    Notizen: The new concept for the design of a neutron time of flight spectrometer operating at high rates of events has been studied based on instrumental modeling and neutron transport calculations. The aim is to find an optimized design for an instrument that can measure the 2.5 MeV neutron emission from deuterium plasmas and attain diagnostics capabilities comparable to those as has been demonstrated possible with the 14 MeV magnetic proton recoil spectrometer for deuterium–tritium plasmas at Joint European Torus. Preliminary results are presented and used to project the potential for realizing an advanced neutron emission spectrometry diagnosis of plasmas. © 2001 American Institute of Physics.

    Notizen: The principles and operation at Joint European Torus (JET) of the first magnetic proton recoil (MPR) spectrometer for measurement of fusion neutron spectra are described. Some technical aspects of the instrument are discussed, including energy calibration and monitoring system. Data from the recent experimental campaign with tritium at JET are presented as examples of the MPR's diagnostic abilities. It is shown that the spectrometer is a flexible and versatile instrument, capable of delivering high-quality data on a number of plasma parameters. In particular, the observation of the so-called α knock-on effect in neutron emission spectroscopy is discussed. © 2001 American Institute of Physics.

    Notizen: The energy spectrum of the DT neutron emission was measured in experiments carried out at JET for plasmas of deuterium–tritium heated with different ion cyclotron resonance heating (ICRH) schemes. The spectrum was measured with the magnetic proton recoil spectrometer. The data were analyzed in up to three spectral components, which were ascribed to the underlying reactions involving thermal, epithermal, and high-energy ions. The results include time resolved information for individual discharges. High-energy components in the fuel ion population were found in most cases and analyzed in terms of a tail temperature. Changes in the (minority) deuterium concentration in the range of 12–20% were found to affect significant the ICRH power absorption and its perturbation of the minority velocity distribution. © 2001 American Institute of Physics.

    Notizen: Summary Metformin has been demonstrated to lower blood glucose in vivo by a mechanism which increases peripheral glucose uptake. Furthermore, the therapeutic concentration of metformin has been estimated to be in the order of 0.01 mmol/l. We investigated the effect of metformin on insulin-stimulated 3-0-methylglucose transport in isolated skeletal muscle obtained from seven patients with non-insulin-dependent diabetes mellitus (NIDDM) and from eight healthy subjects. Whole body insulin-mediated glucose utilization was decreased by 45 % (p 〈 0.05) in the diabetic subjects when studied at 8 mmol/l glucose, compared to the healthy subjects studied at 5 mmol/l glucose. Metformin, at concentrations of 0.1 and 0.01 mmol/l, had no effect on basal or insulin-stimulated (100 μU/ml) glucose transport in muscle strips from either of the groups. However, the two control subjects and three patients with NIDDM which displayed a low rate of insulin-mediated glucose utilization (〈 20 μmol · kg−1· min−1), as well as in vitro insulin resistance, demonstrated increased insulin-stimulated glucose transport in the presence of metformin at 0.1 mmol/l (p 〈 0.05). In conclusion, the concentration of metformin resulting in a potentiating effect on insulin-stimulated glucose transport in insulin-resistant human skeletal muscle is 10-fold higher than the therapeutic concentrations administered to patients with NIDDM. Thus, it is conceivable that the hypoglycaemic effect of metformin in vivo may be due to an accumulation of the drug in the extracellular space of skeletal muscle, or to an effect of the drug distal to the glucose transport step.     Notizen: Summary Short-term administration of physiological amounts of C-peptide to patients with insulin-dependent diabetes was found to reduce the glomerular hyperfiltration in these patients as well as augment whole body glucose utilization. It could also be shown that C-peptide administration increases blood flow, oxygen uptake and capillary diffusion capacity of exercising forearm muscle in IDDM patients, probably by increasing capillary recruitment in the working muscle. Studies under in vitro conditions have shown that C-peptide stimulates glucose transport in skeletal muscle with its maximal effect within the physiological concentration range. The findings in a clinical study in which IDDM patients were given C-peptide and insulin or insulin alone for 4 weeks in a double-blind randomized study design, indicate that C-peptide improves renal function by reducing urinary albumin excretion and glomerular filtration, decreases blood retinal barrier leakage and improves metabolic control. Preliminary findings suggest that C-peptide administration on a short-term basis (3 h) may ameliorate autonomic neuropathy by restoring to near normal the heart rate variability in response to expiration and inspiration. Insight into a possible mechanism of action of C-peptide is provided by the finding that C-peptide stimulates Na+K+-ATPase activity in renal tubular segments. In conclusion, the present results suggest that, contrary to the prevailing view, C-peptide possesses important physiological effects.

    Notizen: Summary No objective evidence has been presented to support the beneficial effect of physical training on glycaemic control in Type 1 (insulin-dependent) diabetic patients trained two to three times a week for several months. In the present study we examined the possibility that a daily exercise programme would be more suitable for improving glycaemic control. Thirteen patients completed a 5-month study; 6 were randomized to exercise training (20 min daily bicycle exercise) and 7 served as non-exercising controls. The training resulted in an 8% increase in maximal oxygen uptake (p 〈 0.05). No change in glycaemic control occurred during the study period in either group. In addition, serum lipid and lipoprotein levels were followed. Total cholesterol decreased during the study period irrespective of training. No effect was noted on the levels of LDL, VLDL, HDL and HDL2 cholesterol. A significant training effect was obtained in the HDL3 subfraction (−10%,p 〈 0.05). Total triglycerides were unchanged, but a decrease in the level of LDL triglycerides was observed with training (−12%,p 〈 0.01). It is concluded that, in female Type 1 diabetic patients, daily physical training for several months does not improve glycaemic control and results only in minor changes in serum lipoprotein profiles.
