    Description / Table of Contents: Résumé La thermographie d'infrarouge a permis de démontrer les anomalies caractéristiques de l'image d'émission au niveau des pieds et des mains de 16 jeunes diabétiques avec et sans manifestations de complications vasculaires. La thermographie d'infrarouge semble être une méthode utile pour l'étude de la circulation et du métabolisme au niveau des mains et des pieds chez les diabétiques.
    Abstract: Zusammenfassung Die Infrarot-Thermographie zeigte typische Abnormitäten in den Emissionsbildern über Händen und Füßen von 16 jungen Diabetikern mit oder ohne manifeste Gefäßkomplikationen. Die Infrarot-Thermographie erscheint methodisch gut geeignet für weitere Untersuchungen des Kreislaufs und Stoffwechsels in den Händen und Füßen von Diabetikern.
    Notes: Summary Infrared thermography revealed characteristic abnormalities in the emission patterns over the hands and feet of 16 young diabetics with or without manifest vascular complications. Infrared thermography seems to be a method well suited for further studies of circulation and metabolism in the hands and feet of diabetics.

    Description / Table of Contents: Résumé La rhéologie de cellules sanguines intravasculaires et les relations entre ces cellules et l'endothélium ont été analysées in vivo par la microscopie vitale de haute résolution chez quatre diabétiques ayant la maladie depuis 3 à 15 ans. Dans un fragment de peau préparé chirurgicalement en forme de tube a été implantée une chambre de titane avec un système optique. Une mince couche de tissu a été observée au microscope pendant une période allant jusqu'à quatre mois. La structure microvasculaire et la fonction ont été analysées sous des conditions de circulation normale et pendant différentes situations de test avec blocage partiel ou total de la circulation, pendant une lipémie et après des traumatismes locaux légers. En contradiction avec les opinions publiées dans la littérature, il n'y a pas d'aggrégation de cellules sanguines chez des diabétiques, sauf au cours d'une perturbation métabolique prononcée comme dans l'acidose. La rhéologie des différents types de cellules sanguines est normale même dans le diabète de longue durée. On donne une explication du concept admis jusqu'à maintenant, de l'aggrégation des cellules sanguines dans le diabète.
    Abstract: Zusammenfassung Die Rheologie intravasculär untersuchter Blutzellen und die Beziehungen zwischen Blutzellen und Endothelium wurden vitalmikroskopisch unter hoher Auflösung bei vier Diabetikern mit unterschiedlicher Krankheitsdauer in vivo analysiert. In einen zweizipfligen, röhrenförmigen Hautlappen wurde eine Titanium-Kammer mit Optik implantiert. In dieser Kammer wurde eine dünne Gewebsschicht bis zu vier Monaten mikroskopisch untersucht. Die mikrovasculäre Struktur und ihre Funktion wurden unter normalen Strömungsverhältnissen und während verschiedener Test-Situationen mit partieller oder totaler Strömungsunterbrechung bei Lipämie und nach leichten lokalen Gewebstraumen analysiert. Im Gegensatz zu bisher in der Literatur vertretenen Ansichten fand sich dabei nur dann eine Aggregation von Blutzellen bei Diabetikern, wenn es unter einer Azidose zu einer schweren Stoffwechselstörung gekommen war. Auch bei Langzeitdiabetes waren die Strömungseigenschaften der verschiedenen Blutzellarten normal.
    Notes: Summary The rheology of blood cells studied intravascularly, and the relation between blood cells and endothelium was analyzed in vivo by high resolution, vital microscopy in four diabetic patients with varying duration of the disease. In a dipedicled, tubed skin-flap a titanium chamber with optics was implanted. A thin layer of tissue in this chamber was observed microscopically over a period of up to 4 months. Microvascular structure and function were analyzed under normal flow conditions as well as in various test situations with reduced or completely blocked flow in lipaemia and when slight local tissue trauma was added. It was found, contrary to what has hitherto been considered in the literature, that there is no aggregation of blood cells in diabetes, unless the metabolic balance is heavily upset as in acidosis. The rheology of the various types of blood cells is normal even in long-standing diabetes.

    Notes: Summary The effect of long-term treatment with phenformin and metformin respectively on blood lactate concentrations in relation to submaximal muscular exercise has been examined in 21 maturity-onset diabetics, using a cross-over method. At similar degrees of diabetic control the mean blood lactate concentration during exercise and shortly thereafter was significantly higher when the patients had received phenformin. The mean fasting lactate concentration was 1.07 mmol/l with phenformin and 1.03 mmol/l with metformin and the peak concentration was 2.56 mmol/l and 2.19 mmol/l respectively. The mean fasting blood glucose concentration before the exercise was 11.2 mmol/l with phenformin and 11.3 mmol/l with metformin; the glucose output in the urine during the preceding 24 hours was 93 mmol and 105 mmol respectively. The mean work load during exercise was 60 watts.

