    Description / Table of Contents: Summary The frequency of recurrences (40%–60%) found during follow-up of closed technique for cholesteatoma surgery suggests that this technique should be systematically carried out in two stages. The greater frequency of recurrence in children indicates the necessity for revision surgery when the closed technique has been used. Where for some reason this cannot be undertaken, the open technique should be chosen at the outset. In case of revision, the safety offered by the closed technique is comparable with that obtained by the open technique and the rate of recurrence would be less than 5%.
    Notes: Zusammenfassung Die Rezidivhäufigkeit (40–60%), die bei der Nachuntersuchung von Cholesteatomoperationen mit geschlossener Technik festgestellt wurde, verlangt konsequent die operative Revision, die in der Regel klein gehalten werden kann. Die große Rezidivhäufigkeit bei Kindern erfordert dagegen bei Anwendung dieser Technik eine ausgedehnte operative Kontrolle. Ist eine solche Kontrolle nicht möglich, so sollte sofort die offene Technik angewendet werden.

    Notes: Summary The authors present their current experience with stored bone grafts, using allografts shaped from the cortices of long bones for reconstructing the tympano-ossicular chain. The materials and the methods are described. The anatomical results have been good in 97% of the cases, while the functional results are as satisfactory as those obtained with bioceramics. In addition to ossiculoplasty, the bone allografts can also be used in otology for reconstructing large bony defects of the temporal bone.

    Notes: Summary In 1987, at the Congress of Implants in Otology in Venice, Italy, we first presented our experiences with stored lyophilized bone allografts for reconstruction of the tympano-ossicular chain. Since then, we have extended our experiences to include reconstruction of the external bony ear canal, the filling of defects in the mastoid bone, reconstructive nasal surgery and rehabilitation of major bony defects of the face. Additionally, we have tried to improve the preparation of the bony implants used clinically and have started a multicenter collaboration with other European universities.

    Notes: Abstract Fifty patients with otosclerosis and tinnitus were studied for the course of the tinnitus after stapes surgery. Tinnitus disappeared in 64% of the cases, improved in 16%, was unchanged in 14%, and worsened in 6%. The reduction in tinnitus was more favorable after a small fenestra stapedotomy than after a partial stapedectomy with removal of the posterior half of the footplate. An unfavorable postoperative course of tinnitus did not appear to be linked to the postoperative audiometric result. In our study the subjective evaluation of pitch and loudness of the tinnitus did not provide significant information regarding its prognosis.

    Notes: Summary We describe our experience with clinical tolerance to total and partial ossicular prostheses using ceramic and plastipore and the functional results obtained. We have found that ceramic is well tolerated (with a 4% incidence of rejection) in contrast to plastipore (which had a 19% incidence of rejection). The ceramic partial and total prostheses give satisfactory function results, and 63% and 62% of our patients, respectively, were found to have postoperative air-bone gaps of less than 20 dB, and average air-bone gap gains of 16 and 21 dB.

    Notes: Summary We examined a group of 115 patients suffering from tinnitus and have attempted to find a statistically significant correlation between the tinnitus experienced and low blood zinc levels (hypozincemia). No particular correlation could be made between the nature of the tinnitus experienced and hypozincemia except for the continuation of head noises, these being more frequently associated with hypozincemia when they are intermittent. Two hypotheses of pathogenesis are proposed to explain this phenomenon and are based on the importance of zinc to the syntheses of enzymes and proteins.

    Notes: Abstract Cogan's syndrome is a rare disorder that is characterized by a rapid onset of bilateral severe sensorineural hearing loss associated with vertigo and tinnitus, ocular involvement (most commonly an interstitial keratitis) and variable systemic symptoms. We review our experience with four cases treated in our department. We describe their symptoms, the evolution of their disease, the diagnosis, and the radiological investigations with computed tomography and/or magnetic resonance imaging. When medical treatment with corticosteroids failed and no benefit could be obtained with hearing aids at each patient's end stage, all patients received a cochlear implant with excellent results. The considerations for implantation in these patients are discussed.

    Description / Table of Contents: Summary Dynamic study of the acoustic reflex has brought into evidence that 1. age has a significant influence on various reflex response parameters 2. study of dynamic parameters of the acoustic reflex enables changes in the reflex are to be objectified. This is therefore an objective measurement of recruitment in impedance measurement. The author proposes to study the following parameters: rise time, recovery time, and the variations of maximum amplitude as function of sound stimulation intensity. Study of these three parameters provides a highly sensitive test of change in the dynamics of the acoustic reflex, for indeed this test reveals pathology well before the Reflex Decay Test does so.
    Notes: Zusammenfassung Untersuchungen der Dynamik des akustischen Reflexes mittels Impedanzmessung an 1660 Ohren zeigten, daß das Alter einen entscheidenden Einfluß auf verschiedene Reflexparameter ausübt. Daneben gestattet die Prüfung dynamischer Parameter dieses Reflexes, Änderungen im Reflexbogen zu objektivieren und ermöglicht so eine objektive Messung des Recruitment. Der Verfasser beabsichtigt nun, Latenz, Erholungszeit und Änderung der maximalen Amplitude als Funktion der Reizintensität zu untersuchen. Die Untersuchung dieser drei Parameter verspricht einen hochempfindlichen Test, der nach seiner Meinung pathologische Veränderungen früher aufzeigt als der Reflex Decay-Test.
