    Notes: Twenty-three adult patients with a columnar lined (Barrett's) oesophagus are being treated with long-term omeprazole, 40 mg daily. Twelve had never undergone anti-reflux surgery (Group 1), the other eleven having previously had insertion of an Angelchik anti-reflux prosthesis (Group 2). Endoscopy was carried out six months before, immediately before and six months, one year and two years into treatment. Multiple and standardized biopsies were taken at each endoscopy. Results from the two groups were similar. During the 6-month run-in period there was a statistically non-significant increase in the linear extent of the columnar mucosa, but this showed a progressive, statistically significant decrease during the two years of treatment. Other evidence for regression of the Barrett's mucosa includes the emergence of large numbers of macroscopic squamous islands within the abnormal mucosa, an increase in the number of microscopic squamous islands, and microscopic squamous encroachment of the abnormal mucosa at the squamocolumnar junction. Histological assessment showed a reduction in the proportion of sulphomucin-rich intestinal metaplasia, but this only achieved statistical significance in Group 1. The results substantiate the importance of acid in the pathogenesis of Barrett's oesophagus. Omeprazole may have a therapeutic role in bringing about regression of the metaplastic epithelium.

    Notes: Ondansetron (GR38032F) is a selective antagonist of 5-hydroxytryptamine (5-HT) type 3 receptors. This randomized, doubleblind, cross-over study was undertaken to evaluate and compare the effect of ondansetron with placebo on gastrointestinal transit in 10 healthy male volunteers. There were no significant differences between the effects of placebo and ondansetron on gastric emptying or mouth-to-caecum transit time. However, significant differences in mean whole-gut transit time were observed, that is 54.8 h with ondansetron and 32.1 h with placebo. Therefore, 5-HT3 receptors may be involved in the regulation of colonic transit and ondansetron may prove useful as an anti-diarrhoeal agent.

    Notes: Abstract Background:Standard therapy for lymphoma consists of acyclophosphamide (C), doxorubicin, vincristine (V), and prednisone (P) (CHOP)combination regimen. Liposomal daunorubicin (DaunoXome®) is an alternativeto doxorubicin for patients with lymphoma because of its more favorable safetyprofile and potentially more selective uptake in lymphoma. The objectives ofthis study were to determine the maximum tolerated dose (MTD) of liposomaldaunorubucin with CVP (COP-X) and the tolerability of the regimen in patientswith indolent lymphoma. Patients and methods:Patients with low-grade andintermediate-grade lymphoma having adequate cardiac, hepatic, and renalfunction were enrolled. Patients received C 750 mg/m2, V 1.4mg/m2 (maximum 2.0 mg), and liposomal daunorubicin 50–100mg/m2 i.v. on day 1 and P 100 mg p.o. on days 1–5. MTD wasthe liposomal daunorubicin dose associated with 20% dose-limitingtoxicity (ANC 〈500/mm3 for 〉5 days or febrile neutropenia). Results:Twenty patients, median age 59 years, were treated. Theliposomal daunorubicin MTD combined with CVP was 70–80 mg/m2,depending on patient population. No significant non-hematologic toxicityoccurred. Response rate was 44% (2 complete and 5 partial responses). Conclusions:A liposomal daunorubicin dose of 80 mg/m2in the COP-X regimen was well tolerated with little non-hematologic toxicity.

    Notes: Summary Electrolytes and phospholipids of cartilage fractions were partitioned by extraction with organic and aqueous solvents into six solubility groups: Electrolytes I, II and III, and Lipids I, II and III. Of the total Ca, only 4% was water soluble (Electrolytes I); 4–12% was complexed with lipids (Electrolytes II); while the majority (84–92%) was insoluble (Electrolytes III). In contrast, nearly half of the Mg and Pi were water soluble. Of theneutral phospholipid, 95% was not complexed with mineral ions (Lipids I), but 30–45% of theacidic phospholipid was (Lipids II). Ca/Pi ratios were extremely low in the water-soluble phase, but were in the range of amorphous calcium phosphate (ACP) in the insoluble. Molar ratios of the lipid-mineral complex were: Ca∶Mg∶Pi:acidic phospholipid, 4∶3∶2∶2. Mg/Ca ratios in the soluble fraction were high (5.5–8.9), sufficient to stabilize ACP. Kinetic studies revealed rapid turnover of soluble Ca, insoluble turning over much more slowly. Labeling of lipid-complexed Ca was rapid in cells, but occurred later in matrix vesicles, suggesting transfer. While lipid-Ca-Pi complexes can nucleate apatite in vitro, those present in vivo inside matrix vesicles apparently do not because of the excess Mg. We conclude therefore, that in vesicle-mediated calcification, lysis of the membrane may be essential to allow release of internal Mg.