    Description / Table of Contents: Résumé L'hématocrite, le volume plasmatique et la viscosité chez 15 diabétiques n'étaient pas notablement différents au repos par rapport aux dix témoins. Après un exercice modéré, le volume plasmatique diminuait et l'hématocrite augmentait significativement plus que chez les témoins. Dans les sous-groupes diabétiques, basés sur les degrés variables d'angiopathie dermique, on n'a pas démontré de différences significatives. Dans des conditions circulatoires stables, pendant l'exercice, les diabétiques répondaient par une augmentation marquée de la pression systolique du sang. La quantité de filtrat du lit vasculaire aux tissus interstitiels n'était pas en corrélation avec la pression systolique accrue. La différence dans l'intensité du travail entre les groupes n'est pas telle qu'elle puisse influencer l'hématocrite ou le volume plasmatique. Ces découvertes peuvent indiquer une perméabilité accrue des parois capillaires à cause des modifications caractéristiques de la microangiopathie diabétique.
    Abstract: Zusammenfassung Der Hämatokrit, das Plasmavolumen und die Plasmaviscosität unterschieden sich im Ruhezustand bei 15 Diabetikern nicht von bei 10 Vergleichspersonen gemessenen Werten. Nach mäßiger körperlicher Belastung kam es zu einem signifikant stärkeren Rückgang des Plasma-Volumens und Anstieg des Hämatokrits als bei den Vergleichspersonen. Bei weiterer Unterteilung des Diabetikerkollektivs je nach Ausprägung von Haut-Angiopathien ließen sich keine signifikanten Unterschiede nachweisen. Unter steady state Bedingungen kam es bei den Diabetikern unter der Belastung zu einem erheblichen Anstieg des systolischen Blutdruckwertes. Die Menge des Filtrates zwischen Gefäßbett und den interstitiellen Geweben ließ sich nicht mit dieser Erhöhung der systolischen Blutdruckwerte korrelieren. Die Unterschiede der Arbeitsintensität beider Gruppen erreichten keine Größenordnung, die den Hämatokrit oder das Plasmavolumen hätte beeinflussen können. Diese Befunde könnten auf eine Zunahme der Permeabilität der Capillarwände infolge von Veränderungen hindeuten, wie sie für die diabetische Mikroangiopathie charakteristisch sind.
    Notes: Summary Haematocrit, plasma volume and viscosity were not notably different at rest in fifteen diabetics from those in ten controls. After moderate exercise, plasma volume was reduced and haematocrit was increased significantly more than those in controls. In the diabetic subgroups based on varying degrees of dermal angiopathy, no significant differences were demonstrated. In circulatory steady state during exercise the diabetics responded with a marked increase of systolic blood pressure. The amount of filtrate from the vascular bed to the interstitial tissues was not correlated to increasing systolic pressure. The difference in work intensity between the groups is not of such an order that it can be assumed to influence haematocrit or plasma volume. These findings may indicate increasing permeability of the capillary walls because of changes characteristic of diabetic microangiopathy.

    Notes: Summary Twelve newly diagnosed and eleven previously unsuccessfully treated diabetics were included in a strictly controlled cross-over trial comparing the efficacy of glipizide with that of a placebo or the previous orally administered antidiabetic drug. All newly diagnosed diabetics responded to glipizide and in eleven of them (91%), the control of the glycaemia was “excellent” or “good”. In the cases with severe diabetes, glipizide achieved on an average a better control of the glycaemia due to meals than that produced by the previous therapy. — Neither hypoglycaemia nor side-effects have been observed. We therefore conclude that glipizide is an effective and safe high potency oral sulphonylurea.

    Notes: Summary The effects of a new sulfonylurea (Ro 6-4563) have been studied on glucose tolerance, plasma insulin, lipids, and lipoprotein pattern in 29 patients with maturity onset diabetes. After 4 weeks of treatment 3 different reactions have been distinguished and are discussed: I. No response, viz. same glucose tolerance and insulin levels before and after treatment. II. Improved glucose tolerance without increased insulin levels. III. Improved glucose tolerance with higher insulin levels. Sulfonylurea treatment had no effect on plasma lipids or lipoprotein pattern. Clinically “good blood sugar control” could not be correlated with good lipid control. Response II supports the view that sulfonylurea compounds do not necessarily act by increasing plasma insulin levels.

    Notes: Abstract Urinary infections are frequent in diabetics. The cause is not clear. Neurogenic disturbances of the urinary bladder might be an important factor. Twenty-eight diabetic males have been investigated, none above 50 years of age. These patients have been compared with 25 controls. Particularly the presence of symptoms and signs from the urinary tract, the appearance of neuropathy and diabetic angiopathy have been studied. The results of the cystometry and micturition-cystography showed among other things that many diabetics, even when symptomless, had a large hypotonic or atomic urinary bladder and sphincter disturbances. The pelvic bottom electromyography showed that the potential durations in diabetics were significantly larger than in controls both in urethra and in anal sphincter as well as in levator ani. p ]Similar investigations in diabetics of different ages have yielded the same results. These results are discussed.
