Résumé Dans le cadre du syndrome de Duane on peut differencier 3 types:Duane I avec parésie de l'abduction,Duane II avec parésie de l'adduction etDuane III avec parésie de l'abduction et de l'adduction. Les 3 types manifestent d'une manière characteristique une fermeture de la fente palpebrale et une rétraction du globe pendant l'effort de l'adduction. Les analyses éléctromyographiques ont révélé une innervation paradoxale du droit externe comme principe pathogénique du syndrome de Duane. Afin d'expliquer cette innervation paradoxale on discute la possibilité d'une innervation anormale du droit externe avec des branches du IIIème nerf seul (Duane III) ou une innervation double avec des branches du VIème nerf et du IIIème nerf (Duane II, Duane I). Dans des travaux anatomiques du siècle dernier on trouve des descriptions de cas présentant une telle innervation anormale, mais jusqu'ici il n'était pas possible de mettre en relation ces anomalies anatomiques avec l'image clinique d'un syndrome de Duane.
Abstract:
Summary Three types of the Duane retraction syndrome can be differentiated:Duane I with palsy of abduction,Duane II with palsy of adduction andDuane HI with palsy of adduction and abduction. All types manifest characteristic more or less pronounced narrowing of the lid fissure and retraction of the globe on attempting adduction. Electromyography has shown a paradoxical innervation of the external rectus to be the pathogenic principle of Duane's syndrome. Such a paradoxic innervation may be due to an abnormal contact of eye muscle nerves with the external rectus, such as a contact only with branches of the oculomotor nerve (Duane III) or a double contact with the abducens nerve and branches of the oculomotor nerve (Duane II, Duane I). There are some anatomical observations, dating from the last century, describing such abnormal contacts of eye muscle nerves with the external rectus, but until to-day no correlation between these findings and the clinical entity of Duane syndrome could be established.

Zusammenfassung Im Rahmen des Duane-Syndromes lassen sich als klinisch unterscheidbar 3 Typen herausdifferenzieren,Duane I mit vorwiegender Abduktionsbehinderung,Duane II mit vorwiegender Adduktionsbehinderung undDuane III mit Ab- und Adduktionsbehinderung, wobei allen Typen die mehr oder weniger ausgesprochene Lidspaltenverengerung und Retraktion des Bulbus beim Versuch zur Adduktion des befallenen Auges gemeinsam ist. Auf Grund elektromyographischer Untersuchungen steht heute fest, daß den Duane-Syndromen als pathogenetisches Prinzip eine paradoxe Innervation des Reclus externus des befallenen Auges zugrunde liegt. Für die Deutung dieser paradoxen Innervation wird die Möglichkeit einer abnormen Versorgung des Rectus externus mit Zweigen des Oculomotorius allein (Duane III) oder eine Doppelversorgung sowohl durch Zweige des Abducens und des Oculomotorius (Duane II, Duane I) diskutiert. Diesbezügliche anatomische Befunde sind beschrieben, jedoch bis heute noch nicht in Korrelation mit dem klinischen Befund eines Duane-Syndromes gebracht worden.
