrombogenic medication and the technique of anastomosis on flap survival in free tissue transfer. In 81 patients, the postoperative medication was dextran and heparin (Group 1), in 123 patients heparin only (Group 2). After dextran and heparin medication arterial thrombosis occurred in six patients (7.4%), after heparin therapy only in eight patients (6.5%, p=0.79). In 154 patients an arterial end-to-end anastomosis, and in 50 patients an end-to-side anastomosis was performed. Arterial occlusion occurred in 8.9% after end-to-end anastomosis and in no case after end-toside anastomosis (p〈0.02). Total flap necrosis occurred in 11 patients (5%), a partial flap necrosis occurred in three patients (2%). The results suggest that it is only the surgical method of anastomosis which has an influence on the survival rate of free flaps, the postoperative medication has no effect.