    Description / Table of Contents: Résumé Dans ce papier nous décrivons un aspect endoscopique du colon sigmoïde caractérisé par une enflure de la muqueuse, un érythème et des hémorragies strictements localisées aux plis muqueux en croissant. Durant une période de trois ans ces modifications ont été retrouvées chez 34 (1,42%) de 2380 coloscopies ou sigmoïdoscopies. La majorité des patients étaient d'un age moyen ou âgé. Une maladie diverticulaire était présente chez la plupart d'entre eux (82%) mais les anomalies étaient localisées aux plis muqueux en croissant épargnant les orifices diverticulaires. La majorité des patients présentaient une histoire d'hémorragie par l'anus. Histologiquement il y avait une variété de modifications allant d'une congestion vasculaire mineure à une inflammation active avec des anomalies architecturales décrites simulant la colite ulcéreuse, mais les biopsies rectales étaient invariablement normales. Trois malades ont fait par la suite une colite ulcérative distale typique et deux autres se présentant à nous avec une maladie endoscopique des plis en croissant avaient une préalable histoire documentée histologiquement de colite ulcérative distale. Ches trois patients les aspects histologiques étaient ceux d'un prolapsus muquene environ la moitié des patients ont en besain d'une thérapié quelconque pour contrôler leurs symptomes. Les stéroïdes et/ou la sulfasalazine fûrent actifs bien que deux patients subirent par la suite une résection sigmoïdienne, l'un pour contrôler une hémorragie et le second pour une sténose diverticulaire associée. Tandis que la maladie endoscopique des plis en croissant représente un aspect spécifique de l'endoscopie, les aspects cliniques et histologiques révèlent une large variété de maladies.
    Notes: Abstract In this paper we describe an endoscopic appearance of the sigmoid colon characterised by mucosal swelling, erythema and haemorrhage strictly localised to the crescentic mucosal folds. In a 5-year period these changes were seen in 34 (1.42%) of 2380 colonoscopies and fibreoptic sigmoidoscopies. The majority of patients were middle-aged or elderly. Diverticular disease was present in most (82%) but the abnormalities were confined to the crescentic mucosal folds with sparing of the diverticular orifices. The majority of patients presented with a history of bleeding per anum. Histologically there was a spectrum of changes varying from minor vascular congestion to florid active inflammatory disease with crypt architectural abnormalities mimicking ulcerative colitis, but rectal biopsies were invariably normal. Three patients later progressed to typical distal ulcerative colitis and two other patients presenting to us with endoscopic crescentic fold disease had a previous histologically documented history of distal ulcerative colitis. In three patients the histological features were of mucosal prolapse. About half the patients required some form of therapy to control their symptoms. Steroids and/or sulphasalazine were of value although two patients subsequently underwent sigmoid resection, one to control bleeding and the second for a diverticulosis-associated stricture. Whilst endoscopic crescentic fold disease represents a specific endoscopic appearance the clinical and histological features indicate a wide spectrum of disease.

    Notes: Abstract Quantifying a composite sample results in a loss of information on the values of the constituent individual samples. As a consequence of this information loss, it is impossible to identify individual samples having large values, based on composite sample measurements alone. However, under certain circumstances, it is possible to identify individual samples having large values without exhaustively measuring all individual samples. In addition to composite sample measurements, a few additional measurements on carefully selected individual samples are sufficient to identify the individual samples having large values. In this paper, we present a statistical method to recover extremely large individual sample values using composite sample measurements. An application to site characterization is used to illustrate the method.

    Notes: Abstract Composite sampling techniques are getting recognition as means of achieving observational economy when the cost of measurement is high. Accordingly, researchers in different disciplines are investigating composite sample techniques in a variety of applications. One can find applications of composite sampling in as diverse areas as soil sampling, water quality monitoring, bioaccumulation monitoring, infection rates, disease transmission rates, multiaccess communications and many more. Although it is very difficult for a researcher in any discipline to be acquainted with the results on composite sampling applied in other disciplines, it is also important for every researcher to be aware of any such development. An annotated bibliography is expected to serve the purpose of creating a common ground for researchers in different areas where composite sampling is of interest. The annotations are kept as brief as possible, but at the same time as descriptive of the annotated articles as desirable. The bibliography is classified by topics to make it easy to search for the topic of interest, and within a topic, the articles are arranged chronologically to give some idea of the development in this subject area during the last 50 years.
