    Notes: Specific PBMC proliferation to penicillin represents one of the in vitro diagnostic procedures to show antigenic sensitization. Nine patients with suggestive clinical history of penicillin hypersensitivity, significant stimulation index in the lymphocyte transformation test (LTT), and increased CD25 expression were selected. Their phenotypic expression of T-cell subset markers was compared with that of six normal subjects after 7 days of culture. Higher percentages of CD45R and CDw29 positive cells were found in penicillin-hypersensitive patients. This group of patients showed a significantly (P 〈 0.01) higher percentage of CD45R positive cells compared with normal subjects, after 7 days of culture. The percentage of CD45R cells did not differ between patients and normal subjects at day 0, and the progressive decrease of CD45R positive cells in culture of normal subjects was not observable in our patients. These results suggest that the patients with penicillin hypersensitivity have an imbalance of CD45R/CDw29 ratio after 7 days of culture, with a higher expression of CD45R positive cells.

    Notes: Studies made in our laboratory have demonstrated the presence in atopic serum of enhancing factors(s) for in vitro IgE synthesis by peripheral blood lymphocytes (BPL). Here we show that these enhancing factors can be recovered from human serum by means of immunoabsorbents. This factor is able to stimulate in vitro IgE synthesis by PBL independent of presence of IgE.

    Notes: Proxicromil is a compound shown in biological tests to have anti-allergic properties. It is well absorbed in humans after oral administration. In order to assess its anti-allergic activity in humans, experiments were carried out in asthmatic patients to investigate its inhibitory effects upon the bronchoconstrictor response to antigen challenge.Two controlled experiments were carried out according to randomized double blind crossover designs, in each of which ten selected patients were pretreated orally with proxicromil before being challenged with inhaled doses of antigen. Inhibition of the immediate response to antigen challenge by single and repeated doses was measured by intermittent recordings of forced expiratory volume in one second (FEV1) for 30 minutes after challenge. The duration of action of proxicromil and studied by varying the time between administration of the last dose given and inhalation of the antigen.The results showed that proxicromil significantly protected patients against the immediate response to challenge, and possessed a long duration of action of at least 12 hours.

    Notes: Patients with allergic alveolitis to various antigens, asymptomatic exposed individuals and healthy control subjects were studied. The lymphocyte transformation test and the formation of two lymphokines were shown to be positive in almost all affected subjects and only rarely in asymptomatic exposed individuals. In most symptomatic subjects, the presence of antigen-antibody complexes could be demonstrated. However, such complexes were also present in six of the nine asymptomatic exposed individuals having precipitating antibodies.Factor(s) released from sensitized monocytes strongly facilitate the in vitro formation of specific antibodies but are poorly effective when incubated with cells from asymptomatic exposed subjects or normal individuals.

    Notes: A large range of immunological investigations in patients with extrinsic or intrinsic asthma showed higher leucocyte counts in intrinsic asthma. Blood sputum eosinophilia were almost equally frequent.Immediate skin responses to inhalant antigens are seldom present in intrinsic asthma, but delayed responses to bacterial antigens, endotoxins, Candida albicans and aspiryl–PPL are frequently positive. This correlates with the frequent presence of precipitins and in vitro stimulation of lymphocytes, and the production of macrophage migration inhibition factor by most of these substances. These results suggest that bacterial factors may play a prominent role as aetiological agents in at least some cases of intrinsic asthma. In intrinsic asthma there was a high incidence of tissue autoantibodies, whereas the incidence was negligible in patients with atopic asthma.The level of serum immunoglobulins was not statistically different between the two groups of patients except for IgE.The patients with intrinsic asthma showed a favorable response to sodium cromoglycate.

    Notes: The efficacy of a 2% solution of sodium cromoglycate has been assessed in the treatment of chronic perennial rhinitis by a double-blind group comparative trial over a period of 4 weeks. The solution was administered to the nose as a fine spray delivered by a metered device at a frequency of six times a day; thus the daily dose of active drug was approximately 31.2 mg.The patients' response was measured by both subjective and objective parameters; in addition nasal smears were examined for eosinophils before and after treatment.Analyses of the subjective data demonstrated a significant difference between the active and placebo solutions at the end of the trial. The objective data was obtained by measurement of nasal resistance and flow at weekly intervals; there was a highly significant difference between the two treatment groups compared to the baseline measurements after only 1 week's treatment. There was a reduction of eosinophils in the nasal smears in those patients allocated the SCG solution, there was virtually no change in those who received placebo; the difference was significant.This study has demonstrated that a 2% solution of SCG is effective in the treatment of perennial rhinitis. It has been shown that measurements of nasal airways resistance and flow are sensitive indices of changes in the mucous membrane of the nose and that beneficial effects of SCG solution can be detected after 7 days treatment.

    Notes: A typical case of pulmonary eosinophilic granuloma, ascertained by lung biopsy, is reported. Immunological studies showed multiple hypersensitivity against wood dust extracts, horse serum, horse hair and danders proteins. Inhalation of these two groups of substances reproduced most of the acute symptoms of the disease. Therefore, a causal relationship seemed highly probable.To our knowledge, this is the first case of pulmonary eosinophilic granuloma in which an allergic origin could be substantiated.

    Notes: Eighty patients suffering from documented hay fever have been entered in a double-blind randomized study with the purpose of evaluating the efficacy of Allerglobuline®, a 10% human immunoglobulin preparation of placental origin, in the treatment of hay fever. The placebo was a 10% human albumin solution. The treatment started 6 weeks before the customary onset time of the clinical symptoms. It included a course often pre-seasonal intramuscular injections, with 1 vial (5 ml) given twice a week, and followed by a booster injection of 1 vial on 15 June and 30 June during the grass pollen season.As the two groups of patients were similar, it was possible to demonstrate a significant symptomatic relief of rhinitis and conjunctivitis in the treated group, compared with the control group (P〈0.0001), and an improvement of the respiratory symptoms. Symptomatic improvement was already apparent at the onset of the pollen season and persisted during the following weeks. An appreciable reduction in the consumption of antihistamines could also be shown (P〈0.0001). Furthermore, a lesser increase in the total IgE level and in the specific cereal and grass pollen RAST could be demonstrated. This treatment can be seriously considered for the management of severe pollen allergy, particularly in those cases that show a poor response to immunotherapy.

    Notes: The degree of bronchoconstriction after acetylcholine challenge in twenty patients with extrinsic asthma was diminished significantly after 3 months' treatment with 1 mg ketotifen b.i.d. Since ketotifen has no appreciable anticholinergic effect this observation is an indication that ketotifen decreases the non-specific bronchial hyperreactivity when administered over 3 months.
