    Notes: Abstract In 16 patients with mucocutaneous lymphnode syndrome (MCLS) during the first 2 weeks after the onset (acute phase) and 1 month after the onset (remission phase), measurement of the circulating immune complex (CIC) was performed by a C1q-binding assay (C1q-B.A.) and/or a Protein-A precipitation test (Protein-A P.T.). Seven out of 12 samples and four out of nine samples were shown to have raised levels of CIC in the acute phase with the C1q-B.A. and Protein-A P.T. test. In the remission phase, on the other hand, positive results were found in one out of six samples with the C1q-B.A. test and in three out of nine samples with the Protein-A P.T. test. High levels of CIC and disease activity were parallel. Our finding of a higher incidence of positive CIC in MCLS supports the possibility of the immunopathological mechanism.

    Notes: Abstract Background: The usefulness of transthoracoscopic needle biopsy for preoperatively indeterminate intrapulmonary nodules was evaluated. Methods: Thoracoscopy was performed on 38 patients with pulmonary solitary nodules suspected to be primary lung carcinomas. When the nodule was localized by simple observation or tactile sensor, a biopsy specimen of the tumor was obtained by a biopsy needle introduced through a trocar. Results: The nodules were 7 to 55 mm in diameter. All were located in the peripheral region of the lung. Biopsy specimens were obtained even from 17 nodules with no associated pleural changes. By cytology, all the malignant tumors were precisely diagnosed, 29 as primary lung cancers and 3 as metastatic lung neoplasms. Five of the remaining six benign nodules were not precisely diagnosed. However, they were cytologically classified as class I. Conclusions: Transthoracoscopic needle biopsy is feasible for diagnosing small intrapulmonary nodules, particularly those of malignant neoplasms. As compared with thoracoscopic excisional biopsy, transthoracoscopic needle biopsy saves time and may reduce the possibility of tumor dissemination during the procedure.

    Notes: Flexible and rigid epoxy and urethane polymers containing heparin chemically and uniformly incorporated throughout and/or ionically complexed to quaternized amine grops were prepared and shown to have excellent thromboresistance in vitro and in vivo. These heparin-modified polymers were implanted into the vena cava of dogs and successfully passed multiple acute (2 hr) and chronic (2 week) exposure periods. It was also demonstrated that the chemically coupled heparinized epoxies had no measurable adverse effects on blood cells, platelets, or plasma proteins, and that the heparin was not extractable. Significantly, epoxies containing polyoxypropylene/polyoxyethylene glycol (an additive which had no thrombus-inhibiting properties of its own) also exhibited excellent thromboresistance when characterized in vivo in dogs. The applicability of these various materials for the fabrication of circulatory assist device components was demonstrated. The effect of polymer polarity, surface charge, and conductivity were investigated with indeterminate results and unmodified commercial polymers uniformly exhibited poor clotting characteristics. In vitro clot-inhibiting characteristics were shown to be a sufficient, but not necessary, condition for indicating in vivo thromboresistant activity in polymer systems.

    Notes: Organic-free specimens of 5 metals were prepared and characterized by parameters indicating their relative surface charge, surface texture, and surface energy prior to evaluation of their degree of thromboresistance. For the materials studied, there was no predictive correlation between the pre-implatation electrical rest potentials and ultimate thromboresistance when standard prosthetic rings were placed in the canine inferior vena cava. Surface textural inhomogeneities greater thatn a micrometer or so made thrombus formation a certainty. There was a positive postimplantation correlation between the presence of a thin, labile, proteinaceous deposit on the prosthetic ring lumens and the passivation of the “high-energy” materials towards thrombus accumulation. The metals which did consistently passivate, 316 Stainless Steel and Stellite 21, did so dynamically by acquiring an organic film within 2 hr after implantation. During continued residence in fresh, flowing blood these metals ultimately became dominated by an apparently monodisperse protein component of low critical surface tension. These findings were confirmed when thromboresistant pyrolytic carbon having initial surface properties similar to the metals but lacking reactivity and corrosivity was used as a comparison standard.

    Notes: Glow discharge treated (GDT) inorganic materials are sterile and free of all organic contaminants. Such materials implanted in the canine inferior vena cava often demonstrate significant thromboresistance. Standard Pyrex glass tubing was GDT in argon to provide a surface free energy above 70 dynes/cm, slight surface negativity, and other surface-physical modifications of its interface. Chromic acid cleaned lengths of the same tubing, and short segments whose edges were not fire-polished were used as controls. Upon canine implantation, the in vivo results were similar in independent surgical laboratories. The glass was implanted as 1.4 cm long rings or as 5 cm long tubes in separate medical centers according to different protocols. GDT specimens remained patent; all controls accumulated thrombi. A remarkably pure, low critical surface tension, labile protein coating covered all cylindrical lumens after 2 hr in vivo. This coating diminished within 2 weeks and was essentially absent after 480 days, even though scanning EM showed evidence of a micron thick luminal film, probably eroding glass. No emboli were found in the implant animal's kidney or lungs at any stage.
